Key duties

• Reviews patients periodically to assess their progress and to ensure that high quality care has been provided and to ascertain patients’ needs for additional care / services

The Emergency Department at Croydon University Hospital (CUH) is one of the busiest in London and is a designated Trauma Unit, part of the South West London and Surrey Trauma Network. This post is part of an expansion of the clinical workforce delivering Emergency Medicine including middle grades and consultants. We are increasing our Junior Clinical Fellow posts up to 15 as part of a trainee and trust post roster of 21 at the level of ST1-3. The Emergency Department is part of the Emergency Clinical Business Unit alongside the Acute Medical service for Adults. Emergency, alongside Specialist Medicine and Elderly Care and Community Services Clinical Business Units makes up the Directorate of Integrated Adult Care (IAC). Collectively IAC is responsible for the clinical services for urgent and emergency pathways and adult inpatient medical and community services and therapies.



Whilst Paediatric Emergency Care is partnered with adult Emergency services, the specialty Paediatric Care Services are organisationally arranged within the Integrated Women’s and Children’s Service Directorate. It is aimed at doctors who have completed their Foundation training (or equivalent) with and interest in Emergency medicine through to those looking to undertake their MRCEM or those who can demonstrate the equivalent competencies. This is full time APP position with the Surgical Associates provider group (general surgery) working specifically with the Acute Care Surgery (ACS) service. Our ACS service is an inpatient based service, providing care to emergency general surgery and trauma patients. The APP will work closely with the ACS team consisting of the on call general surgeon, APPs, and General Surgery residents. This is inpatient, shift based work – shifts run from 6am-4pm; the APP will work 4, 10 hour shifts in a Monday – Friday timeframe. This is an APP2 position, meaning there will be a weekend and holiday requirement – this will be further discussed through the interview process. Experience preferred, but not required. Summary

The Licensed Practical Nurse (LPN) will participate in the implementation and evaluation of patient care, under the supervision of a registered nurse, advanced practice provider, or physician.

The Medical Assistant (MA) functions within the administration pre-defined scope of practice guidelines per state location of practice.

Job Description

The LPN ensures the health, comfort and safety of patients. Documents a thorough medical history from patient. Contributes to the assessment of patients and administers medications or treatments as ordered. Provides technical support to healthcare professionals as needed. Counseling patients and family members, under the direction of a registered nurse, on prevention and treatment plans.

The LPN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting.

The MA in the ambulatory care setting collects subjective and objective health status data from the patient or caregiver and communicates this data to the health care provider. Participates in the care of patients by providing contributing data to licensed health care professionals; and following through on the patient's plan of care under the direction of the provider.

Communicates the provider's written instructions for care to the patient, or caregiver, by transmission of specifically defined information. Will participate in care for patients, across the lifespan, in all phases of preventative care, health maintenance, treatment, and follow-up as patients move in and out of care settings.

Performs a variety of duties under the direct supervision of a person licensed to practice medicine. A sampling of the specialized clinical duties performed includes, but is not limited to, obtaining vital signs, preparing patients for examinations, observing and reporting patient's signs or symptoms, and performing point of care testing.

Qualifications

The LPN is a graduate from an accredited practical nursing program as a Licensed Practical Nurse.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications.

The MA must be a graduate of a recognized Medical Assistant program. Prior experience in a healthcare setting is preferred.  Nationally certified or registered as a Medical Assistant (MA).

Certification (CMA) obtained through a nationally approved certification program for medical assistant, including but not limited to: American Association of Medical Assistants (AAMA), Certified Clinical Medical Assistant (CCMA) through the National HealthCareer Association (NHA), Clinical Medical Assistant Certification (CMAC) through the American Medical Certification Association (AMCA), or National Certified Medical Assistant (NCMA) through the National Center for Competency Testing (NCCT), etc. Registration (RMA) obtained through the American Medical Technologists (AMT) also acceptable.

Additional state requirements include: North Dakota (ND) registration as an MAIII through the ND Board of Nursing. MAs working within Minnesota (MN), Iowa (IA), South Dakota (SD) and Oregon (OR) do not require state registration.

Basic Life Support (BLS) certification required within six months of employment. Re-certification as required.

Provides service to patients in acute, short and long term care settings by assisting physicians, assessing patients, and treating injuries and ailments. Obtains a thorough medical history from patient and dependent on patient population and setting will perform an examination and/or assessment to determine patient's needs. With other healthcare professionals, analyze and interpret information collected from patient, medical records, symptoms, physical findings, or diagnostic information, to develop and establish appropriate diagnosis. Acknowledges and demonstrates the importance of care coordination and navigation by rounding on patients when necessary. Works in collaboration with providers and independently to provide care. Formulates a plan for treatment of the patient and prescribes medications based on efficacy, safety, and cost as legally authorized if necessary, in order to aid recovery and manage pain. Maintain complete and detailed records of patients' health care plans and prognoses in a timely manner. Consult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise. Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability. Counsels' patients and family members about self-management on prevention and treatment plan for health issues, tailoring instructions to patients' individual circumstances. May cover hospital consultations, rounding, and assist with patient procedures as needed. Conducts research into area of specialty and uses findings to provide measurable improvements in patient care and clinical outcomes. Incorporates evidence based practice guidelines into care. Maintain current knowledge of state legal regulations for advanced practice provider practices, including reimbursement of services. Keep abreast of regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sources, as applicable. Demonstrate extensive advanced knowledge of medical principles, practices and techniques.

Qualifications

Completion of a master’s, postmaster’s, or doctorate from an nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission. Licensing prior to August 1, 1995, master's degree in nursing is preferred.

Demonstrated current competence and provision of care, treatment, or services for an adequate volume of patients in the past twelve months, or completion of master’s/post-master’s degree program in the past twelve months. Experience must correlate to the privileges requested.

Current licensure by the applicable state board of nursing for advanced practice. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances. Certified Family Nurse Practitioner (NP-C) or (FNP-BC). Obtains and subsequently maintains required department specific competencies and certifications.

This full-time post is aimed at junior surgical Specialty Trainees (or overseas equivalent) and is for a minimum period of 6 months extendable to 12 months.

This post will provide both specialist Oesophago-Gastric resectional surgery training (with an emphasis on laparoscopic surgery) and General Surgical Training.

1. On-call for general surgery at a 1:12 duty. When on-call the Fellow is expected to be contactable at all times. When on-call at night the Fellow is not expected to be resident but must be available to attend within 15 minutes.

2. A commitment to undergraduate and postgraduate medical and dental education for 2 hours per week.

3. Support the elective clinical activities of the Oesophago-gastric Surgical Unit
