Senior Clinical Fellow in Emergency Medicine We are advertising Senior Clinical Fellow posts, the successful candidate will have the opportunity to undertake our rotational programme at Lewisham & Greenwich NHS Trust, based in the Emergency Department at Queen Elizabeth Hospital, Woolwich.The post is intended to provide the clinician with the support and training required to successfully complete all aspects of the process. Our current trainees are doing secondments in ITU, Acute Medicine and Paediatrics to gain relevant educational and competency needs. The programme is for four years to be eligible for specialist registration subject to completion of FRCEM. We are exploring the possibility to have a secondment at our local major trauma centre. The remaining nine months of the year will be working in our adult emergency department. Our department is well renowned for its training and support. We operate weekly teaching sessions incorporating a monthly journal club, simulation MDT training (with in-situ component), FRCEM mock OSCE and SAQ practice by our FRCEM examiners and supervised US training.The Consultant body contains current FRCEM examiners, ATLSAPLS & ALS instructors and all of whom are level 1 and some Level 2 US trained. The evidence speaks for itself; we have a proven track record of our long term middle grades successfully becoming consultants via the process with the full support of the department, they would be happy to talk to you about this informally. There will be a specifically designed training and teaching package for all successful applicants and there will be a designated Consultant supervisor for the duration of the rotation.From a clinical perspective these posts will participate in the 24 hour Emergency Department rota. The rotation is initially offered on a fixed term contract for 12 months after which we would envisage an extension on a mutually agreed basis to a 12 month rolling contract. There is also a capacity to do extra bank work within the ED and our staff are given priority so that we can secure our own trusted staff to ensure no compromise in patient care.

The Department DEPARTMENT OF GENERAL SURGERY There are twelve consultant surgeons in the Surgical Department. The surgeons are arranged into four teams (Colorectal, UGI, General and Breast) Other Medical Staff The department is well staffed across all three non-consultant tiers with a range of Associate Specialists, Speciality Doctors and Senior Clinical Fellows at middle grade level. The Core Trainee and FY1 rotas are supported by a number of Junior Clinical Fellows and Advanced Clinical Practitioners. The Post There are 12 Consultant Surgeons together with Specialist Registrars, Core Surgical Trainees and Foundation Trainees. The department provides a wide range of in-patient and day case procedures and provides emergency and elective general surgery services as well as subspecialty services in Colorectal, Breast and Upper GI. The department has an excellent reputation for laparoscopic surgery and performs significant numbers of laparoscopic colorectal resections, groin and ventral hernia repairs and fundoplications each year. The laparoscopic approach is routine for appendicectomy and cholecystectomy (both elective and emergency). The breast unit perform over 250 cancer resections per year with both immediate and delayed reconstructions. Complex abdominal wall reconstructions are undertaken regularly for complex/giant incisional hernia. Acute Surgical Unit In 2019 the Trust opened a new Acute Surgical Unit, with bespoke assessment facilities aimed at maximising our ambulatory care pathways for surgical patients. A senior surgical assessor reviews patients on presentation and integral ultrasound assessment speeds diagnosis. It has reduced non-elective admissions and reduced length of stay. Initially aimed at General Surgery patients, work is underway to broaden the approach to other surgical specialities who could take advantage of the facilities and ambulatory pathways. Urology The department has 5 Consultants plus Specialty and Foundation Trainees. Activity includes outpatient, day case surgery, inpatient elective procedures and emergency admissions ON-CALL We work on a 1 in 9 full shift rota. Post holders will be expected to co-operate in the introduction of new ways of working and monitoring processes with the support of the junior medical doctors. Patients requiring major vascular procedures are transferred to the Sheffield Vascular Institute, which also provides emergency vascular cover. The ST3+ and equivalents cover urology admissions out of hours and are expected to liaise with the consultant urologist who is on call 24/7 if there are urological problems. There is a daily (including weekends) consultant-led urology ward round. Education and Training The successful candidates will have access to all resources currently available in the Trust. This will involve Library and Knowledge Services, e-learning resources, Clinical Skills Training programmes, simulation training and other opportunities. Induction will be provided covering both generic and specialty specific requirements. For new starters in the UK there will be a one-week programme in PGME prior to starting clinical duties to prepare the post holder for practice in the UK. This may be extended according to individual needs. Initial work in clinical areas would be supernumerary until the Clinical Supervisor agrees the doctor is ready to do on call. The post holders will be expected to develop a portfolio showing on-going professional development and can access support from PGME to do this. The post holders will have a named Educational Supervisor; with whom they will meet on a regular basis for appraisal. They will agree to a Personal Development Plan and receive assessment in the workplace and regular feedback, including multisource feedback. Each clinical department will provide access to in-house teaching equivalent to training grade doctors. It is intended that a monthly teaching session for doctors on this rotation will be established. Funding for external study leave may be available from the department equivalent in value to career grade staff employed by the Trust.

St Georges Hospital is a major teaching hospital in SW London. General Paediatrics:

This post will suit those wishing to gain substantial experience in a large, well supervised unit with a mixed general and subspecialist patient group. Paediatric Infectious Diseases/oncology:

You will be part of the ward team caring for all patients on Pinckney ward, the paediatric infectious diseases and oncology ward. Children admitted to Pinckney ward have a variety of infectious conditions such as tuberculosis, meningitis and HIV. You will also be involved in caring for patients with an oncological diagnosis who are admitted either for chemotherapy or febrile illness. You will have the opportunity to attend infectious diseases meeting, and attend specialty clinics.

Paediatric Surgery:

St George’s is the regional referral centre for children requiring paediatric surgery. Children are admitted electively for a range of procedures, many of these with co-morbid medical conditions and complex needs. Acutely children are also referred for emergency surgery from regional hospitals.

Paediatric Neurosciences:

St George’s is also a regional neurosurgical centre and hosts regional tertiary paediatric neurology. The teams work closely in the care of children with complex needs and provide a number of specialist treatments. St George’s is also a major trauma centre and provides care for children with the sequelae of major trauma. Duties will involve daily assessment of the children, participating in neurosurgical and neurology consultant led ward rounds, and working with the specialists to arrange investigations and treatment.

Paediatric Community Services:

The paediatric neurodevelopmental, neurodisability and epilepsy service is well established and offers a child-centred, consultant led service. It is part of the integrated child health department of St George’s Hospital and offers a comprehensive service for a wide range of neurodevelopmental problems (including ADHD and ASD), complex neurodisability, Down’s Syndrome, Cerebral Palsy and Epilepsy. The ADHD Service is a specialist Attention Deficit Hyperactivity Disorder (ADHD) service within the developmental paediatric team. The service supports parents and children through the assessment, diagnosis and management of ADHD. We take referrals of children under the age of 11 years old who are registered with a GP in the Borough of Wandsworth. We provide follow up care for children who are under our service, until they are old enough to transition to adult services.

The post is a full time role (40 hours per week), during COVID recovery we are not fully open but when back to full timetable the post holder will take part in 1 in 3 Saturday shift. The post provides out-patient care for patients attending the Integrated Sexual Health clinic. Training and experience in contraception and GUM, although desirable is not essential. The post holder will be supported and trained in these disciplines with support and supervision to hand at all times.

The majority of sessions will be general Sexual Health Clinics. The post holder will, on a rotational basis contribute the medical cover of the weekly Young Persons sexual health clinic.

The job plan will include clinical admin time (where the post holder will manage clinical results for the service), an audit session, and an education/teaching session. The education session, on Wednesday morning, is when clinics are closed and consists of weekly departmental teaching (journal club, audit, case presentations), and small group self-directed teaching. The administration/audit and educational time will be protected.

The post holder is responsible to the Consultant Lead in Sexual Health. The post holder will be expected to continually update their education in Genito-Urinary Medicine and Sexual & Reproductive Healthcare and to fulfil their Continuing Medical Education and GMC revalidation requirements.

The proposed job plan will be discussed in detail with the post holder on appointment. On appointment of the post holder, the Clinical Lead will commence discussions to finalise the job plan which will become operational on appointment. The post would suit an individual seeking Out of Programme for Experience (OOPE) an individual looking for post-CCT experience.
