Further your career in a position that’s more than just a job. Join our team and become an integral part of Polln, our digital clinic, as you guide patients through virtual treatment plans, prioritise medical assessments, and deliver comprehensive care.
As a registered nurse, you’ll work closely with medical teams and facilitate interdisciplinary care that caters to each member's unique needs. Your key responsibilities will include managing caseloads, offering ongoing support, and educating patients on doctor-prescribed treatment plans, including dosing and titration.
As a respected link between patients and doctors, you'll also provide assistance with complex cases, address symptoms or side effects, and collaborate with internal and external healthcare experts.
About You
If you're a Registered Nurse with over 4+ years of healthcare experience, fuelled by a genuine passion for patient care, problem-solving, and taking ownership of your work, we want you on our team.
Adaptability is your second nature, and you're excited about building your career within a national digital health organisation. You’ve got a keen eye for detail, exceptional communication and writing skills, and you’re committed to achieving the best clinical outcomes.
You've got previous experience in a GP clinic, can conduct thorough patient assessments, maintain accurate records, and are comfortable with various computer systems.
This is a full-time position (5 days per week) based in Hawthorn, Victoria. Salary commensurate with experience.
Role Responsibilities
Serve as a crucial link between patients and doctors, managing escalations, addressing inquiries, completing clinical assessments, and escalating issues when necessary for optimal patient outcomes.
Provide patient support through digital solutions, optimising care and treatment plans, with a specific focus on chronic conditions.
Empower patients with medical education for effective chronic condition management, including prescribed medications and treatment plans.
Acquire and retain patients, providing clinical education on individual eligibility and government compliance processes (TGA).
Support clinical initiatives through data collection, analysis, and reporting trends.
Ensure precise and prompt medical record-keeping with detailed and accurate documentation.
Explain and support patients through the intricacies of medicinal cannabis treatments.
Required Experience & Qualifications
AHPRA Registration as a Registered Nurse with a minimum of 4+ years of experience in chronic disease management within a healthcare setting.
Deep understanding of chronic disease pathophysiology, pharmacodynamics, and the importance of integrating holistic models of care.
Proven abilities in communicative excellence, collaborating effectively with diverse healthcare professionals for comprehensive patient care.
Desirable
Experience with medicinal cannabis treatments and care plans.
