The Disability Carer Coordinator will provide support to people with disability, using a flexible, responsive, and individualised approach to assist participants in meeting their individual and articulated goals. The Disability Carer Coordinator is responsible for ensuring that Care Plans are executed and overseeing the work of a team of Disability Carers for each service outlet.
This role will require working in a diverse range of settings including, but not limited to, a person’s home or place of residence, other services, community groups and other community settings.
MAIN DUTIES/RESPONSIBILITIES:
Collaborate with clients (and relevant stakeholders such as their family members and the Disability Carer) in the development of their individualized Care Plan;
Coordinate the intake of new clients into services and ensure vacancies are minimized;
Assist clients to undertake a range of daily living activities, such as attending appointments, shopping, and engaging in community and social activities;
Assist clients to undertake personal and self-care activities, such as showering, dressing and eating;
Assist clients to undertake domestic responsibilities in their home, such as cooking, cleaning and managing their household duties;
Assist clients who have high physical needs and/or behaviours of concern.
Support clients to use their preferred method of communication and assist them to be understood, listened to and responded to at all times;
Undertake rostering of Disability Carers, ensuring that clients have appropriate in-home and/or community care; and
Provide on-going training and support to their team of Disability Carers in the execution of their care duties.
Essential Criteria:
Possess a positive attitude and a friendly personality
Highly developed interpersonal, verbal and written communication skills
Exceptional time management and organisational skills
Professional presentation and demeanour
Intermediate to advanced Microsoft office skills (Word, Excel, PowerPoint and Outlook)
Self-directed, organised and able to meet deadlines.
Ability to display discretion and sound judgment with confidential and sensitive information.
Desirable Criteria:
Minimum Certificate III in Disability Services or equivalent.
Previous employment experience in the disability/community services sector
Proven experience in working with people with disability, including psychosocial disabilities, and providing quality services that assist individuals to maintain their independence and participate in their community.
Proven experience in working within an Individual planning process for people with disability within the context of the National Disability insurance Scheme (NDIS).

Full-Time $75,000 to $85,000 per annum plus Superannuation
