SAMH manages the Activity Hub, where psychosocial rehabilitative programmes are conducted to assist residents in their recovery from mental illness and to reintegrating them into the community. Activity Hub’s services include occupational therapy, social work, psychological services, and vocational placements.

Purpose

Promote residents’ quality of life and reintegration to the community by focusing on the areas of vocational, activities of daily living and leisure activities

Core Duties:

1. Clinical Work

• Conduct therapeutic group work and track outcomes and collect data to ensure effectiveness
• Conduct individual / group sessions for skills training, psychoeducation and support (such as budgeting/community resources/conflict resolution, etc)
• Conduct home visits where necessary
• Conduct needs analysis and expand the types of volunteer opportunities for residents
• Work collaboratively with OTs / Psychologists / Vocational Placement Officers towards developing intervention and discharged plans for ICPs
• Make referrals to other services when appropriate
• Share new practice knowledge with other to encourage a culture of learning and continuous improvements

For Senior position:

• Lead and conduct case assessment and risk assessment for complex cases with high risk
• Ensure the case work management meet the requirement of MSF
• Review social reports and other documents prepared by social/case workers to ensure quality standard
• Develop / Fine tune service standards, performance indicators and operating guidelines of the department in consultation with RO
• Provide supervision to team on regular basis in their professional development
• Supervise NUS / SUSS social work students on placement

2. Administrative

• Ensure proper maintenance, collation and timely submission of statistics / period reports / annual reports to funding bodies, external agency and SAMH management
• Attend and present regular case conferences and case discussions (eg. ICP)

For Senior position:

• Lead, design and conduct research on current programmes / specified issues

3. Others

• Assist in daily operations eg. Incident reporting, crisis management, sending and receiving residents, business contingency plan
• Organize social and recreational activities to improve the quality of life of the residents

Mindset Learning Hub (MLH) aims to provide Workforce Skills Qualifications (WSQ) training and employment placement, in industries such as cleaning, food and beverage, hospitality, healthcare

support and retail, for Persons-In-Recovery.

Responsibilities:

Case Management

· Manage cases and perform case intervention and management to persons with mental health issues or those with multiple disability

· Conduct referral and pre-intake discussions with stakeholders that include hospitals and other social service agencies

· Conduct intake assessments for information gathering for assessment of suitability or make relevant referrals

· Develop and monitor individualised vocational plans and keep proper documentation of session records

· Provide psychoeducation to enable mental illness management

· Provide service linkage and referrals for other supports, when required

· Participate in regular case presentations and discussions within the team, and case conferences with hospitals and other agencies

Purpose

Reporting to the Head of Mobile Support Team, the Assistant Manager maintains an oversight of the planning of both COMIT services, programme objectives, implementation and service delivery of overall COMIT programmes.

He/She is in-charge of the review of service manual/work processes which impacts referral flow of clients as well as on budget planning and utilization based on approved budget. He/she also monitors and reviews the progress of the COMIT services performance.

He/She works closely with the Programme Head and team members to discuss and review new and existing guidelines and procedures for clinical issues within COMIT.

The job holder is accountable for COMIT team results, clinical and administrative matters.

Core Duties:

• Lead two COMIT teams of clinical staff to ensure smooth running of the programme’s services
• Responsible for proper maintenance, collation and submission of statistics to funding bodies and management as required
• Provide staff supervision and facilitate the development of staff’s growth with agency

Oasis Day Centre (ODC) is a psychiatric rehabilitation programme for people in recovery of mental illness or dual diagnosis (i.e., mild developmental disorders and mental illness). The main objective is to optimise clients’ level of function in the community, so as to cultivate independence, enhance interpersonal skills, and improve job readiness.

Purpose:

As a Social Work Associate, you are required to provide emotional and practical support to clients and their caregivers in order to help clients achieving their goals and improve their lives.

Responsibilities:

· Assist the Intake and discharge processes of clients

· Support Social Worker and Case Worker in clients’ progress and intervention plans

· Provide family support and intervention to facilitate clients’ attainment of goals

· Attend to disputes between clients and emergency situations

· Support groupwork, psychoeducation workshops and other therapeutic sessions

· Assist Social Worker and Case Worker in social reports, incident reports, case recordings, and letters to agencies and caregivers.
