About the Role

As a Social Care Assistant, your role will be to assist qualified practitioners with a variety of tasks, including assessing people's needs and carrying out regular reviews. You will work in partnership with individuals, their carers, professional staff in other agencies and service providers to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

You will be responsible for the completion of Care Act Assessments to facilitate a smooth transition for service users when they leave hospital and for supporting discharges from intermediate care bed / inpatient rehab settings. A vacancy has arisen for an Assistant Social Care Worker within our small and friendly team at Eadar Glinn.

The successful candidate must have a clear understanding of how to meet the care needs of frail older people using an approach that is relationship and person centred.

The post holder must be prepared to work flexibly, following a four week rolling rota.

The post holder must comply with SSSC registration terms and conditions.

About the position: The Social Care Assistant is responsible for facilitating a range of individual, group and community based lifestyle activities according to residents’ individual physical, social, cultural and spiritual needs and preferences.

Key Responsibilities

• Document social profiles of new residents and participate in the development, implementation and evaluation of social care plans.
• Participate in the planning, implementation and evaluation of the activities program.
• Maintain resources for the activities program.
• Prepare newsletters, activity notices and other correspondence for residents and other representatives.
• Assist the General Manager with appropriate delegation and support for volunteers.

Essential:

• Certificate III or IV Qualification in aged care or similar discipline.
• Current National Police Certificate (or ability to acquire).
• Has empathy and sensitivity to the needs of the residents from a variety of cultural and social backgrounds.
• Ability to communicate effectively with the elderly and their families, employees and visitors.
• Experience in the aged care sector, especially as a Social Care Assistant or Leisure & Lifestyle Assistant.
