This is an exciting opportunity to join a dynamic and motivated Pain Management Service, working cross site at Chelsea & Westminster and West Middlesex University Hospitals. This is an established service that provides highly specialised services to adult and paediatric patients with acute and chronic pain, within a multidisciplinary setting. The Clinical Nurse Specialist is expected to support all areas of the service, with support from the MDT.

The individual must demonstrate excellent communication skills, be a role model, enthusiastic, and be able to motivate and support individuals in the team, to enable the continuing and on-going development of the service. They will be responsible for providing education and assist in developing audit and research strategies for the service.

There is great scope in this role to develop both personally and professionally. We are looking for an individual with some experience of working within a pain management service who is looking for a developmental opportunity or a new challenge. This post is a 1 year fixed-term contract to cover Maternity Leave, however there may be other opportunities to join the team in the future.

The post holder will play a key role in improving the delivery of our service to meet the needs of our patients. This will be through working in both the inpatient and outpatient pain service. The role involves reviewing inpatients with acute and chronic pain. The outpatient role involves supporting the pain clinic, neuromodulation and capsaicin service. Additionally, conducting TENS, post procedure follow up and medication review clinics. This is an integrated role and you will work closely with the rest of the pain service, to allow for cross cover and support for the service. The service has developed as a multidisciplinary service involving physiotherapists, psychologists, neurosurgeons, neurologists, as well as pain physicians and the successful applicant will be a valuable part of this team.

The role of the Nurse Specialist includes:
• Receiving referrals
• Providing skilled assessment, planning, delivery and evaluation of specialist care within defined guidelines and protocols

Clinical Responsibilities
• To work as an autonomous practitioner, delivering expert nursing service for inpatients and outpatients. This includes participating in nurse-led follow up clinics for patients after pain related procedures, including telephone clinics and medication review clinics.
• Providing advice and support to patient and relatives during treatments or therapies such as spinal cord stimulators (SCS), percutaneous electrical nerve stimulation (PENS) and capsaicin treatment
• To manage and discharge patients within agreed department protocols and guidelines. To arrange appropriate follow up and discuss with the consultant as necessary
• To provide specialist advice and education to patients to ensure that they are able to make an informed choice about therapies and medicines
• To have a role in guiding neuromodulation services
• Accurately maintaining data bases for SCS, capsaicin and inpatient pain patients.
• Establishing and maintaining good working relationships with supporting clinical services.
• Participating in pain service multidisciplinary meetings
• Learn and participate in reprogramming SCS patients
• To have a role in coordinating theatre lists for patients requiring neuromodulation. Ensuring effective communication between administration staff and consultants.
• Responsibility for liaising with patients about their pain journey maintaining communication and answering their questions
• Being aware of side effects and efficacy of drugs and treatments involved, ensuring patients results are checked and abnormalities reported.
• Contacting patients when they are required to attend theatre for the SCS trial and implant
• Providing a high standard of nursing care to patients whilst working in close collaboration and cooperation with other members of the multidisciplinary team

Education
• To recognise areas for clinical development and respond appropriately to these, utilising available resources.

• Report any potential complaints or untoward incidents to the Lead Nurse and participate in the Trusts Risk management process

• Having a role in ordering specialist equipment e.g TENS machines and Neuromodulation kit
• Development of new initiatives to run cost effective services, within the constraints of budgets and resources

To participate in a high quality, patient focused Medicines Optimisation model for technicians, at ward and community team level. To monitor and supervise the work of the pharmacy assistants and participate in medication top-up at team base/ward level, to ensure good stock control. Job Description

Essential Functions:

• Review and abstract information in the medical record to accurately code for that episode of care.
• Code hospital and professional services by accurately assigning ICD-10, CPT and HCPSCS codes for appropriate billing.
• Apply knowledge of medical science, medical terminology, anatomy and physiology and the official coding guidelines to select the correct codes.

Clinical Data Coordinator

The Clinical Data Coordinator will coordinate activities focused on ensuring the accuracy and integrity of clinical data, as well as its compliance with regulatory standards and hospital revenue initiatives. Provide expertise in the areas of coding and classification systems to healthcare providers throughout our hospital, and help drive improvements in documentation, reimbursement and the revenue cycle. Job Summary

Responsible for providing comprehensive assessment, planning, implementation and overall evaluation of individual patient needs; Works collaborate with the Physicians, patient/family, nursing, utilization review and other members of the healthcare team to assure patient management  that efficiently and effectively aligns with patient needs using resources to meet quality, clinical and cost effective outcomes. Coordinates a team approach designed to facilitate the achievement of expected patient outcomes with appropriate transitions to the next level of care; Responsible for length of stay management, regulatory compliance, and attending/participating with interdisciplinary team rounds on assigned unit; Collaborates with community providers to facilitate and coordinate the plan of care for post-hospitalization needs of the patient. This position will come in contact with patients in the neonate, infant, child, adolescent, adult, and geriatric age groups; Employees will perform clinical duties in accordance with population specific guidelines and adhere to National Patient Safety Guidelines. Provides cross coverage for all RNCM as required across all settings in the care continuum, including weekend rotation (as needed). 
 

Minimum Job Qualifications

• Licensure or other certifications: Licensed to practice as an RN in Georgia. CCM (Case Management Certification) or ACM (American Case Management Certification) required within 3 years of date of hire.

• Educational Requirements: Associates Degree. Graduate of an accredited school of nursing. Eligibility requirements for certification: 12 months full-time case management experience supervised by a certified case manager, or 24 months full-time case management experience not supervised by a certified case manager. 

• Minimum Experience: Three (3) to five (5) years of experience in direct patient care and/or case management. Financial and discharge planning experience.

Essential Tasks and Responsibilities

• Communicates with Physician, patient/family, and other disciplines the expected length of stay, along with patient progress towards discharge.

• Provides coordination and facilitation oversight of patient care to assure required interventions occur in proper sequence and processes occur in a timely manner without delays. Identifies and acts upon potential delays in services; escalates unresolved delays to management for appropriate intervention. 

• Assess, coordinate and facilitate patient's discharge plan to assure post-acute needs are arranged and secured prior to discharge; Communicate discharge plan with Physician, patient/family, and other members of the healthcare team as appropriate; Reassess discharge plan routinely throughout patient's stay to ensure timely, safe discharge and appropriate transition to the next level of care. Provides patient/family with information regarding their plan of care, discharge and any financial responsibility of inpatient or post-hospitalization services.

• Maintain knowledge of reimbursement methodologies and general coverage guidelines for all levels of inpatient and outpatient care. Communicate with Physician, patient/family or other team members as needed to ensure services will be covered.

• Coordinate and communicate with Utilization Review Nurse on a daily, consistent basis to ensure patients are in the right status and level of care. Facilitate changes by communicating with Physician, mid-level or nursing staff as needed.

• Serve as liaison to patients family, Physicians, nursing staff and all other disciplines to achieve optimal outcomes in the development of patient's discharge plan. Serve as a leader on assigned unit in the areas of discharge planning, social service issues, community resources/referrals and financial information related to patient care and outcomes. Empowered to think outside of the box to consistently create new, and effective solutions to complex problems or opportunities. 

• Actively supports a customer service oriented environment to continually enhance customer service; Communicates directly with Physicians, nursing staff, patient/family and other disciplines to ensure collaborative practice. Provide appropriate hand-off communication as patients transition from one unit to another to ensure and achieve optimal outcomes.

• Maintains positive attitude, and communicates appropriately with patients/families, Physician, management and other staff; responds positively to change and offers suggestions to effectively incorporate change as needed in daily workflow.

• Maintain detailed knowledge of community resources, governmental regulations, third party payers (PPO/HMO's) to facilitate appropriate outcomes.

The post holder will manage a section of the management accounts team and work as part of a dynamic team in delivering an effective service supporting managers and staff across GM Mental Health services to understand, plan and manage their financial position. This will include the production of monthly reports, forecasting, creation of annual budgets, report-writing and liaising with a variety of stakeholders to ensure that risks and issues affecting financial performance are communicated clearly.

To manage and be fully responsible for a section of the Management Accounts team, this includes the Service Accountant and Management Accounts Assistant.

To prepare appropriate financial reports to set timelines with written analysis / commentaries for the various service line senior finance managers enabling them to manage their areas of the business, analysing reasons for under / over performance and ensuring that the likely full year outcome is well explained.
