The Sioux Falls Veteran's Affairs Healthcare System (SFVAHCS) Community Care (CC) Registered Nurse (RN) Home and Community Based Services Case Manager functions independently and has a high level of initiative, innovation, and resourcefulness. The HCBS team is focused on Aging in Place and the 4Ms (What Matters, Medication, Mentation and Mobility) adapted by the Age-Friendly Health System Movement and encouraging restorative care versus dependence. The RN works as part of an interdisciplinary leadership team guiding this care across the facility. The case manager is responsible for coordinating the care of Veterans receiving both internal VA and external community care-based care within any of the Home and Community Based Services including but not limited to skilled home care, non-skilled home care, adult day health care, and respite care. This involves ensuring there is not duplication of services between internal VA programs and community-based care. Works with interprofessional team members at VISN and facility levels to ensure services and eligibility determinations are delivered objectively with strong, consistent reliability with documentation and communication of outcomes in a direct and coordinated manner. He/she is responsible to coordinate the episodic care of Veterans, who are seeking treatment using a community provider or need urgent referral for community services. He/she will serve as a resource to the community vendors regarding VA processes in using Third Party Administrator and Mission Act. Performs care coordination daily for the CC Veteran population including low and highly complex care needs which is also defined further as performing case management, direct communication with interdisciplinary teams (IDT), chronic disease management and monitoring, and coordination of needed services. He/she will assist in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and services to improve the quality of Veteran care in the community and assist with seamless transition of the Veteran back into the VA system. The CC RN HCBS Case Manager is responsible for providing clinical review and have the expertise for the coordination of the SFV AHCS CC authorization program. He/she possesses intimate knowledge of the CC federal regulations, VHA guidelines, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. He/she supports the SFVAHCS mission and works collaboratively with all IDT services. The CC HCBS Case Manager utilize all avenues of resources available to provide safe, efficient, and timely coordination of quality health care services for Veterans. The Registered Nurse (Risk Manager) is responsible for the oversight, coordination, implementation, and evaluation of all facets of an effective Risk Management Program across all William Jennings Bryan Dorn (Dorn) VA Medical Center (VAMC) programs, services, and facilities which provides clinical and administrative recommendations relative to clinical risk management functions in accordance with regulatory agencies, VHA, Network and facility requirements.

The incumbent executes position responsibilities that demonstrate leadership, experience, and creative approaches to provide an effective Enterprise Risk Management Program and to the delivery of quality patient care. This includes responsibility for the daily management and/or oversight of Risk Management activities including interpreting VA handbooks and directives related to risk management; day-to-day coordination and implementation of Occurrence Screen processes, case management of Peer Reviews and mortality data tracking; Institutional Disclosure process oversight; processes relating to Tort Claims, State Licensure, and National Practitioner Data Bank reporting, including tracking and trending; process management and oversight for Administrative Boards of Investigation, and formal fact findings.

The Registered Nurse (Risk Manager) reviews all risk management issues that relate to the facility; maintain necessary records; and ensure that issues are handled in an appropriate, accurate, and timely manner. The RN serves as a liaison to District Counsel for technical-legal clinical and administrative issues, facilitator/consultant for Root Cause Analysis (RCA) teams, facilitator/consultant for process improvement teams, and consultant for policies and procedures involving risk management related issues. Provides direct staff support to service chiefs, and others in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and/ or services to improve the quality of patient care and the appropriateness and timeliness of services provided. Collaborates with the Patient Safety Manager to trend and analyze patient incidents thus ensuring the integration of the Patient Safety and Risk Management Programs.

• Responsible for the operation of a clinical laboratory with ancillary testing performed on site and at five Community Based Outpatient Clinics (CBOCs). The complexity index group 2 hospital laboratory operates 7 days per week, 24 hours per day, 365 days per year and performs over 1,000,000 tests per year. The laboratory performs and/or is responsible for waived, low, moderate and high complex testing. Plans, implements, controls and supervises all administrative functions of P&LMS including decision making authority. The incumbent has full supervisory responsibility for the staff consisting of Medical Technicians, Medical Technologists, Administrative staff, Lead Technologists, Supervisory staff, LIM, Ancillary Testing Coordinator, and Histology Technicians.
• Uses advanced knowledge of concepts, principles and methodology of a major clinical laboratory program and operations to assess program effectiveness and provide authoritative guidance for operations, personnel and management. Utilizes information to plan and execute short and long range programs.
• Conducts annual performance ratings and evaluations.
• serves as a Contractor Representative (COR) for contracts for services outside the Medical Center. As such, the incumbent collaborates with Contracting Officers and Contracting Liaisons in writing Statements of Work/Performance Work Statements and evaluating proposals that impact the service line. Additionally, the incumbent is responsible for developing monitoring procedures for contractor performance and compliance as well as verification and authorization for payment of bills for services.
• Secondary P&LMS Contracting Officer Representative (COR) and performs contract authorization within delegated authority and provides technical expertise, both before and after contract award. The incumbent is responsible for developing the statement of work, pre-solicitation paperwork and monitoring contract compliance.
• Designs, develops and establishes quality assurance programs and measures, including training in the care, calibration, standardization and use of laboratory equipment; participation in proficiency testing programs; and standardization of procedures for managing supplies and equipment.
• Identifies training needs of employees by consulting with section leads and supervisors. Training includes new employee orientation, competency re-training, in-house education modules, off-site workshops, visits to other institutions to evaluate operational equipment and procedures, and visits to manufacturing sites for instrument training. Consults with specialists and vendors to develop and provide training to employees.
• Responsible for service preparation for external reviews which include Joint Commission, OIG, CAP, FDA, AWE, AABB, NEO, etc., which includes completing survey questionnaires, updating and revising appropriate policies, and assuring that departmental records are accurate and maintained appropriately. Ensures staff is trained on accreditation rules and regulations.
• Responsible for the activities of the service with the respective departments of the affiliated medical school, the current reference laboratories, and other VA hospital laboratories. Prepares contract requests, fee basis testing, and prepares sharing agreements in which the service participates. Coordinates prompt resolution of any problems of an administrative nature with other services in the medical center, neighboring universities or hospitals.
• Directs and schedules quarterly, annual, and long-range plans and strategies in order to effectively project staffing and budgetary requirements of a clinical laboratory with an annual budget over $5,000,000, approximately 40 FTEE and a fund control point.
• Manages significant resources including equipment, supplies, personnel, space, and budget; analyzes budget expenditures; identifies workload trends; prepares and monitors budget and control expenditures; approves expenditures on all recurring and non-recurring items; determines staffing needs and projections; and prepares funding and staffing requests to the appropriate committees.
• Interviews candidates for positions in P&LMS and selects for appointment. Assures that all new personnel receive both the hospital and laboratory orientation. Makes recommendations for selection, promotion, and reassignment of personnel.
• Counsels and resolves complaints from employees, individually or as a group when not resolved at the supervisory level.
• Reviews any serious disciplinary cases of employees and disciplinary problems involving supervisory personnel. Recommends action on the more serious cases. Participates in the resolution of Union and EEO complaints within Pathology and Laboratory Medicine Service.

The Nurse Manager (ICMHR/MHICM/STARS) demonstrates clinical expertise, leadership, and creative approaches to management of complex care and patients with DSM 5 diagnoses such as seriously mentally ill and substance abuse clients. In addition, applies skills necessary to identify, coordinate, and/or identify discipline-specific education, as well as training in leadership, professional staff development and emerging needs, to all levels of staff.

The Nurse Manager is a nurse supervisor and provides oversight for staffing, counseling, and disciplinary actions. This position is for the Mental Health Intensive Case Management (MHICM) program and the person will be engaged in administrative nursing practice characterized by both increased complexity of clinical decision making and enhanced skill in nursing management. The Nurse Manager provides leadership in day-to-day operations of MHICM program at the Columbia and Anderson sites. In addition to MHICM, this Nurse Manager will also supervise the RN Case Manager for the STARS (Substance Treatment and Recovery) program. They must engage in recovery principles and assist staff to optimize the resources of the seriously mentally ill client in community and residential care facilities.

Additionally, the Nurse Manager is responsible for performance improvement, customer service/patient satisfaction, patient and employee safety, interviewing applicants, employee relations, standards of care and practice, the work environment, and accreditation and regulatory standards. Actively develops and manages collaborative partnerships with physicians and other clinicians. Responsible for staffing their units on a continual basis (24/7), validating the daily patient classifications, and working with other Nurse Managers to identify and share resources. Responsible for time scheduling of the unit, coach's staff, deals with employee conduct and performance issues, completes recurring reports and performance evaluations in a timely manner. Participates on service and medical center committees and task forces to improve nursing practice and patient care; Identifies educational needs of staff and ensures that these needs are addressed. Has an integral role in supporting and facilitating student nurse clinical experiences, as well as orienting new Nurse Managers. Works in partnership with the union and adheres to EEO principles. In the absence of a Nurse Manager, a cross-covering Nurse Manager will be designated to handle administrative and clinical issues, such as staffing coverage, granting leave, and patient and/or staff issues on the inpatient psychiatric unit.
