Neonatal Transitional Care Lead Key Result Areas Offer professional advice and support to women whose babies require NTC within the Maternity Inpatient services. Identify deteriorating babies and take appropriate action to manage the situation, liaising with the multidisciplinary team as appropriate. Work, in partnership with parents/carers to assess, teach, support, and guide them to carry out effective care of infants requiring a higher level of supervision on postnatal wards. Safely administer prescribed medications and monitor their effects. Provide NTC with the aim of supporting mothers as the primary care providers for their babys routine newborn care, including other aspects of care that do not require admission to the Special care Baby Unit (SCBU) but can be provided on NTC. Support initiatives towards achieving and sustaining NTC in line with the Trusts guidelines, striving to raise the care provided in line with the BAPM Framework for Transitional Care. Support the weekly review of term admissions to the neonatal unit identifying themes and trends to inform improvement projects. Attend and present term admission cases at the joint weekly perinatal meeting. Enter babies admission episodes on Badger Net and include NTC activity in quarterly reports to inform future planning. Audit practice against best practice standards, develop regular reporting on progress to the Lead for the service and Maternity Management Team. Act as a resource for staff in both hospital and community settings. Maintain own clinical competency in respect of NTC, wider nursing practice and opportunities for professional development. Develop and sustain resources for use by staff and mothers that support and promote NTC. Maintain an active role in clinical governance initiatives. Investigate incidents, reported via the Trust Datix systems, related to NTC, such as where babies have been re-admitted requiring phototherapy and audit re-admissions requiring NTC. Have in-depth knowledge and understanding of local and national NTC strategies and directives and disseminate this to South Warwickshire University (SWFT) healthcare professionals and staff as required. Be responsible for the assessment of care needs and plan follow-up of any babies identified as requiring NTC in the postnatal period. Knowledge, Skills and Experience Providing and delivering education for relevant grades of staff ensuring the curriculum delivered meets the NTC requirements, including monitoring the education data base of staff training. Identifying training needs for anyone involved in caring for babies on TC with regard to all aspects of NTC, in collaboration with the Practice Development Team. Ensuring effective use of the Trust Mandatory Training database to monitor staff training needs and that attendance is adhered to. Identifying and devising plans to train staff for other care pathways that should be delivered in NTC as outlined by BAPM framework. Identifying and meeting own learning needs. Maintaining own clinical competency in respect of NTC and wider nursing practice, utilising opportunities and professional development. Demonstrating personal commitment to attaining educational, research and clinical skills and to sharing expertise and experience with colleagues both within the team and from other clinical areas. Measurable Result Areas Maintaining up-to-date procedures, policies and guidelines applicable to the service and writing NTC reports as required. Ensuring that the NTC service meets CNST Safety Action 3 - demonstrating that transitional care services are in place to minimise separation of mothers from their babies and to support the recommendations made in the Avoiding Term Admissions into Neonatal units Programme. Responsibility for auditing NTC care pathways so that all babies identified as requiring NTC care are able to receive care based on acceptable standards. Clinical Governance Nursing Lead Key Result Areas Demonstrate visible accessible leadership and support the senior neonatal nursing team to ensure clinical governance is embedded within the Neonatal unit. Develop and implement policies and procedures related to neonatal patient care in collaboration with the medical and nursing team. Ensure compliance with all regulatory requirements and standards of care, including those related to patient safety and infection control. Work closely with the senior nursing team to improve areas of performance, communicating the need for change and devising a plan of how this is to be achieved. Monitor and analyse quality metrics related to patient care, such as patient outcomes, length of stay, re-admission rates, NNAP data and develop strategies to improve performance. Support the service to improve parent feedback surveys and staff survey response. Knowledge, Skills and Experience Leading and supporting others whilst undertaking Serious Incident Reviews and Root Cause Analysis, producing reports and action plans for the Maternity and Neonatal Senior Management Team meetings. Supporting the senior nursing team in the achievement of all performance related targets, including CQC targets and locally set commissioner and local authority targets. Working closely with the Patient Safety Team and being an active member of the Patient Safety Review Team providing expert input to the group. Supporting clinical nursing shifts in all areas on the Neonatal Unit at times of high acuity under the direction of the management team. Measurable Result areas. Developing and implementing policies and procedures related to neonatal patient care in collaboration with the medical and nursing team. Attending monthly Quality Improvement and Patient Safety (QIPS) meetings, Maternity Neonatal meetings, weekly review meeting of reported incidences (Datix) to facilitate and support the group by ensuring that the groups performance, audits and mortality reviews are achieved in a timely manner. Ensuring all risks within the group are assessed when identified and managed in line with the Trusts Risk Assessment Policy. Providing organisational and administrative support for peer reviews and other external reviews including monitoring action plans and reporting exceptions. Managing sometimes conflicting workload and priorities across different agendas ensuring that internal and external deadlines are met. Adhering to trust-wide standard operating procedures and policies developed for processes as well as devising, documenting and managing new policies and processes. Managing and prioritising work in line with agreed schedules and in accordance with departmental professional standards.
