This postholder will work alongside the neuromuscular clinical research team. Key results from the job holder To be a full and active member of a multidisciplinary all age service which focuses upon the health care needs of children, young people and adults who have a learning disability and unmet health needs. This may include challenging behaviour, forensic needs, autism, mental health needs, terminal illness, communication needs, sexual health and health promotion needs. The nurse will have a specific focus on providing specialised nursing assessments, planning intervention and evaluation of programmes of care for those individuals who require specialist support to access health services, those individuals who may be at risk of developing early aging conditions, those children, young people and adults who present with significant behavioural challenges and high dependency needs to themselves, their families and carers and services, and young people between the ages of 14 to 25 who are transitioning from childrens to adult services. To have an understanding of Health and Social care policy and legislation for the service user group in relation to your professional practice. To independently prioritise, maintain and manage a complex specialist personal caseload. To be highly proficient in the assessment, identification, treatment and management of the specialist health and nursing needs of the learning disability population. This includes risk assessment, assessment of complex conditions e.g. challenging behaviour including forensic issues, palliative care, mental health, epilepsy etc. To liaise and communicate effectively and sensitively with other professionals in the MDT and external agencies (e.g. Mental Health Teams, Drug and Alcohol, Palliative and Childrens Services) providing highly specialist advice as necessary. To lead in, implement and evaluate specialist programmes of care which support the health care of individuals or groups with Learning disabilities and complex needs. Lead in the co ordination of complex case work as a case manager for individuals. To have a good understanding of the specific needs of the service user group including at times of crisis. E.g. during placement breakdowns, mental health and physical health relapse, times of transition and periods of challenging behaviour. To work collaboratively with service users, their carers and families as appropriate, to make decisions relevant to the service users individual needs. To recognise the impact of a persons communication needs and tailor interaction to meet this need. To plan, implement and regularly review integrated individual support plans for each client in conjunction with team members as appropriate. To work in service users homes and a variety of community settings where conditions may pose a risk, including lone working. JOB SUMMARY The post holder will be part of a cohesive and enthusiastic multi-disciplinary team and work closely with the other parts of the locality health team and In-Patient team where required. You will be involved in providing specialist assessment, support and treatment for individuals receiving intensive support and care, enabling service users to engage in a wide range of individual and group based treatment activities. The post holder will promote a positive behavioural approach which will be underpinned by professional skills and additional training as required. KEY RESULT AREAS: Clinical 1. To manage an individual caseload, providing assessments for service users receiving in-patient care and within the community, prioritising clinical needs and providing an effective service. 2. To work with service users to identify goals as part of the overall care plan, using appropriate assessment tools based on current evidence based best practice. 3. To plan and implement a client centred treatment programme on an individual and/or group basis, to achieve therapeutic goals. 4. To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. 5. To be responsible for ensuring that the policies and procedures of the relevant Code of Professional Ethics and Professional Conduct and local and national standards of practice are properly implemented. 4. To take a role, in collaboration with other senior staff, in the development of services, including operational planning, and implementation of policy and service development. 5. To be aware of national guidelines and legislation relating to health and social care in your specialist service area. Training/Education 1. To be responsible for a designated caseload, including liaison with other agencies, service users and carers. To utilise nursing knowledge and skills in assessing health care needs of people with learning disabilities in a range of environments to identify their health-care needs, and implement nursing interventions as required. To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved. Any interventions must be within the post holders scope of clinical practice ensuring that training is maintained and skills are in line with current guidance. To provide specialist advice and training to paid and unpaid carers about conditions associated with learning disabilities, to enable them to improve and enhance their caring role.
