Job Description

· Discharge the duties of a Workplace Safety and Health Officer (WSHO) in accordance to the Workplace Safety and Health (WSH) Act and the WSH (WSHO) Regulations.

· Develop/implement WSH management system and risk management plans pertaining to the research and general activities in the workplace.

• Conducts rounds in various clinical and non-clinical areas of the facility (including but not limited to inpatient wards, outpatient clinics, sterile processing service, sterile compounding areas, engineering plant, operative and procedural areas) to conduct visual inspections and tracers evaluating infection control and prevention practices, documents findings of these rounds and compiles, tracks, and trends these findings to use in presentations and recommendations to leadership on the effectiveness of the practices and any changes needed for processes, policies, or resources.
• Reviews electronic and material data and applies qualitative and quantitative analysis methods to the data in order to track and trend various types of infections including specific organisms (such as Methicillin Resistant Staphylococcus Aureus (MRSA) and Clostridium Difficile (C. Diff)) and types of infections (such as Surgical Site Infections (SSI), Central Line Associated Blood Stream Infections (CLABSI), and Catheter Associated Urinary Tract Infections (CAUTI)).
• Analyzes reports for infectious deviances and outliers, bringing such information to the attention to the Quality Manager and the Infection Control Committee as well as any other related committees (Facility Surgical Work Group, Executive Committee of the Medical Staff, Environment of Care Committee, Reusable Medical Equipment Committee for example), makes recommendations based on evidence and best practice to address outliers and deviances. Performs routine audits to ensure quality control and data integrity of surveillance data.
• Develops and maintains infection control statistical databases for the facility that are then used to inform recommendations on changes to policies, procedures, and resource utilization to improve infection control and prevention throughout the facility, develops interpretative graphs and other visual aids for reporting results of analyses performed; presents this work to service management, senior leadership, and management in other related services regularly as a part of meetings and committee standing and ad-hoc reports and makes recommendations based on this work to the committee or to leadership on changes to programs, policies, or resource utilization.
• Tracks, trends, analyzes, and reports hand hygiene compliance data to present at the director's morning report, infection control committee, and the Executive Committee of the Medical Staff monthly and makes recommendations based on findings to improve infection control and prevention practices throughout the facility.

As a Senior Social Worker, the incumbent must be able to perform at the expert level and work independently with limited supervision, including in rural and isolated areas of New Mexico.

The Senior Social Worker is assigned to Social Work Service and reports directly to the HUDVASH Supervisor. S/he is assigned to the Gallup, NM community and provides comprehensive social work services to veterans and their families. Responsibilities include the management, coordination and provision of social work services to veteran and their families in inpatient, outpatient and community settings.

• Overseeing all aspects of the HUD-VASH program with veterans served by the Gallup CBOC.
• Reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
• Establishing a process for referrals and admissions to the HUD-VASH Program, ensuring that homeless veterans are considered as primary participants for program acceptance.
• Performing site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes; verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
• Providing oversight of HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing.
• Coordinating care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the CBOC, VA Medical Center, VA Regional Offices and/or community agencies.
• Participating in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH; responsible for developing and administering local policy and procedures for the HUD-VASH Program.
• Maintaining policies and documentation as required for CARF and JCAHO accreditation; responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures to the HUD-VASH Program Coordinator.
• Participating in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
• Performing accountability functions: 1) document all contacts with patients/families in the medical charts; 2) completes statistical, administrative, staffing studies and other reports in accordance with VA Medical Center and respective professional discipline service policies.
• Participating in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and CBOC personnel on the biopsychosocial aspects of homelessness; takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
• Will conduct psychosocial assessments to determine needs of veterans and family members; actively participates in the treatment planning process with other disciplines; participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate; provides individual, family and group counseling as appropriate; will provide case management to veterans and families, as needed, throughout the continuum of care; will provide consultation/education to veterans and families on community resources, advance directives and VA benefits.
