- Incumbent has a considerable amount of responsibility for the overall operation of Hot Springs VA Medical Center Pathology and Laboratory Medicine Service with the laboratory supporting inpatient and outpatient services, Urgent Care services, and Community Outpatient Clinics. - Directs the performance of the laboratory through scheduling, work prioritization and supervision. - Ensures verification procedures used are adequate to determine the accuracy, precision, and other pertinent performance characteristics of testing methods.- Ensures laboratory personnel are performing the test methods as required for accurate and reliable results.- Ensures the laboratory is enrolled in an HHS-approved proficiency testing program for the testing performed.
- Ensures proficiency testing samples are tested as required under CLIA regulations and that the results are returned within the timeframes established by the proficiency testing program.
- Ensures all proficiency testing reports received are reviewed by the appropriate staff to evaluate each laboratory's performance and to identify any problems that require corrective action.
- Ensures an approved corrective action plan is followed when any proficiency testing result is found to be unacceptable or unsatisfactory.- Ensures the quality control and quality assessment programs are established and maintained to assure the quality of laboratory services provided and to identify failures in quality as they occur.
- Ensure the establishment and maintenance of acceptable levels of analytical performance for each test system.
- Ensures all necessary remedial actions are taken and documented whenever significant deviations from each laboratory's established performance characteristics are identified, and that patient test results are reported only when the system is functioning properly.
- Ensures reports of test results include pertinent information required for interpretation.
- Ensures consultation is available to each laboratory's clients on matters relating to the quality of the test results reported and their interpretation concerning specific patient conditions.
- Ensures adequate laboratory personnel with the appropriate education and either experience or training needed to provide quality laboratory services.
- Ensures prior to testing specimens, all personnel have the appropriate education and experience, receive training for the type and complexity of the services offered, and have demonstrated they can perform all testing operations reliably to provide and report accurate results.
- Ensures policies and procedures are established for monitoring individuals who conduct preanalytical, analytical, and postanalytical phases of testing to assure they are competent and maintain their competency to process specimens, perform test procedures and report test results promptly and proficiently, and whenever necessary, identify needs for remedial training or continuing education to improve skills.
- Ensures an approved procedure manual is available to all personnel responsible for any aspect of the testing process.
- Ensures there are documented responsibilities and duties of each consultant and each supervisor, as well as each person engaged in the performance of the preanalytical, analytic, and postanalytical phases of testing, that identifies which examinations and procedures each individual is authorized to perform, whether supervision is required for specimen processing, test performance or result reporting and whether supervisory or director review is required prior to reporting patient test results.
- Participates in applicable cross organizational performance-improvement activities, develops and communicates objectives, and coordinates efforts to integrate patient care and support services.

The Sioux Falls Veteran's Affairs Healthcare System (SFVAHCS) Community Care (CC) Registered Nurse (RN) Home and Community Based Services Case Manager functions independently and has a high level of initiative, innovation, and resourcefulness. The HCBS team is focused on Aging in Place and the 4Ms (What Matters, Medication, Mentation and Mobility) adapted by the Age-Friendly Health System Movement and encouraging restorative care versus dependence. The RN works as part of an interdisciplinary leadership team guiding this care across the facility. The case manager is responsible for coordinating the care of Veterans receiving both internal VA and external community care-based care within any of the Home and Community Based Services including but not limited to skilled home care, non-skilled home care, adult day health care, and respite care. This involves ensuring there is not duplication of services between internal VA programs and community-based care. Works with interprofessional team members at VISN and facility levels to ensure services and eligibility determinations are delivered objectively with strong, consistent reliability with documentation and communication of outcomes in a direct and coordinated manner. He/she is responsible to coordinate the episodic care of Veterans, who are seeking treatment using a community provider or need urgent referral for community services. He/she will serve as a resource to the community vendors regarding VA processes in using Third Party Administrator and Mission Act. Performs care coordination daily for the CC Veteran population including low and highly complex care needs which is also defined further as performing case management, direct communication with interdisciplinary teams (IDT), chronic disease management and monitoring, and coordination of needed services. He/she will assist in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and services to improve the quality of Veteran care in the community and assist with seamless transition of the Veteran back into the VA system. The CC RN HCBS Case Manager is responsible for providing clinical review and have the expertise for the coordination of the SFV AHCS CC authorization program. He/she possesses intimate knowledge of the CC federal regulations, VHA guidelines, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. He/she supports the SFVAHCS mission and works collaboratively with all IDT services. The CC HCBS Case Manager utilize all avenues of resources available to provide safe, efficient, and timely coordination of quality health care services for Veterans. He/she utilizes the appropriate Hierarchy of Care to ensure that CC services are utilized after exhausting available internal VA resources.
