GENERAL SUMMARY:
To work collaboratively with members of an interdisciplinary team in order to assist patients and families with complex psychosocial needs. These interventions may include, but are not exclusive to: alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).
ESSENTIAL DUTIES:
Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan. Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient’s needs. Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs and psychosocial imperatives. Provides consultation and resources to members of the healthcare team. Responsible for maintaining relevant and current knowledge of community resources. Participates or assists in department or hospital committees, projects etc., as assigned/requested. Develops and maintains current knowledge of federal and state regulations as they pertain to role.
Inpatient psychiatric duties: (when exists)
Conducts daily group therapy sessions with eligible inpatients and Partial program patients and also may perform utilization review for this patient population Provides discharge planning and utilization approvals for post inpatient care.

Practice and Culture
Family medicine includes patients from newborn to end of life. 20% of the patients seen here are non-English speaking (Vietnamese and Spanish). The position is responsible for tracking and managing all referrals with the intention and outcome to close any patient care gaps, along with providing documentation to promote team awareness.
CORE FUNCTIONS
1. Schedules appointments for specialty physician services and diagnostic testing per provider request and communicates with the patient on a timely basis for all scheduling requirements
2. Acts as a liaison between patients, providers and staff members for patients’ referral and follow up needs. Provides prompt and professional service for the patients by assisting in educating patient/family and assisting patients with external resources when needed.
3. Provides all pertinent clinical information needed for the payor authorization, the facility or specialist prior to the services being rendered by verifying coverage, obtaining authorization and communicating with receiving facilities
4. Tracks and manages all referrals by monitoring outstanding referrals at 30/60/90 day intervals and following up with patients if appointments are not kept
5. Applies knowledge of medical terminology and maintains up to date knowledge of insurance environment. Utilizes internal and external resources to seek knowledge about regulations regarding various payor sources
6. Collaborates with outside referral sources and other community resources. Maintains an updated list of community resources and networks with colleagues to develop additional referral sources
7. This position has frequent communications with patients, physicians, staff, and third party payers. The position must work with and understand the concepts of managed health care and be able to prioritize tasks within established guidelines with moderate supervision
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Possesses 1 year of direct patient interaction/experience in a medical, insurance, or healthcare related industry.
Knowledge of HIPAA regulations. Strong customer service focus and willingness to problem solve. Effective verbal and written communication skills and the ability to manage competing priorities. Must be proficient with commonly used office software.

Assists providers with exams and minor in-office procedures which could include taking patients’ blood pressure, documenting respiration and heart rate/pulse, taking temperature, as well as performing phlebotomies, ear irrigation and EKG’s. Acts as chaperon as needed. Uses universal blood and body fluid precautions at all times and personal protective equipment as needed. Responsible for prioritization of patient flow through clinical process.
3. Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.
4. Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.
5. Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.
6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).
7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.
8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.
9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician’s offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge. Completion of an approved medical assistant training program as defined by state regulations.
Active Medical Assistant Certification is required. Certification or additional training must meet the requirements for Meaningful use. BLS certification required. Active Medical Assistant certification such as: Registered Medical Assistant (RMA) with proof of current membership from American Medical Technologists (AMT) or Certified Medical Assistant (CMA) with proof of current membership from American Association of Medical Assistants (AAMA) or Certified Clinical Medical Assistant (CCMA) with proof of current membership from National Health Career Association (NHA) (Test administered after 7/1/2017 only) or National Certified Medical Assistant (NCMA) with proof of current membership from National Certified Competency Testing (NCCT).

The Specialty Pharmacy Technician & Benefits Coordinator is responsible for completing necessary functions to ensure optimal reimbursement for high-cost medications in a specialty, outpatient pharmacy. Duties and responsibilities include: performing tasks related to the management of third-party billing, insurance verification, prior authorizations, and financial support.

Responsibilities:
Accurately enter in prescriptions to the pharmacy’s electronic system.
Review and screen new patients at intake to determine if patient has adequate prescription coverage.
Provide customer service and monthly adherence calls through the telephone.
Function as primary third-party billing resource for Outpatient pharmacy staff.
Troubleshoot and resolve billing rejections and subsequently rebill claims to appropriate carrier for all commercial, federal, and state insurance plans.
Submit prior authorization requests, follow up on prior authorization requests, and submit claims with approvals as appropriate.
Identify patients who may qualify for patient medication assistance programs, grants or copay assistance.
Contact payers for lost medication overrides, replacement, and vacation override approvals.

Establish and maintain good working relationships with specialty Outpatient Clinic staff to maintain effective communication regarding third-party billing issues.
Assist in the development, implementation, and evaluation of third-party billing policies and procedures.
Educate pharmacy staff about third-party billing policies and procedures as needed.
Draft various correspondence letters and send to referral source when necessary.
Gather and document demographics, insurance information, needs assessment, and other data.
Communicate the anticipated self-pay portion (co-payments/deductibles/co-insurance) with patients and establish payment arrangements based on the Financial Policy and document the financial resolution in the registration/billing systems.
Participate in ordering of medications and pharmacy supplies.
Assist in filling all specialty medications.
Participate in educational programs to meet mandatory requirements and identified needs with regard to job and personal growth.
Participate in performance and quality improvement activities as assigned by Specialty Pharmacy Manager.
Assist in oversight of compliance of the 340B Drug Discount Program.
Ensure appropriate storage of all medications and assist patients with accurate delivery of medication via courier, pharmacy pick-up, or mail.
