Key Responsibilities: To deliver a 12-week programme of support to service users who are referred to the service. To deliver tobacco harm reduction assessments, advice, and support sessions, including advice on stop smoking medications and vapes. To provide 1:1 and group behavioural support and advice to service users with an SMI who wish to switch to a vape and access nicotine replacement therapy (NRT). To understand the range of interventions available, the evidence base and potential outcomes for service users and apply appropriate either in group or individual settings. Job Summary: The key function of this job is to support the smooth, efficient, and effective running of the Tobacco Dependence Treatment (TDT) service. The post holder is responsible for supporting the TDT Team as the Team are based in University Hospital Lewisham (UHL) and Queen Elizabeth Hospital (QEH). The post-holder will assist the department in the efficient running and delivery of smoking cessation service at UHL and QEH, including the ward-based service. They will independently carry out assessments with patients to identify them as current smokers who wish to access smoking cessation support. They will be expected to work under their own initiative but with access to support and supervision from senior colleagues. They will provide a key role in the management of adults who smoke and are admitted to the inpatient wards. The post-holder will have a good knowledge of smoking cessation, including the practical and safety aspects of smoking cessation brief advice as well as NRT provision. Under the supervision of the smoking cessation practitioners, they will enter data as required, contributing to patients assessment and progress through the service. They will support with administrative tasks as needed and be responsible for the regular maintenance of smoking cessation equipment. To proactively engage patients, staff, and visitors to reinforce the ward-based TDT interventions, smokefree grounds policy, and to encourage smokers to access the available treatment services. The post holder will assist in the triaging of referrals to the smoking cessation service and complete telephone reviews as required. Key Tasks & Responsibilities: Work closely with the tobacco dependence lead and tobacco dependence team, to support implementation of the tobacco dependence programme of work. Participate in the creation of the public health teams annual business plan and service objectives. Support delivery of specific objectives in the tobacco dependence programme. Establish effective relationships with relevant stakeholders alongside the tobacco dependence lead. Participate in and provide support for teaching and training, research and evaluation activities as appropriate. Be a supportive agent for change, recognising the culture and goals of different teams and settings. Through accurate record keeping ensure the trust can demonstrate excellence in the implementation of the national tobacco dependence programme across care pathways, aligned with national guidance on the role of NHS providers in prevention and population health, including guidance published by NICE, the Care Quality Commission and NHS England/Improvement.

The Tobacco Dependency Advisor will work across inpatient, rehabilitation and community teams under the supervision of the Clinical Matron, undertaking tobacco dependence assessments, educating service users about options to manage their tobacco dependence differently, without harm from smoking and supporting quit attempts. The Tobacco Dependency Advisor will be responsible for creating personalized QUITT Plans and assist service users to access the support they need for their quit attempt both during their inpatient stay and arranging and liaising with the relevant teams for discharge. The Tobacco Dependency Advisor will also be responsible for supporting service users maintain their temporary abstinence from smoking while admitted to our services. The Tobacco Dependency Advisor will also collect data in accordance with the metrics required by the ICS and NHSE monitoring. The Tobacco Dependency Advisor will play a key role within a wide multidisciplinary team, to raise awareness of the harm from tobacco and the effects on long term physical health, particularly for people with serious mental illness. The Tobacco Dependency Advisor will develop links with other local & regional Tobacco Dependency services. They will play a key role in developing a regional Tobacco Dependency advisor network and supporting the research aims of the Tobacco Dependency team.
