You will play a central role in facilitating case conferences, managing disputes, and responding to incidents within the facility. Your responsibilities will include conducting intake assessments, referring residents to external resources, and providing counselling and support to address their needs. Social Work

• Oversee a range of social work activities such as intake assessments, allocation of rooms, and referrals to external / community stakeholders, partners, and resources.
• Oversee the admission interviews for the referred Authorised Occupier(s) seeking admission.

• Communication: Excellent interpersonal and communication skills for effective communication with residents, team, community partners, and stakeholders.
• Leadership: Excellent team player with strong leadership and responsibility skills.
• Adaptability: Strong team player who is resourceful, adaptable, and able to perform multitasks under a given timeline.
• Collaboration: Willing to work with the homeless and destitute. Able to handle shelter residents politely yet firmly.
• Understands the principles of engaging and working with vulnerable families and individuals.
• Case management and casework knowledge and skills.
• Risk assessment and risk management.
• Good report writing skills.
• Self-motivated individual with the ability to work independently.

Technical Competencies

• Proficiency in IT Systems and Microsoft Office Suite (e.g., Microsoft Word, Excel, PowerPoint, etc.).

Education and Experience:

• Degree in Social Work from an accredited institution
• Accredited as Registered Social Worker with SASW
• At least 4 years of relevant working experience in similar sectors

About Active Ageing Centres (AAC)

Our AAC serve as a key point of contact for seniors under our care, where we proactively reach out to them, organize social activities, and provide opportunities for interaction with the community. Our aim is to enable seniors to adopt an active lifestyle, pre-empt social isolation, and refer them to assistance in a timely fashion. We hope to improve the health outcomes of seniors living in the community through providing a suite of programs to help them live healthier lives and support their aspirations to age in place.

Responsibilities

• Recruit, manage and engage volunteers
• ⁠Handle programme enquiries and registration
• Assist in the coordination and delivery of Volunteer Training courses successfully
• Assist Programme Executive to prepare quarterly reports
• General administrative duties

Requirements:

• Min. GCE ‘A’ Level, Diploma with Social Service
• ⁠Experience in coordinating and recruitment work preferred
• Good communication & interpersonal skills
• Preferably with interest and passion for the elderly
• This position is on contract basis
