Job summary Care coordinators in the One Weston Care Home Hub provide a dual role: of administration support to the care home clinical team and also of managing and organising a caseload of care home residents looked after by the care home hub, acting as a key portal of communication between both care home staff and clinicians and other external organisations forming part of this interface. The One Weston Care Home Hub is a growing team of multidisciplinary clinicians including GPs, Advanced Nurse Practitioners, Pharmacists and Pharmacy technicians, paramedics, mental health and dementia professionals and community nursing. We also host placements for clinicians in training, and currently look after 23 of our 65 care homes in Weston, with plans to increase this footprint in the very near future. Working with different practices and teams both within the Pier Health family, and outside of our organisation will be the focus of care co-ordinator work, and post holders will need good interpersonal skills and attention to detail to ensure clinicians provide timely and clear management to care home residents, and that changes and actions are completed promptly and communicated clearly. To help patients manage their needs through proactively responding to queries, making and managing appointments, other interventions that progress the patients care journey, holistically bringing together all of a persons identified care and support needs, and explore options to meet these within a single Personalised Care and Support Plan (PCSP), in line with PCSP best practice, based on what matters to the person. Modify and adapt working practices to meet the needs of the patient, where appropriate, providing coordination and navigation through the aid of digital tools for patients and their carers across the health and care services. Work as an effective member of a multi-professional team, giving support to non-registered staff as required, without assuming supervision, education or line management role. Complete documentation (including electronic patient records where used) as per PCN guidelines and Information Governance (IG) requirements, e.g. Data Protection. Support the production and implementation of new working processes within the practice to optimise the quality of prescribing and patient care. To support the overall effectiveness of exemplary coordination and delivery of the MDTs within the PCN.

Key Responsibilities: Provide support with strategies for BPAS roles providing advice and support to the Recruitment Manager and the hiring line managers. Produce and manage job adverts across various channels including social media. To monitor the recruitment inbox and respond to emails in a timely manner. To distribute emails to wider team where appropriate. Support in the sourcing of opportunities for attraction events and take the a leading role in attendance. Identify areas where BPAS can improve and streamline processes. Manage the BPAS pre-employment screening process including Right to Work documentation. Experience of recruiting via social media and managing direct hiring channels. Ability to multi-task and prioritise appropriately.
