SUMMARY
As a Registered Nurse (RN) Care Manager, your excellent interpersonal, leadership and collaboration skills ensure the appropriate allocation of hospital resources while maintaining the quality of care.
STATUS: Part Time (24 hours per week)

DEPARTMENT: Care Management
SCHEDULE: Monday - Friday, Days
ATTRIBUTES
Requires an Associate’s Degree in Nursing (ASN) and a Bachelor’s Degree in another discipline - or a Bachelors in Nursing (BSN). The BSN is preferred but not required.
Three (3) years of acute hospital care, case management or utilization management experience preferred
Excellent oral and written communication and interpersonal skills
Good understanding of medical management techniques and processes
Knowledge of federal and state regulations (DOH, Medicaid/Medicare)
Strong organizational and problem-solving skills
Knowledge of third party payor and/or managed care payment principles helpful
RESPONSIBILITIES
Evaluation & Review. Evaluate hospital admissions to assure care is medically necessary and delivered at the most appropriate level; actively participate in the concurrent review process and communicate/intervene with third party payors to ensure reimbursement for services are provided while maintaining quality of care
Patient Management. Collaborate with providers, patients and families to address concerns and identify safe, effective discharge plans; intervene to identify and facilitate early referral for discharge planning; facilitate the discharge process by working with the healthcare team to identify alternative solutions to minimize system delays while ensuring a safe discharge plan
Recommendation. Coordinate and participate in the development, implementation and revisions of evidence based clinical pathways and guidelines; participate in driving clinical and administrative compliance with care guidelines and core measures; identify and recommend actions for improvement

The Pharmacy Technician Trainee demonstrates consistent application of knowledge and skills in assisting the pharmacist in the execution of appropriate, safe, efficacious, efficient, and cost-effective pharmaceutical care. The incumbent participates in many procedural aspects of pharmacy practice under the supervision of a licensed pharmacist and is an integral part of the pharmacy team. Caregivers in this position are expected to practice in compliance with their pharmacy technician trainee licensure.Scope
This position supports Pharmacy Services in Community-Based Care and Specialty-Based Care locations. Pharmacy Technician Trainees may not be paid for externship hours required by the educational institution. Trainees in this role work additional hours separate and distinct from their externship in support of staffing need.
Job Essentials
The Pharmacy Technician Trainee demonstrates consistent application of knowledge and skills in assisting the pharmacist in the execution of appropriate, safe, efficacious, efficient, and cost-effective pharmaceutical care. The incumbent participates in many procedural aspects of pharmacy practice under the supervision of a licensed pharmacist and is an integral part of the pharmacy team. Caregivers in this position will receive paid on the job training and are expected to practice in compliance with pharmacy technician trainee licensure.
The Pharmacy Technician Trainee will be enrolled in an approved program and complete coursework to become a licensed pharmacy technician. Upon completion of the program the trainee is required to submit for licensure. It is expected that any trainee will complete coursework within one year of starting the program. Technician trainee will be paid for practical experience. Didactic coursework will be completed by the technician trainee outside of working hours and is not compensated. Trainee may be required to work weekends, nights, and holidays.
The Pharmacy Technician Trainee will prepare medication orders, receive prescriptions, complete required paperwork, assist with billing, stock medication, and deliver medication to patients. Supports patients, pharmacists, pharmacy students, pharmacy technicians, and other healthcare professionals in relevant areas of responsibility. Communicates effectively with all caregivers and stakeholders regarding responsibilities and identified concerns. Competently and effectively learns technical responsibilities as assigned and seeks appropriate training and peer feedback on routine responsibilities. The Pharmacy Technician Trainee understands and complies with state and federal laws, regulations, policies and procedures, and guidelines related to their specific area of practice and participates and supports financial responsibilities of the department, facility, and Intermountain Healthcare. Responsible to provide delivery of medications, information, and services in a professional, timely manner and to seek a team approach to developing and achieving best practices and processes. Consistently attains appropriate supervision and accuracy checks by the pharmacist(s) and clearly understands when a pharmacist is required to verify tasks and activities to deliver detail-oriented, consistent, and flexible care.

This position will be PRN/On-call with no guaranteed hours. You could work 0 to 40 hours in a week and the days may vary.
Minimum Qualifications
Candidate will enroll or be enrolled in an accredited Pharmacy Technician training program
Must apply for Pharmacy Technician Trainee License within 4 weeks of hire date and obtain within 8 weeks of hire
Demonstrated basic computer skills and attention to detail.
