We are excited to offer this combined research and clinical post with a focus on delivering research within our 50 bed Critical Care Service. Our service provides care for critically unwell patients in in the setting of our large London teaching hospital. Royal Free London is a tertiary referral centre for liver failure, liver transplantation, hepatobiliary surgery, vascular surgery, nephrology, renal transplantation, infectious diseases and complex abdominal surgery.

We would welcome candidates from a wide variety of clinical backgrounds including Medicine, Hepatology, Anaesthesia, and Emergency medicine. Airway skills are not specifically required and therefore these posts would be ideal for any candidate wishing to extend their experience in critical care medicine. This post would be well suited to a senior trainee who is taking time out of programme from higher specialist training in order to focus on research and prepare for applications for higher degrees. You will be well supported by our dedicated team of Critical Care Consultants, and we will help you develop mentorship and support from colleagues in our academic partner UCL.

The Royal Free Hospital Critical Care Service is recognised for training at all levels of intensive care medicine. Educational supervision is provided and attendance at ICU teaching programme is expected. The duration of this post will be 12 months.

Start Date: 7th August 2024 onwards

The Royal Free Hospital has a total complement of 42 Intensive Care beds (mixed level 2 and level 3) an 8 bedded post anaesthesia care unit (PACU). All medical and surgical specialties, with exception of neurosurgery and cardiothoracic surgery and ECMO are covered. The Renal unit at the Royal Free provides services for the whole of the North Central sector. Tertiary specialist services include liver transplantation, hepatobiliary (HPB), plastic and vascular surgery. With the recent expansion and amalgamation of HPB services at the Royal Free Hospital, we are now one of the largest centres in Europe in terms of these surgical procedures.

The total throughput of patients is in excess of 1600 per year. Senior medical cover is provided by 24 Consultants, who provide a 24 hour service.

Senior Clinical Fellow (s) in Intensive Care Medicine

We are excited to offer an opportunity for a Senior Clinical Fellow(s) who wishes to gain further experience of Intensive Care Medicine in the setting of a large London teaching hospital.

We would welcome candidates from a wide variety of clinical backgrounds including Medicine, Anaesthesia, and Emergency medicine. Airway skills are not specifically required and therefore these posts would be ideal for any candidate wishing to gain experience in Intensive care medicine and the recognition and management of the critically ill patient. Previous ICU experience is not required. You will be well supported by our dedicated team of 24 ICU Consultants.

We admit approximately 2,000 patients each year and senior medical cover is provided by 26 Consultants, who provide a 24-hour cover worked in blocks to provide continuity of care.

The post is for 6-12 months, commencing February 2024 and is based at the Royal Free Hospital site in Hampstead, London.

Start Date: August 2024 onwards.

The Intensive Care department at the Royal Free is recognised by FICM for training in Intensive Care Medicine at all levels. In participating in the clinical activities on the ICU, the post holder will expand on his/her experience of:
• Resuscitation of the sick patient
• Communication with the multi-disciplinary team
• Criteria for admission and discharge from critical care
• An EBM approach to, and an application of care bundles, to the management of critically ill patients.
• Recognition and management of the critically ill patient
• Recognition and management of acute medical emergencies
• Supporting the sick patient to prevent further deterioration and hence the need for admission to Intensive Care
• Circulatory support, advanced respiratory support and renal replacement therapies
• Practical procedures including arterial and venous cannulation, the use of ultrasound
• The interpretation of complex monitoring used in intensive care
• The management of patients following complex major surgery
• Identification of patients who will benefit from Intensive Care
• Management of complex ethical issues and the delivery of difficult news to relatives

We believe this post offers an ideal opportunity to enhance their higher surgical experience by being exposed to all aspects of HPB surgery with an opportunity to enhance skills in HPB Surgery and Liver Transplantation. The Royal free has a busy Liver Transplant and HPB surgical Unit and is one of the most prestigious medical institutions in the United Kingdom. The work of the unit consists of complex biliary and pancreatic procedures, liver resections, abdominal sarcomas, and liver transplantation. The specialist nature of the work includes minimal access surgery and a living related liver transplantation programme. The successful candidate will be expected to work at a senior clinical fellow level within this team and will be required to work in the operating theatre, take part in daily ward rounds, assist in out-patient clinics, participate in MDT meetings, and perform other relevant work as directed by the Consultants. The post-holder will also be required to participate in the sub-specialty on-call rota for liver transplant (supported by a consultant). This post is EWTD compliant and attracts a banding of 1A. It is not recognised by the Deanery for training, however, opportunities may be available for some of the post-holders to join the active postgraduate research programme within the department including, where appropriate, registration for a higher degree. The Royal Free also runs simulator training courses in advanced laparoscopic surgery in addition to having an active clinical programme in minimally invasive surgery. The successful candidate will be encouraged to make use of all the post-graduate training facilities and the diverse range of management and clinically orientated courses. be kept and form part of the assessment of completeness of training.

A Senior Clinical Fellow (IMT3+, registrar grade) post is available in Acute Medicine at the Royal Free Hospital, London, Hampstead site. The posts will be available immediately for a fixed term period, for a minimum of 12 months, with the option of extending if mutually desirable. The role will be split into 80% clinical and 20% protected non-clinical work with a focus on the development of Same Day Emergency Care (SDEC) services.

Clinical work will involve both leading and supporting the medical take, ongoing care of inpatients based on the Acute Medical Unit (AMU) and the Adult Assessment Unit (AAU) and working within the Medical Same-Day Emergency Care (SDEC) department. Non-clinical work will focus primarily on the development of the Same Day Emergency Care service with additional opportunities to participate in the department’s teaching, audit, governance, and educational activities.

This is an ideal post for bridging the gap between core and specialty training, or for trainees who wish to gain more experience in acute care and service development. There will be support for signing off IMT 3 equivalency with the alternative certificate to enter group 1 higher specialty training.

We are looking to appoint 2 senior clinical fellows in Geriatric Medicine at the Royal Free Hospital.

This is an ideal opportunity to work in a busy but well supported department with a team of highly motivated consultants and trainees. The successful applicants will gain experience across a wide variety of medical conditions and will have access to the full range of in-house educational opportunities afforded to other trainees.

These posts will be well suited to those looking to bridge the gap between core and speciality training in acute medicine and geriatric medicine or for trainees who simply wish to gain more experience in both these fields. Our department has a well- developed front door admissions avoidance service as well as integrated services such as care home and community geriatric medicine.

The successful applicants will be able to participate in the department’s teaching, audit, and educational activities. In addition, the department will be able to support exposure to other specialist clinics.

The clinical fellow will primarily be based on the geriatric medicine wards with responsibility for the ongoing care of patients on the 32-bedded ward. The exact pattern of working days will be decided with the successful applicants with the aim of providing as much cover and continuity as their resource allows. They will enable rapid discharge and provide timely review of patients under their care. The clinical fellow will be directly supervised by a senior trainee in addition to the ward consultant.



The post holder will participate in HSEP ward rounds and multidisciplinary meetings. They will develop skills in identifying and managing frailty, delirium, and dementia. Working with the ward MDT and community teams they will develop person-centred plans for the management plans for in-patient care and post-discharge.
