POSITION SUMMARY

This position evaluates medical records, provides clinical and surgical abstraction for full range of complex and/or multispecialty surgical, procedural and E&M professional services in accordance with nationally recognized coding guidelines. Utilize coding knowledge and expertise to support department projects, validation edits and/or revisions.

CORE FUNCTIONS

1. Analyzes medical information from medical records. Accurately codes diagnostic and procedural information in accordance with national coding guidelines and appropriate reimbursement requirements. Consults with medical providers to clarify missing or inadequate record information and to determine appropriate diagnostic and procedure codes. Provides thorough, timely and accurate coding in accordance to department specific productivity and quality standards. Codes ICD CM and CPT4 for accurate APC assignment. Addresses National Correct Coding Initiative (NCCI) edits as appropriate.  Reconciliation of charges as required.

2. Abstracts clinical diagnoses, procedure codes and documents other pertinent information obtained from the medical record into the electronic medical records. Seeks out missing information and creates complete records, including items such as disease and procedure codes, discharge disposition, date of surgery, attending physician, consulting physicians, surgeons and anesthesiologists, and appropriate signatures/authorizations. Refers inconsistent patient treatment information/documentation to coding quality analysts, supervisor or individual department for clarification/additional information for accurate code assignment.

3. Provides quality assurance for medical records. For all assigned records and/or areas assures compliance with coding rules and regulations according to regulatory agencies for state Medicaid plans, Center for Medicare Services (CMS), Office of the Inspector General (OIG) and the Health Care Financing Administration (HCFA), as well as company and applicable professional standards.

4. As assigned, compiles daily and monthly reports; tabulates data from medical records for research or analysis purposes.

5. Able to identify validation edits and revision issues to ensure compliant coding.

6. Recognizes and distinguishes complex diagnoses and procedures and has attention to detail to make needed corrections and ensure accurate coding, reimbursement, and compliance.

7. Works independently under regular supervision. Uses specialized knowledge for accurate assignment of ICD/CPT codes according to national guidelines. May seek guidance for correct interpretation of coding guidelines and LCDs (Local Coverage Determinations).

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge and specialized formal training equivalent to the two year certification course in medical record keeping principles and practices, anatomy, physiology, pathology, medical terminology, standard nomenclature, and classification of diagnoses and operations, or an Associate’s degree in a related health care field.

Requires at least one of the following: Certified Professional Coder (CPC), Certified Coding Specialist (CCS), Certified Coding Specialist – Physician (CCS-P), Registered Health Information Administrator (RHIA), or Registered Health Information Technician (RHIT), in an active status with the American Health Information Management Association (AHIMA) or American Academy of Professional Coders (AAPC).  Certification may also include a general area of specialty.

Requires three or more years of complex professional coding experience within specialty.

Must demonstrate a level of knowledge and understanding of ICD and CPT coding principles as recommended by the American Health Information Management Association coding competencies, and as normally demonstrated by certification by the American Academy of Professional Coders.

Must be able to work effectively and efficiently in a remote setting, utilizing common office programs, coding software and abstracting systems.

PREFERRED QUALIFICATIONS

Specialty Certification. Radiology Certified Coder (RCC) if employed in the Imaging space.

Incumbent to fill this role will be responsible for functions which fall under the scope of the Radiology service. These include clinical and administrative management and include but are not limited to the following:

Clinical Responsibilities

• Incumbent spends 75% of time with clinical responsibilities. During clinical time the incumbent will:

Provide Geropsychiatry consultation services to veterans with dementia residing in our Community Living Center, along with recommendations for pharmacologic or nonpharmacologic interventions, diagnostic evaluations, and/or treatment of other mental health issues and behavioral/psychological symptoms. Are you a superstar strong Surgical Oncology Complex Coder | Medical Coder looking for the opportunity to code a wide variety of accounts? This Surgical Coder will be supporting very busy providers/surgeons in our non-academic and academic arena. Ideal candidate would have 2 years+ of coding experience within Surgical Oncology, and have surgical reconstruction experience in Plastics Reconstruction, Cardio Thoracic, Hepato-Pancreatic-Biliary (HPB). Additionally, someone with strong surgical experience in General Surgery, GI, Colorectal, Breast, and ENT also helpful. This position has a variety of specialties that will be coded to keep your job interesting and fun. There are also opportunities for overtime with special projects from time to time. This requires being fully CPC (AAPC) or CCS (AHIMA) certified, a CPC-A will not be accepted in this position.

This is an Oncology Coding Team with 11 coders that work with E/M, procedures and surgical cases – The team has 1 Sr. Coder, an Associate Manager, Associate Director, Director has the hierarchy. Production standards are as follows:

Academic: 6-9 E/M per hour

                10-20 minutes per surgical case depending on complexity

Non-Academic: 9-12 E/M per hour

                10-20 minutes per surgical case depending on complexity

You should have 6.5 hours of productivity time - 1.5 hours non-production would be spent in (2) 15 min breaks, tasks, emails, meetings.

POSITION SUMMARY

This position evaluates medical records, provides clinical and surgical abstraction for full range of complex and/or multispecialty surgical, procedural and E&M professional services in accordance with nationally recognized coding guidelines. Utilize coding knowledge and expertise to support department projects, validation edits and/or revisions.

CORE FUNCTIONS

1. Analyzes medical information from medical records. Accurately codes diagnostic and procedural information in accordance with national coding guidelines and appropriate reimbursement requirements. Consults with medical providers to clarify missing or inadequate record information and to determine appropriate diagnostic and procedure codes. Provides thorough, timely and accurate coding in accordance to department specific productivity and quality standards. Codes ICD CM and CPT4 for accurate APC assignment. Addresses National Correct Coding Initiative (NCCI) edits as appropriate.  Reconciliation of charges as required.

2. Abstracts clinical diagnoses, procedure codes and documents other pertinent information obtained from the medical record into the electronic medical records. Seeks out missing information and creates complete records, including items such as disease and procedure codes, discharge disposition, date of surgery, attending physician, consulting physicians, surgeons and anesthesiologists, and appropriate signatures/authorizations. Refers inconsistent patient treatment information/documentation to coding quality analysts, supervisor or individual department for clarification/additional information for accurate code assignment.

3. Provides quality assurance for medical records. For all assigned records and/or areas assures compliance with coding rules and regulations according to regulatory agencies for state Medicaid plans, Center for Medicare Services (CMS), Office of the Inspector General (OIG) and the Health Care Financing Administration (HCFA), as well as company and applicable professional standards.

4. As assigned, compiles daily and monthly reports; tabulates data from medical records for research or analysis purposes.

5. Able to identify validation edits and revision issues to ensure compliant coding.

6. Recognizes and distinguishes complex diagnoses and procedures and has attention to detail to make needed corrections and ensure accurate coding, reimbursement, and compliance.

7. Works independently under regular supervision. Uses specialized knowledge for accurate assignment of ICD/CPT codes according to national guidelines. May seek guidance for correct interpretation of coding guidelines and LCDs (Local Coverage Determinations).

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge and specialized formal training equivalent to the two year certification course in medical record keeping principles and practices, anatomy, physiology, pathology, medical terminology, standard nomenclature, and classification of diagnoses and operations, or an Associate’s degree in a related health care field.

Requires at least one of the following: Certified Professional Coder (CPC), Certified Coding Specialist (CCS), Certified Coding Specialist – Physician (CCS-P), Registered Health Information Administrator (RHIA), or Registered Health Information Technician (RHIT), in an active status with the American Health Information Management Association (AHIMA) or American Academy of Professional Coders (AAPC).  Certification may also include a general area of specialty.

Requires three or more years of complex professional coding experience within specialty.

Must demonstrate a level of knowledge and understanding of ICD and CPT coding principles as recommended by the American Health Information Management Association coding competencies, and as normally demonstrated by certification by the American Academy of Professional Coders.

Must be able to work effectively and efficiently in a remote setting, utilizing common office programs, coding software and abstracting systems.

PREFERRED QUALIFICATIONS

Specialty Certification. Radiology Certified Coder (RCC) if employed in the Imaging space.
Experience in a large, multi-system physician practice preferred.

The general surgeon will diagnose and treat medical and surgical disorders using the most appropriate techniques in providing quality care. Board Certified/Board Eligible general surgeon is responsible for initial evaluation and overall workups, history & physicals, and admission of general surgery patients as needed.

Clinical duties include but are not limited to:

• Operating Room (experience in wide variety of standard general surgical procedures and operations), in-patient rounds, Emergency Room and Outpatient Clinics.
• Perform a wide variety of general surgical procedures.
• May supervise residents from the Phoenix Integrated Surgical Residency Program, Mayo Clinic, and other training programs, as well as medical students from University of Arizona, Mayo Clinic and Podiatry students and residents who rotate through the General Surgery Service.
• Required to participate in all surgical quality/performance improvement initiatives as appropriate, including VASQIP.
• Participate in general surgery night call and perform emergency general surgical procedures.
• Responsible for providing patients with education concerning their disease state and treatment regimen.

The Southern Arizona VA Healthcare System (SAVAHCS) is recruiting a full-time Physician Radiologist - (Diagnostic Imaging/Mammography) to join a dynamic team of dedicated providers and ancillary staff. The selected provider will report directly to the Section Chief of Radiology.

Board Certified or eligible in Diagnostic Radiology.

Significant experience with Breast Imaging interpretation or have recently completed a fellowship in Mammography.

Capability of performing breast biopsies, including stereotactic biopsies, is required. Experience with MR Breast and MR guided breast biopsies is preferred.

Processing the capability to interpret examinations related to other specialties is required, to particularly include cross sectional imaging, as well as the ability to interpret radiographs. The ability to perform fluoroscopic exams and cover Nuclear Medicine is desired, but not a requirement.
