The Community Specialist Palliative Care Team is looking to recruit a Clinical Nurse Specialist with proven experience within specialist palliative care. The role will be varied and challenging, the service facilitates 7 day responsive visits and clinical triage as part of a Specialist Palliative Care Triage. This involves gatekeeping and initiating a clinical assessment at point of contact for all Specialist Palliative Care services.



The Post Holder will be expected to act as an autonomous practitioner providing complex Specialist Palliative Care symptom management and advice to support patients their families and carers. Specialist Palliative Care Services
The practitioner will manage an autonomous community caseload requiring specialist input for complex palliative care needs.

To act as an autonomous practitioner, providing complex specialist palliative care symptom management, education, advice, counselling, and support to community patients, their families and carers.

Provide clinical triage, adhering to the Specialist Palliative Care (SPC) referral criteria, on a rota basis to ensure patients, their families and carers with complex needs receive the most appropriate specialist input in a timely and responsive manner.

AUDIT/INFORMATION TECHNOLOGY/RESEARCH ACTIVITY To complete infection prevention and control audits to ensure standards are in compliance with national set standards. Formulates strategies with the Lead Infection prevention and Control Nurse to promote and facilitate the review of clinical practice in response to the results of surveillance/audit. To actively promote evidence-based practice in own and the practice of other members of staff involved in the specialist care of the patient/client.
