The Patient Care Technician (PCT) -Nursing (NUR) will work under the direction of the Registered Nurse. During the assigned work period, PCT-NURs will collect patient data, complete personal care of the patient, perform point of care testing such as whole blood glucose, turn & position and/or mobilize patients, and assist with caring for the needs of assigned patients with a focus on progress toward discharge, including during transitions within the acute care stay. The PCT-NUR promotes the availability of resources for patient care for use, stocking, and storage of supplies.
The Patient Care Technician- Nursing participates in the creation and maintenance of a clean, orderly, and safe environment of care.  PCT-NURs may also be asked to provide assistance with patients requiring 1:1 observation.

The Colorectal Surgery and Urology Unit consists of 22 semi-private rooms. This Post-Surgical setting cares for patients that have undergone colorectal and urologic surgery. The nursing staff will work closely with the Wound Ostomy and Continence Nurses for ostomy and wound care needs. RNs will also care for post-operative patients with TPN, NG tubes, and central lines.

d. Collaborates with the health care team and appropriate department in the management of care across the continuum of care, including pre-admission, discharge, post-discharge, planning length of stay, and utilization of resources.

e. Utilizes own special body of knowledge and evidence based guidelines to provide leadership and guidance to the health care team in formulating an individualized multidisciplinary plan of care to include: pre hospitalization, acute hospital care, discharge education, transition to home and use of community resources.

2. Utilization Review

b. Contacts payer source to confirm/negotiate benefits and provide concurrent reviews.

c. Identifies capitated patients to determine appropriate utilization of series and coordinates post hospital care using defined standards.

d. Identifies high-risk patients based on clinical and financial criteria for collaboration with patient financial services to problem-solve available resources.

e. Ensures that appropriate medical/legal documentation is contained in patient's records.
