Main duties and responsibilities In liaison with the Lead Clinician, manage the UK Pulmonary Hypertension Service. To be responsible for organising six-monthly meetings between the GOSH team and the responsible Consultants in the principle collaborating Trusts. To attend the weekly multi-disciplinary meetings as the unit administrator of the UK Pulmonary Hypertension Service for Children. To take the minutes at these meetings and distribute to all parties involved, within the Trust and external agencies. To be responsible for maintaining diaries and arranging meetings as required by the Pulmonary Hypertension Team. Administration/Secretarial Duties To provide the full range of secretarial support to the Pulmonary Hypertension Team. Such support includes accurate and efficient typing of letters, correspondence and reports, presentations, appointments and waiting list, and general filing. To ensure all referral letters are processed in a timely manner. To be responsible for the organisation of the outreach clinics undertaken by the Consultant and any other secretarial work resulting from these clinics.Working alongside the Cardio-respiratory booking office, maintain the Pulmonary Hypertension waiting list on EPIC, ensuring the list is regularly validated and accurately reflects demand. To arrange new and follow-up appointments and arrange transport as necessary. To liaise with relevant departments regarding investigations and results.To cancel clinics and complete appropriate clinic forms and advise all parties. To organise peripheral clinics including arranging clinic dates, making appointments, filing and arranging admissions for tests/surgery and maintain the peripheral clinics spreadsheet for clinics held by the Consultant. To oversee departmental projects and implement any new changes required, ensuring staff in the unit are aware of the changes. To be responsible for routine administration, handling post and ensuring that computer and other office equipment remains in working order and is appropriate to the needs of the office. To ensure the accurate and appropriate use of the EPIC system to register, make appointments for and code patients. To work with the Cardio-Respiratory Management Team in line with the Modernisation and NHS Plan in relation to outpatient waiting times ensuring all patients are seen within agreed targets. CommunicationIn liaison with clinical staff, establish appropriate working patterns in order to provide an optimum secretarial service and communicate any changes to appropriate staff. To establish close working relationships with other clinical units in Great Ormond Street Hospital, other Trusts, external agencies to ensure a cohesive and efficient service. Maintain absolute confidentiality regarding patient information to all parties unrelated to their medical care. Appropriately receive, re-direct and make telephone calls to and from the department as required and ensure, where possible, that telephone calls are diverted to an appropriate person if the office or department is unmanned. To monitor queries and ensure that they are responded to in a timely fashion. To ensure that visitors and enquiries to the ZCR office, in person or by telephone, are handled courteously, efficiently and with sensitivity (e.g. with regard to bereaved parents) and that they receive accurate information and appropriate advice. General Administration Duties To know how accurate and timely data collection helps the Trust deliver patient care now and in the future. To help your team meet its data quality targets by: using data reports to correct errors and be aware of ongoing performance. Errors/omissions identified in data quality reports should be corrected within a month, for those data items where it is possible to do so referring to the EPIC business procedures, data dictionary and user guides, all available on the Great Ormond Street Web. To adhere to the Trusts service agreement procedures and manage the processing of referrals and admissions to ensure that all patient episodes are covered by a service agreement or overseas visitor authorisation. Yorkshire Ambulance Service (YAS) is expanding its clinical workforce by introducing a new Specialist Paramedic Mental Health(SPMH) role open to suitably experienced Paramedics.

The MHRVs attend to specific mental health jobs where a double-crewed ambulance and transport to the ED may not be required as there is no physical injury. Patients are transported to safe spaces and put in touch with crisis teams to access support and Mental Health Assessment.

The successful candidates will join the team as a Paramedic in Mental Health and then start their PG Dip at Sheffield Hallam University (SHU). On completion they will become a Specialist Paramedic in Mental Health.

You will rotate between working as a double crew on the Mental Health Response Vehicles (MHRV) and Emergency Operations Centre (EOC) including placements in local mental health crisis teams, safe spaces, primary care and other providers. This is a leadership role that works across the 4 pillars of specialist and advanced practice.

Specialist Paramedic Mental Health (SPMH) are level 7 practitioners and can work to the full scope of practice. Paramedic Mental Health (PMH) is the title used during development and training as the clinician transitions to specialist practice. Paramedic Mental Health is a non-substantive role and clinicians are required to successfully complete the specialist practice pathway.

Working with a greater autonomy, the post holder will provide enhanced clinical management and coordinate the care of patients with complex and challenging healthcare needs, responding to patients with acute or critical illness, serious injury or major trauma in emotive and demanding situations or environments. The Specialist Paramedic Mental Health will support patients experiencing mental health crisis when they need a response (either telephone or face to face from the ambulance service).

Utilising their clinical skills and knowledge required of the role, the post holder is responsible for the competent assessment, diagnosis, care, treatment and referral and/or transport of patients and their carers/relatives in emergency or life-threatening situations.

The post holder will be clinically responsible for ensuring their own safe and effective clinical performance, and demonstrate excellence in clinical leadership through role modelling values and professional standards and mentorship in practice.

Emergency Department and Front Door Services Work with the lead consultants, Doctors, ACP and nursing teams ED, A&E obs, Ambulatory. To review, develop and maintain policies, guidelines and procedures to ensure safe and effective supply, storage, prescribing, administration and monitoring of medication. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and Health Care Commission Requirements. As an expert practitioner, provide support and advice trust-wide to medical staff initiating, maintaining and monitoring patients in ED, encouraging adherence to Trust acute medicine PGDs/guidelines/ policy. Ensure accurate transfer of necessary information to GPs to enable continuation of treatment and prevent re-admission to hospital, including meeting all relevant requirements of the Area Prescribing Committee. Proactively work with prescribers to ensure safe prescribing within the ED and practice is embedded across the Trust. To the management of End of Life and Palliative patients which come through the ED to support the specialist palliative care team. Lead, develop and deliver education and training to junior doctors, nurses and pharmacists regarding prescribing, administration and monitoring. Audit and monitor practice against national standards and Trust guidance to provide assurance to the Medication Safety Committee with regards to safe use of medicine within ED setting across the Trust. Educate and counsel patients regarding their therapies. To develop and provide a comprehensive clinical pharmacy service for the patients in your areas a SWFT, attending ward rounds and multidisciplinary team meetings as required. General Clinical Service To liaise with hospital colleagues and contribute to the provision of clinical pharmacy services at SWFT. To contribute to directorate work for all medical specialities. To advise on / recommend / adapt clinical treatment guidelines that have been produced locally or by national bodies e.g. NICE, NSFs, National Prescribing Centre, for use in the specialty area. To be a lead pharmacist in advising the Principal Pharmacist, and the Head of Pharmacy, on the strategic development of clinical pharmacy services to the designated specialist services. Work with the ward based and dispensary pharmacy teams in the development of and maintenance of the Trusts pharmacy services. To ensure that safe systems of handling medicines are in place and that national guidelines and local protocols relating to medicines management are adhered to. To advise medical, nursing, other professional staff, managers and patients on any aspect of drug therapy or drug control as required or when considered necessary. To maintain an awareness of current developments in pharmacy practice pertinent to the services, by liaising with relevant bodies, outside agencies, special interest groups and other pharmacists. To inform and advise senior pharmacy staff as appropriate. Governance To work directly with the medicine Safety officer and principle pharmacist in addressing the pharmacy and medicines management governance agenda within the Trust relevant to their designated specialty. To represent Pharmacy on the Audit and Operational Governance group of the relevant division, in the postholders own right, or as alternate. To undertake the reporting of medication incidents when necessary, according to the Trusts Incident Reporting Policy, to assist other pharmacists in reporting, to investigate medication incidents as necessary, to provide senior pharmacist input to incident report forms and root cause analyses, and to recommend improvements and implement agreed actions. As a registered practising Pharmacist with the General Pharmaceutical Council, the post-holder must undertake Continuing Professional Development CPD and meet the mandatory requirements. Operational Pharmacy To contribute to duties as a clinical pharmacist to other wards in the Trust when necessary. Supporting the assessment of medicines management competencies. To deputize for senior pharmacy managers as required. To liaise with the Senior Pharmacists for the allocation of staff, notably to negotiate staffing resource to provide cover of the specialist services in the postholders absence. To participate in the provision of the total pharmaceutical service, including a contribution to dispensary rotas on weekdays, late duties, and public holidays. The department provides a 7 day pharmacy service and the post holder will be expected to work weekends on a regular basis. To participate in oncall service depending on service needs. To undertake routine duties within the dispensary as required, including supervising the work of junior pharmacists, pharmacy technicians and (senior) ATOs. To assure the standards of pharmaceutical care provided to patients on designated wards, ensuring that a satisfactory level of pharmacy service is provided within the resources available. Education and Training The post holder will support the pharmacy departments aspirations in achieving and maintaining the highest possible standards of clinical pharmacy excellence. If agreed, to act as a Pre-Registration Pharmacist tutor, or as a clinical coordinator for a post-graduate diploma student. To provide clinical education and training to medical, nursing and pharmacy staff, both undergraduate and postgraduate, including tutoring and mentoring. To supervise, contribute to and undertake research and audit projects in the specialty areas, particularly those concerned with the clinical and cost effective use of drugs, compliance with safer prescribing and drug administration policies, procedures and guidelines, and implementation of pharmacy service developments. To provide reports and present the audit findings within the Trust or to outside organizations or regional or national meetings where appropriate. To support the Education and Training Lead Pharmacist to develop and maintain a training Programme for clinical pharmacists allocated to the specialty, and to develop and administer a competency assessment process to ensure the appropriate standards are achieved. To provide clinical pharmacy training for undergraduate pharmacy students, post-grad. diploma students, vacation students, pre-registration pharmacists (hospital and community-based) and hospital clinical pharmacists. Research The post holder may undertake practice research and will support research and audit by students and others within the department. Managerial Responsibilities To provide short-term cover for colleagues during periods of leave. • Manage a clinical caseload of patients with complex mental health needs
