Duties and Responsibilities Deliver a number of change projects across strategic, frontline and support functions, driving progress and delivering stated outcomes Identify all potential partners and stakeholders and ensure that they are fully engaged in the development of transformation projects Work collaboratively with clinical and non-clinical staff, commissioners, providers, patients and service users to develop innovative and workable solutions to highly complex problems and challenges to improve outcomes and delivery, reduce inequalities and improve efficiency and quality Develop and secure agreement from stakeholders on project principles and ways of working to achieve delivery of the project(s) Provide facilitation for project meetings and related events as required Ensure that projects support the development of effective community based care in Lewisham which: Is population based. Expands and strengthens primary and community care Promotes health and wellbeing Provides an integrated response to the needs of the individual Is evidence based and outcome focused Is co-produced with patients, service users, carers and the wider community Adopts a whole system approach Communications & Relationships Secure commitment to change by engaging and managing stakeholder interests and tailoring communications to different audiences in ways that invigorates interest and relays information easily and effectively Identify all potential stakeholders and ensure that they are fully engaged in the development of business cases and subsequent project plans Work with health and care communication leads across the system to develop effective communication on specific projects to a wider local audience Ensure all key stakeholders are engaged in the implementation of projects and informed of progress Develop effective networks and good working relationships with service managers and front line staff across the relevant service areas Promote a culture which is positive, forward looking, innovative, responsive to change and customer focused. Financial and Physical Resources Identify the resources required to secure delivery and implementation of the project Work with workforce leads to assess the workforce capability and capacity required to within the system Manage any agreed budgets to deliver projects and ensure that appropriate budget monitoring and forecasting takes place and that the budget is used efficiently and effectively. Respond to external funding opportunities as they arise and manage all external funding secured. Staff Management and leadership No direct reports but the post holder will need to lead and be comfortable and experienced in influencing colleagues over which they have no direct authority, inspiring people and working towards common goals Maintain constructive relationships with a broad range of stakeholders and senior colleagues Policy and Service Development Ensure that the projects for which the post holder is responsible support and align with wider partnership strategies and policies, including those at a national, SEL and local level Maintain a current knowledge of health and care policy, legislative requirements, research and initiatives that are relevant to each project Ensure that any service development or redesign takes account of the interface with other services not in scope of the project Analytical and Judgement Skills Collect, analyse and present data on areas for improvement Support evaluation of the agreed activity by gathering the necessary data and undertaking appropriate analysis Monitor performance against agreed indicators and targets to ensure that projects are meeting agreed objectives Project management Drive the delivery of projects, taking a lead role in developing, negotiating and implementing improvements and new approaches Gather insights, prototype solutions and implement creative ideas for change Lead, influence and stimulate change by using redesign and change techniques with individuals, groups and teams in a positive and supportive way Maintain the project plan(s), providing regular reports on progress and risks as required Identify and manage the day to day project actions, issues, risks, enablers and dependencies Risk & governance Identify project risks and mitigation activity and escalate if resolution cannot be achieved by the project group Provide regular reports on activity and progress to the Director of System Transformation, Place Executive Group, LCP Strategic Board and other relevant boards and groups as required. Planning and Organisational skills Produce a detailed action plan and risk register to drive progress and ensure the timely implementation of the project Ensure that projects contribute to the achievement of the vision and aims for community-based care as articulated by the Lewisham Local Care Partnership Ensure that Equalities Impact Assessments are undertaken and the impact on protected groups are fully considered in the development and delivery of projects Autonomy/Freedom to act Provide support on health and care transformation projects with minimal direction while ensuring that the activity aligns with the overall strategic aims and objectives for health and care in Lewisham as agreed by Lewisham Health and Care partners. Responsibilities:

• Oversee and guide teammates to achieve targets
• Assume responsibility for addressing clients and project-related problems, collaborating with teammates to resolve
• Facilitate collaboration with other stakeholders, both locally and internationally, when necessary.
• Offer suggestions to mitigate issues as needed.

Requirements:

• Minimum Degree
• Minimum 3 years of relevant experience in Automotive industry
