This is an early intervention service aimed at CYPs with mild to moderated mental health and emotional difficulties. The main duties will include triage, consultation, advice and support and evidence based low-intensity interventions. A major part of the role will be consultation and signposting to a wider range of community mental health services. As a Senior Mental Health Practitioner you will be embedded within the PCNs multi-disciplinary team as the first contact for Children and Young People presenting with mental health and emotional difficulties. You will work alongside other PCN professionals including GPs, nurses, and other ARRS practitioners to ensure a holistic approach by providing early and timely assessments to ensure the right information, advice and support is provided at the right time. This integral innovative way of working will seek to eliminate current barriers to accessing services whilst meeting the needs of our CYP, carers and families at the earliest point to avoid deterioration in presentations and improve population health, experiences and outcomes close to their own homes. A key co-ordinating role at the heart of the GP practices and are a fully integrated member of the multi-disciplinary team with regards to response and management pathways. Work with CYP up to the age of 18 whose emotional and mental health needs can be best met within primary care services and whose difficulties are best understood within a biopsychosocial Act as a first point of contact within the PCN and triage referrals. Be able to work dynamically and autonomously to provide a combined triage, consultation, advice and liaison function in accordance with agreed protocols Support CYP and their families who present to their GP with identified or suspected emotional and mental health issues such as anxiety, low mood, eating difficulties. Develop a holistic personalised care and support plan with the young person and their families. Responsive to prevent difficulties escalating and integrated working to ensure the child/young person is receiving the right care at the right point. Be able to identify and intervene to meet presenting needs by providing extra time, capacity and expertise to make sure that the appropriate support is made available to CYP and their families to ensure that their needs are addressed timely and effectively. Work closely with PCN colleagues and other community care-based services to ensure CYP and their families receive the care that is responsive to their needs and designed in co-operation with the young person and their family. Promote and deliver low level early intervention and treatment which may include brief psychological interventions as well as discharge appropriately when completed. Facilitate onward access to other community care-based services, or third sector services that are integrated within the CYP mental health Getting Help pathways. May supervise junior staff members and coordinate the day-to-day functioning and delivery of the clinical They will also receive appropriate clinical and managerial supervision from their Line Manager or appropriately identified lead. Contribute to delivering and co-developing the primary care mental health service model. This includes participating and contributing to the development of care pathways for the provision of integrated physical and mental health care and also offering consultation and training to primary care team.
