Keystone Mental Health Services is currently seeking a Program Coordinator to join our team in making a positive difference in the lives of our neighbors and the community.

The Work:

• Provide quality supports to adults with mental illness in a residential home
• Act as a role model and mentor to staff and assist the Program Supervisor to monitor aspects of program operations as needed
• Assist individuals to become more independent, allowing each person in our services to take a lead role in their own rehabilitation process
• Provide personal care, administer medications, and mentor individuals following their treatment and support plans
• Assist with cooking, cleaning, and other household responsibilities.
• Plan meaningful recreational activities and take individuals into the community and to appointments as needed
• Complete daily documentation as required
• Participate in on-call coverage as scheduled

Minimum Requirements:

• High school diploma or equivalent
• 3-6 months experience working in human services or related field
• 21 years of age or older
• Ability to learn and apply proficient computer skills, including the use of software and web based system
• Effective written and verbal communication skills; ability to read, interpret and apply policies and regulations
• Ability to obtain Program Administrator certification within 6 months of hire
• Valid driver's license and daily access to a properly insured vehicle
• Successful completion of the pre-employment process, including motor vehicle record and criminal background checks

Job Summary

This Position Is Responsible For Contacting Members And Community Partners In Assigned Areas And Is Responsible For Performing Ongoing Functions To Support The Social Service Department. Responsible For Contacting Identified Members To Inform And Educate Them On Health Care Programs, Responding To Inquiries From Members, And Supporting The Clinicians In The Physical And Behavioral Health Management Department With Their Provider And Member Activities. This Position Is Also Responsible For Developing Relationships With Community Partners, Working With Community Leaders And Contributing To The Design Of Community Health Outreach Programs. Also Assists As Needed In Identifying Local Social Service Resources For Members.

JOB REQUIREMENTS:
* Bachelor Degree OR 4 years’ health, social services or community advocacy experience,
* Knowledge of medical terminology
* Experience coordinating member related needs, providing assistance to members, and analyzing member needs
* PC proficiency including Microsoft Office applications
* Customer service skills
* Verbal and written communications skills including developing written correspondence to members and to other department personnel and coaching skills, including motivational interviewing, to educate members on medical and BH issues

PREFERRED JOB REQUIREMENTS:
* Bachelor’s degree in Social Work
* Community Health Worker (CHW) certification

Mental Health Coordinator We have an exciting opportunity for an individual to join our Liverpool FC Foundation team as a Mental Health Coordinator. In this role you will oversee the day-to-day coordination of the On Target programme and a large element will involve delivery of educational workshops, within a range of environments including secondary schools. What will you be doing?

• Work closely with the Mental Health Manager to establish strong and lasting relationships with educational providers, key stakeholders and partners.

Who are we looking for? To be successful in this role you will have proven experience working with children and young people in a youth work or school setting. You will understand the issues around mental health and well being. You’ll have a youth work and/or mental health qualification, including experience delivering informal education workshops. You will be able to demonstrate excellent interpersonal skills, empathy and resilience, including excellent personal organisational skills. It is desired you have experience of delivering educational programmes in schools and delivering music or art-based workshops with children or young people.
