These positions are based at the John Radcliffe and Churchill Hospitals based in Headington, Oxford. It is roughly three miles from the city centre, with exceptional public transport to and from the hospital as well as onsite parking. Key clinical responsibilities To manage a highly complex caseload of patients on the critical care unit and support outpatient clinics as appropriate, act as clinical lead and co-ordinate service improvement projects in this area. To manage a highly complex caseload of patients requiring parenteral nutrition, assisting in service improvement projects in this area. To provide leadership across the critical care and medical team working to ensure the delivery of an effective, high quality, evidence based dietetics service. This will involve working collaboratively with the gastroenterology and surgical dietetics team lead. To have an in-depth understanding of and provide leadership to ensure the effective running of the critical care and medical dietetics service. To accept clinical responsibility for a designated caseload of patients within your own scope of practice and organise this effectively and efficiently with regard to clinical priorities and use of time. To work as an autonomous practitioner by planning and prioritising own workload according to locally agreed priority coding and supporting dietetic assistant practitioners and less experienced dietitians to do the same. To undertake a comprehensive assessment (interpretation of biochemistry, anthropometrics, clinical condition and diet history) of patients including those with highly complex presentations/multi pathologies and use advanced clinical reasoning skills to set nutritional goals for each patient. Develop individual treatment plans for patients by calculating the nutritional requirements and support the prescription of feeds where required. To give expert opinion on highly complex nutrition related issues where numerous social, ethical, psychological and medical factors have to be considered and where there may be conflicting ideas from the multidisciplinary team. To negotiate and agree dietary interventions with patients and their carers, using appropriate counselling and motivational skills to optimise their nutritional status. To make recommendations for appropriate prescription of nutritional products according to local guidelines regarding the use of nutritional supplements and enteral feeding products. To ensure adequate monitoring and review of patients. To assess the suitability of incoming referrals according to the agreed referral criteria and liaise with the dietetic assistant practitioner if further information is required prior to acceptance. To co-ordinate appropriate follow up, discharge arrangements and outward referrals to other services in order to facilitate the safe and timely discharge/transfer of care. To be responsible for safe discharge planning including co-ordination of patients requiring home enteral feeding. To have insight to recognise and act appropriately if vulnerable patients are at risk or deemed to be in need. To be responsible for maintaining accurate and comprehensive patient records in line with BDA, HCPC, Trust and department standards. To take appropriate action using highly advanced problem solving skills to assist patients, health and social care professionals and members of the public with queries relating to your own clinical areas. To work flexibly to support the critical care and medicine dietetics team. To provide cover for other Dietetic teams at short notice to cover unplanned and planned absences within the post holders competency and skill level. Is wholly accountable for his / her practice in line with the NMC code of professional conduct and takes every reasonable opportunity to sustain and improve his / her knowledge and professional competence. Result Areas With the assistance of nominated pharmacists and medicines management technicians, to lead the provision of a high level, responsive, and developing clinical pharmacy service to designated specialist surgical wards, theatres, clinics and critical care at Warwick Hospital. To design and deliver packages of pharmaceutical care to individual patients on the designated wards and assist other pharmacists to undertake such duties. To assure the standards of pharmaceutical care provided to patients on the designated wards, ensuring that a satisfactory level of pharmacy service is provided within the staffing resources available. To identify and propose opportunities to realise a cost improvement in the drugs budget. To recommend increased investment in drugs budgets when this would be cost effective for the overall care of patients. To horizon scan and advise on budget implications of new and developing drug therapies in the designated specialty areas. To support the specialty to write medication-based treatment guidelines and clinical protocols, patient group directions, standard operating procedures, and other documents. To advise on/recommend/adapt clinical treatment guidelines for use in the specialty area that have been produced locally or by national bodies e.g. NICE, NSFs, National Prescribing Centre guidelines etc. To supervise, contribute to and undertake research and audit projects in the specialty areas particularly those concerned with the clinical and cost effective use of drugs, compliance with safer prescribing and drug administration policies, procedures and guidelines, and implementation of pharmacy service developments. To provide reports and present the audit findings within the Trust or to outside organisations or regional or national meetings where appropriate. To highlight any practice which has an implication for Primary Care, or which requires particular attention at the interface when patients are admitted or discharged. To develop and maintain a training programme for clinical pharmacists allocated to the specialty areas and to develop and administer a competency assessment process to ensure the appropriate standards are achieved. To undertake the formal duties of a pre-registration tutor/clinical coordinator for the Clinical Diploma in accordance with the requirements of the General Pharmaceutical Council (GPhC) and Universities. To ensure that all appropriate training is provided, reviews are undertaken at the specified times and liaison with the regulatory body and training providers is carried out as necessary. To provide clinical pharmacy training for undergraduate pharmacy students, vacation students, pre-registration pharmacists (hospital and community-based) and hospital clinical pharmacists. We are excited to be advertising a new Band 6 post to work in the Critical Care Outreach Team based at York Teaching Hospital, with occasional cross site cover. Our team deliver a high standard of individualised care to deteriorating and acutely ill patients throughout the hospital 24 hours a day, working closely with the Intensive Care Team and associated multi-disciplinary teams.

Applicants should be highly motivated and able to demonstrate leadership and advanced communication skills. This will be a challenging but highly rewarding role, providing opportunity to work within a team of highly skilled and experienced nurses.

Delivering formal and informal education is an integral part of the role and successful applicants will take an active part in education around the management of deteriorating patients.
