We are seeking a patient focused pharmacist aiming to build on established clinical skills and work closely with clinicians in providing a high-quality service to patients within ESHT. We also require someone who can demonstrate a commitment to personal and staff development. This specialist pharmacist post is based within the Acute Trust Division and is responsible for supporting the Lead Divisional Pharmacist(s) in providing clinical pharmacy and business level support within East Sussex Healthcare NHS Trust.

In addition, tosupport the Trust Lead Antimicrobial/Divisional pharmacist on the use and management of antimicrobial therapy at ESHT.





We are looking for a dynamic and experienced hospital pharmacist to work with us in a portfolio style role within acute medicine division and antimicrobial use. This post will be based at Conquest Hospital, although there may be an element of cross site working at Eastbourne and involve evening duties. Under the supervision of a designated Lead Pharmacist, you will work closely with consultants, other medical staff, nurses, pharmacy staff and other allied healthcare professionals in ensuring that medicine use is safe, effective and evidence based across ESHT. The post holder will be involved in providing clinical pharmacy services to Acute inpatient wards and occasional support to the dispensary. This post also holds an education and training commitment, which in the past has proved successful in developing staff. The successful candidate will also be responsible for helping to train and mentor pre-registration pharmacists, pharmacy undergraduates and rotational clinical pharmacists. The applicant will be required to join the department weekend and on-call service on a rota basis following a period of orientation and induction.

Job responsibilities To lead and support the work of the Primary Care Network to ensure that all care home residents receive an annual Structured Medication Review. To support patients to take their medication correctly and to ensure they get the best possible outcomes. Keep records of reviews and issues arising to identify trends. Feedback identified prescribing issues to support the care homes team in developing quality initiatives and action plans. Make amendments to patient records, written and/or computer, as appropriate to ensure agreed changes to therapy are actioned in a precise and systematic fashion. Liaise and communicate sensitively and effectively with patients and/or carers, who on occasions may be resistant to change, on subjects which are often of a very sensitive nature, on all aspects of good medicines management with the objective of optimising patient outcomes. Lead provision of patient-oriented pharmaceutical care services including creation and maintenance of pharmaceutical care plan, cost-effective therapeutic drug monitoring, drug utilization, guideline development, and drug use evaluation in assigned clinical teams. Provide training and educational support for development of clinical skills in professional staff. Required Qualifications: 

• Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program OR possess a Bachelor or Master of pharmacy degree with 10 years of experience required.

• Graduate of an American Society of Health-System Pharmacists (ASHP) accredited postgraduate year one residency with a focus in ambulatory care or an ASHP accredited postgraduate year two residency relevant to practice area or have 4 years of pharmacist experience in an ambulatory care environment or have pharmacist experience within a Nebraska Medicine ambulatory care clinic required.

• Demonstrate excellence in clinical areas as displayed by ability to consistently and completely meet and contribute to updating standards of practice required.

• Licensed in the state of Nebraska or jurisdiction in which pharmacy practice is conducted upon start or transfer required.

• Basic Life Support (BLS) certification as deemed appropriate and relevant to practice required, must be from an American Heart Association (AHA) endorsed program (effective for new hires starting January 1, 2023 or later).

As an Internal Medicine Clinical Pharmacist Specialist, you will be responsible for developing and maintaining a high-level clinical pharmacy practice in adult Internal Medicine. POSITION SUMMARY
This position actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity. This position is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Conducts educational activities, applies principles of performance improvement, and participates in Pharmacy and Therapeutics Committee/Clinical Consensus Group activities as appropriate. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients being served. Accountable for departmental, and individual patient outcomes by assessing and evaluating data reflective of the patient’s status, interpreting the appropriate information needed to identify each patient’s requirements, and providing pharmaceutical care based on individual needs. Responsible for the delivery of pharmaceutical care to patients using the medication use process. Oversees the activities of the pharmacy technicians, coordinating workflow, and the checking of medications prior to being dispensed. Adheres to local, state, and federal laws regarding the profession of pharmacy practice.

CORE FUNCTIONS
1. Critically evaluates the management of disease states and adjusts or recommends adjustments to therapy to   improve patient medication regimens. Ensures appropriate and cost-effective medication therapy. Areas of focus may include but are not limited to evaluating disease state management, medication management, and adverse drug reactions. Adheres to all clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g., collaborative practice agreements). Accurately and completely documents patient assessment data and clinical interventions.

2. Drug information expert in area of specialty and may act as a system resource. Provides detailed drug information and education to healthcare team, pharmacy, patients, and community. Provides education for pharmacy staff, students, and residents where applicable.

3. Reviews and evaluates medication orders for appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, allergy, laboratory values and other pertinent patient information. Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g. therapeutic substitutions, formulary management).

4. Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and facility/system-based projects (i.e. data collection, documentation, med error and adverse drug event trending). Participates in drug use evaluations and facility level medication safety oversight.

5. Participates in local and system-based committees (e.g. area of specialty or other related area) and effectively communicates to stakeholders. Participates in the development of innovative drug-related patient care practices in their specialty at both institutional and system level. Serves as a resource and mentor to other pharmacists, students, residents, and other health-care providers at both the facility and system level.

 

6. Meets ASHP preceptor qualifications. Participates as primary preceptor to residents where applicable. When needed, serves as the Program Director for the institution’s Pharmacy Practice Residency Program. Serves as an educator, resource expert, and mentor to other pharmacists, students, residents, and/or other healthcare providers.

7. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. MINIMUM QUALIFICATIONS
 

A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc).

Additional minimum qualifications include board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP). Board certification must be active and maintained.

For pharmacists providing care in Investigational Drug Services, the ASHP Investigational Drug Service Certificate will be allowed. Candidates who are not Board Certified will be given 24 months to achieve certification.
 

Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer. Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy. May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.

Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.

Job Description:

Responsible for performing all duties associated with interpretation, order verification, dispensing and monitoring of medications ordered by prescribers; the evaluation and dissemination of comprehensive information about medications and their use, to patients, families, and medical staff; and providing quality pharmaceutical care to make a healthy difference in people’s lives.
