The applicant should be aiming to gain more experience before entering formal training at ST4 onwards or by the CESR route and Article 14, or the Defined Route of Entry into Emergency Medicine (DREEM). This post is a middle grade position which will experience in a supported Emergency Medicine department. This is a new middle grade position as Senior Clinical Fellow in Emergency Medicine with the potential to consolidate their experience in Emergency Medicine. The post will allow the applicant to develop Ultrasound and Research skills. This department has a strong link to ultrasound training and teaching and will provide support to interested candidates to achieve Level 1 & Level 2 Ultrasound skills. The Trust has strong links to Cambridge ultrasound school and will provide the opportunity for your ultrasound competencies to be signed off. This post will also allow you to take part in the active research and quality improvement programs within the department. Many of the research projects have been presented in international conferences and also published. There are four posts in total and will complement a team of fifteen other SPRs in the department. This enables both the provision of high-quality middle doctor staff to the Emergency Department and competencies for their career progression. Senior Clinical Fellow in Emergency Medicine We are advertising Senior Clinical Fellow posts, the successful candidate will have the opportunity to undertake our rotational programme at Lewisham & Greenwich NHS Trust, based in the Emergency Department at Queen Elizabeth Hospital, Woolwich.The post is intended to provide the clinician with the support and training required to successfully complete all aspects of the process. Our current trainees are doing secondments in ITU, Acute Medicine and Paediatrics to gain relevant educational and competency needs. The programme is for four years to be eligible for specialist registration subject to completion of FRCEM. We are exploring the possibility to have a secondment at our local major trauma centre. The remaining nine months of the year will be working in our adult emergency department. Our department is well renowned for its training and support. We operate weekly teaching sessions incorporating a monthly journal club, simulation MDT training (with in-situ component), FRCEM mock OSCE and SAQ practice by our FRCEM examiners and supervised US training.The Consultant body contains current FRCEM examiners, ATLSAPLS & ALS instructors and all of whom are level 1 and some Level 2 US trained. The evidence speaks for itself; we have a proven track record of our long term middle grades successfully becoming consultants via the process with the full support of the department, they would be happy to talk to you about this informally. There will be a specifically designed training and teaching package for all successful applicants and there will be a designated Consultant supervisor for the duration of the rotation.From a clinical perspective these posts will participate in the 24 hour Emergency Department rota. The rotation is initially offered on a fixed term contract for 12 months after which we would envisage an extension on a mutually agreed basis to a 12 month rolling contract. There is also a capacity to do extra bank work within the ED and our staff are given priority so that we can secure our own trusted staff to ensure no compromise in patient care.

The Department DEPARTMENT OF GENERAL SURGERY There are twelve consultant surgeons in the Surgical Department. The surgeons are arranged into four teams (Colorectal, UGI, General and Breast) Other Medical Staff The department is well staffed across all three non-consultant tiers with a range of Associate Specialists, Speciality Doctors and Senior Clinical Fellows at middle grade level. The Core Trainee and FY1 rotas are supported by a number of Junior Clinical Fellows and Advanced Clinical Practitioners. The Post There are 12 Consultant Surgeons together with Specialist Registrars, Core Surgical Trainees and Foundation Trainees. The department provides a wide range of in-patient and day case procedures and provides emergency and elective general surgery services as well as subspecialty services in Colorectal, Breast and Upper GI. The department has an excellent reputation for laparoscopic surgery and performs significant numbers of laparoscopic colorectal resections, groin and ventral hernia repairs and fundoplications each year. The laparoscopic approach is routine for appendicectomy and cholecystectomy (both elective and emergency). The breast unit perform over 250 cancer resections per year with both immediate and delayed reconstructions. Complex abdominal wall reconstructions are undertaken regularly for complex/giant incisional hernia. Acute Surgical Unit In 2019 the Trust opened a new Acute Surgical Unit, with bespoke assessment facilities aimed at maximising our ambulatory care pathways for surgical patients. A senior surgical assessor reviews patients on presentation and integral ultrasound assessment speeds diagnosis. It has reduced non-elective admissions and reduced length of stay. Initially aimed at General Surgery patients, work is underway to broaden the approach to other surgical specialities who could take advantage of the facilities and ambulatory pathways. Urology The department has 5 Consultants plus Specialty and Foundation Trainees. Activity includes outpatient, day case surgery, inpatient elective procedures and emergency admissions ON-CALL We work on a 1 in 9 full shift rota. Post holders will be expected to co-operate in the introduction of new ways of working and monitoring processes with the support of the junior medical doctors. Patients requiring major vascular procedures are transferred to the Sheffield Vascular Institute, which also provides emergency vascular cover. The ST3+ and equivalents cover urology admissions out of hours and are expected to liaise with the consultant urologist who is on call 24/7 if there are urological problems. There is a daily (including weekends) consultant-led urology ward round. Education and Training The successful candidates will have access to all resources currently available in the Trust. This will involve Library and Knowledge Services, e-learning resources, Clinical Skills Training programmes, simulation training and other opportunities. Induction will be provided covering both generic and specialty specific requirements. For new starters in the UK there will be a one-week programme in PGME prior to starting clinical duties to prepare the post holder for practice in the UK. This may be extended according to individual needs. Initial work in clinical areas would be supernumerary until the Clinical Supervisor agrees the doctor is ready to do on call. The post holders will be expected to develop a portfolio showing on-going professional development and can access support from PGME to do this. The post holders will have a named Educational Supervisor; with whom they will meet on a regular basis for appraisal. They will agree to a Personal Development Plan and receive assessment in the workplace and regular feedback, including multisource feedback. Each clinical department will provide access to in-house teaching equivalent to training grade doctors. It is intended that a monthly teaching session for doctors on this rotation will be established. Main Duties & Responsibilities To acquaint him/herself with details of patient management in discussion with the consultants and ensure their efficient execution. To ensure that good communications are maintained with all medical colleagues, e.g. anaesthetists, cardiologists, nursing staff and paramedical departments such as physiotherapists, radiographers and ancillary staff, to the benefit of the patient. To ensure other Specialty Registrars and Consultants are kept informed regarding alteration in the condition of any of their patients. To ensure that all in-patient notes are kept up to date and tidy. Supervise the junior clinical fellow staff (SHOs) in the performance of their duties. Duties - Professional To take a history, examine and record all findings of all patients admitted under the Consultants care. Carry out and arrange all relevant investigations. To see and examine all patients under the care of the Consultant concerned. To attend and assist at out-patient sessions as per timetable of firm. To arrange for the transfer of patients to other hospitals or discharge home, with a discharge summary on the clinical episode. To be responsible for the care of all/any respiratory patients on the wards. To dictate all discharge summaries within 24hrs and ensure that these are forwarded to the patients, General Practitioner and/or hospital of transfer as soon as possible after the patient is discharged and complete letters to General Practitioners. 10 To cross cover, in conjunction with other Specialty Registrars when one Specialty Registrar is absent or on annual or study leave and to ensure that the duties and responsibilities of the absent colleague are carried out. To keep the Consultants informed of any patient who is giving cause for concern. To communicate with patients relative on the progress of the patients. To be available for emergency calls to ITU and other parts of the hospital, if required i.e. for cardiac arrest. To participate in research projects that may already be in progress. To participate in lecturing to other staff if required, e.g. nurses. To participate in the Medical Audit Programme as required. To accept that occasional emergencies and unforeseen circumstances may occur, and to respond to the needs of the service when they arise.

Clinical Attend and assist in the out-patient clinics, taking responsibility to ensure adequate staffing for the clinic and to facilitate its efficient running in any other way. Clinic letters must be dictated promptly. Attend consultant ward rounds. Make formal ward rounds daily including at weekends and holidays as part of the on-call responsibilities. These rounds should be considered a teaching opportunity for more junior staff. Participation in the relevant rota will depend on clinical experience of the successful candidate. Out of hours responsibilities include: Night-time supervision of care of surgical patients including those in the Intensive Care Unit/Recovery Unit and communicate appropriately with the responsible consultant regarding major problems arising with that consultants patients. Weekend on-call responsibilities include ward rounds as well as participation in emergency operations and assessment of in-patients or outside referrals. Advise on the discharge of patients under the care of the cardiothoracic surgical consultants. Theatre responsibilities Ensure that the theatre cases are appropriately staffed. Ensuring that junior staff are involved at an appropriate level wherever possible. Junior staff must be assigned to cases whenever ward responsibilities permits. Ensuring that adequate junior level assistants are available to help open and close cases performed under the direction of, or with the assistance of, a consultant. Ensuring that all cases are sent for promptly and facilitating the efficient transfer of patients if required. Ensuring that communication is maintained with the relatives of patients. Ensure that either the operating surgeon or senior level assistant involved in the operation complete all appropriate data entry tasks at the conclusion of the operation. In addition, the principal operator should be responsible for creating a suitable operation note, which accurately records the events of the procedure. If operations are delayed or cancelled, the post holders are responsible for communicating with the wards appropriately and explaining the situation to the patients. Admission responsibilities Communicate with the consultant staff regarding their respective theatre schedules individually and co-ordinate hospital admission in an integrated and coherent manner such that hospital beds are utilised most efficiently. This may require suggesting modifications to the planned admissions schedule to individual consultants on the basis of in-patient referrals, emergency cases or theatre cancellations. Participation in weekly planning meetings is required. Waiting list management Maintain knowledge of the active waiting list for your consultant and participate in their selection for admission and surgery in a manner that is equitable for patients as much as possible. Be involved in the evaluation of patients referred to the service, particularly in-patient referrals. Respond to queries from the patients and their GPs (both by telephone and letter) regarding their position on the waiting list and anticipated time of operation. Administration of service As part of their overall responsibilities for the patients, post holders must ensure that medical records are adequately kept by the junior medical staff including daily notes on in-patients and prompt dictation of discharge summaries. Notes must be representative and appropriate to the patients true condition. Medical audit Post holders must ensure that all junior staff are present at the monthly medical audit meeting. Opportunities to present cases for discussion from the firm will arise and should be prepared for and presented by post holders or assigned to a colleague for presentation. Research Research is an integral component of the training programme. Post holders are encouraged and expected to participate in clinical or basic science research. Teaching Post holders are expected to take an active role in the postgraduate teaching activities. In addition he/she may be required to undertake teaching of medical students, nurses and other paramedical staff. Clinical research is continuously undertaken within the unit and the post holder will be encouraged to participate.
