Clinical work and leadership The consultant is responsible within the multi-disciplinary team for the assessment and management of people with cognitive problems in Enfield Memory Service. A willingness to be flexible in altering the timetable according to service developments. Communication with the Enfield Social Services Department and adherence to local Safeguarding protocols. Involvement in providing leadership and management to the service. Liaison with Trust services and relevant non-statutory organisations Maintaining good relations with general practitioners and other stakeholders, e.g. Social Services Departments and Clinical Commissioning Groups. The role within the memory service sits adjacent to the Community Mental Health Service for Older people, which the post holder will be expected to liaise with where patients require more intensive support due to risk concerns. There is a pathway mapped for moving patients between CMHT for older people and memory service. There may be instances where liaison with the Psychiatric liaison teams in the local acute hospitals is required, to ensure the patients receive consistent care and also with the mental health for older peoples wards if a memory service patient is subsequently admitted to hospital. To attend regular patient flow meetings within HPFT and relevant Acute Trusts as needed Oversight of clinical development & training of team with team leaders Clinical supervision of the junior medical staff in the team Close working with Enhanced patient flow team, inpatient teams and crisis/community teams for smooth handover of treatment plans when transferring service users Contribute to wider workforce development (teaching/training on MH issues) to general hospital workforce. We are seeking an excellent Clinician who thrives in the busy environment of a modern Emergency Department. Dedicated attending Emergency Physician sessions will be integral to the postholders job plan. These will involve managing patient flow along with Senior Nursing staff, supervising Junior Medical staff, being part of an initial assessment team, signing off their patients and leading in all resuscitation room cases. The senior team believes very much in a hands-on approach to consultants clinical activity, and any new appointment would be expected to act similarly. For a PEM Consultant we would be offering a shared role of up to 50% between paediatric and adult EM. During these sessions the postholder will wear scrubs and will be always available to staff needing assistance. The establishment of this role has been a vital step to improve the clinical safety and efficiency of the department. The trust has developed a Front of House approach to emergency care since 2004. This involves very close working relationships between EM and acute medicine, and the emergency departments act as the single portal of entry for most non-elective patients to ensure consistency of assessment and most appropriate use of in-patient capacity. The ethos is to promote senior involvement in a patients care as soon as possible, allowing early decision-making. You will need to be willing to think about a patients requirements beyond the first 4 hours of care in a way in which you would not be expected to in many other departments. Same Day Emergency Care A new Ambulatory Care Unit at NSECH and provides SDEC 7 days per week as part of acute medicine, surgery and gynaecology. It also houses the Trust virtual fracture clinic. This area is run by senior clinicians who will liaise directly with the Emergency Department team to ensure patients who can be treated through Ambulatory Care are streamlined from the Emergency Department into there and an admission avoided. During the recent pandemic we have developed a front door streaming process run by the senior ED team to direct patients to the most appropriate place. It is recognised that new consultants will have variable knowledge of management issues and techniques. The post-holder will be introduced to this aspect of the post according to their experience and abilities. Attendance at strategic, planning and operational meetings will be expected from the outset. The post holder will also be required to actively participate in strategic and operational projects. More difficult roles such as appraisal, recruitment of staff and personnel management roles will be introduced gradually and participation in trust and regional courses in these areas will be very much encouraged. Overall responsibility for Clinical Care in the Emergency Departments of the Trust lies with the Head of Service, currently Dr Jo Hughes (Emergency Physician).

The Post-holder will ensure clinical outcomes are clearly shared with the GP via primary care Electronic Clinical Record (ECR) e.g., SystmOne/ EMIS, subject to Information Governance arrangements supporting this access. The post-holder will ensure a letter is generated within the primary care electronic record, this can be emailed to HPFT single point of access which will save it as additional information to Paris. Consultation in primary care can be sent as a letter to Specialist Mental Health Service or primary care mental health services. This may utilise HPFT primary care administrative support where available. The post-holder will provide additional support to the primary and specialist care Multi-Disciplinary Teams (MDT) regarding diagnoses, risk formulation and medications. This will include support to integrated multi-disciplinary or multi system team meetings for mental health case discussion in primary care networks. GPs in the PCN can refer Adults with moderate to severe mental ill health or stable severe mental illness with low risk to self and others. Primary Care Mental Health practitioners may discuss patients in the lower (17-18 yrs old) and upper age range (over 65 yrs old) with the post-holder who may offer outpatient appointment, but this will be on case-by-case basis. The service user must be aware of the referral and in agreement with it. Based on need, current presentation, and risk, they may be referred on to different community health services, suchas secondary community mental health services, CRHTT (Crisis Team), SPA, and First Response Service. People are not eligible to access this service if they are at acute risk to self or others; or they meet the threshold for the crisis team; if they are already receiving care from secondary mental health services (unless for supported discharge to community); if they suffer from a first episode of psychosis; if they take medication that requires a shared care protocol such as Clozapine or Lithium. You must ensure the relevant professional registration is maintained, including Section 12, Approved Clinician, and Responsible Clinician status. To attend and participate in the academic programme of the Trust, including lectures and seminars as part of the internal CPD programme. To maintain professional registration with the General Medical Council, Mental Health Act Section 12(2) approval, and to abide by professional codes of conduct. To participate annually in a job plan review with the clinical manager, which will include consultation with a relevant manager to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernisation. To work with local managers and professional colleagues in ensuring the efficient running of services and share with consultant colleagues in the medical contribution to management. To comply with the Trusts agreed policies, procedures, standing orders and financial instructions, and to take an active role in the financial management of the service and support the medical director and other managers in preparing plans for services.

Clinical Participate in the Paediatric Intensive Care Consultant on call rota and be responsible for the inpatient care of children on the Paediatric Critical Care Centre (PICU & PHDU).
