An exciting opportunity has arisen for an individual to join our busy and progressive Primary Care Network team as a Practice Nurse. We are looking for either a fully qualified Practice nurse, or a Qualified Nurse looking for a new challenge. Operating out of modern premises our practice is within walking distance of Royal Oak tube station our highly regarded practice cares for 18000 patients in an area of social and cultural diversity. We have 14 doctors, 3 nurses, 3 pharmacists, 4 Health Care assistants, 1 social prescriber, 1 pharmacy technician and 19 admin team members. We also have numerous allied services operating out of the same building, with specialised care for the over 65s, smoking cessation, health visitors and paediatric clinics. We work in West Hill Health Primary Care Network Team which has 10 practices and 80,000 patients. The role would be to work in an established PCN team of over 30 staff members, that will be located in a maximum of 2 practices, but looking after patients from any of our practices. The successful candidate will be a qualified nurse with post qualification experience and strong interpersonal skills and be a keen team player. The role will involve caring for our population in general practice doing roles such as baby immunisations, smears, wound care, travel clinics and chronic disease management. We are happy to support training a hospital or community based nurse in primary care nursing. The role is a full time position and will involve working 37.5 hrs per week. We would also consider part time applications. Oxford Health NHS Foundation Trust is an accelerator site for the Ageing Well national agenda, as part of the NHS Long Term Plan. You will line manage staff in this locality and be involved in supervision and support for Band 6 SLTs, Band 5s including NQPs and SLT TIs, apprentices and students. Oxfordshire Childrens Integrated Therapy Service is made up of Childrens Occupational Therapists, Physiotherapists, Speech and Language Therapists, profession specific Therapy Instructors, Integrated Therapy Assistant Practitioners and an Administrative Team. We are jointly commissioned by the ICB and Oxfordshire County Council. The Service is led by Operational Managers and Professional Leads for each Therapy. The SLT team is made up of Band 5 and 6 Therapists, Technical Assistants, Apprentices, and both Specialist Team Leads and Specialist Clinical Leads (Band 7). We provide Speech and Language Therapy for children and young people aged 0 19 years across Oxfordshire in a range of settings. Job Responsibilities: Financial Management Manage delivery and monitoring of all current contracts, informal business agreements and strategic aspirations set by the partners. Motivate, support and monitor staff in the use of IT; organise and oversee appropriate use of IT by staff and ensure effective training to enhance the digital service offered by the Practice Set targets and monitor standards for data entry and data collection. Ensure that the Practice has effective IT data security, backup, maintenance and disaster recovery plans in place. Liaise with Area Teams regarding systems procurement, IT funding and national IT development programmes. Maintain and periodically update the Practice website accordingly.

Your skills and experiences: This opportunity is available to nurses with leadership experience which does not have to be specifically based in the community. We welcome applications from experienced nurse leaders working in care homes, community hospitals, private hospitals, practice nursing or the acute sector. You will need to work to tight deadlines so that team members prepare for their role in the meeting. Responsible for data quality when updating patient demographics, next of kin, adding referrals, extracting and producing information (including information to patients) in the trust across all of the Patient Administration Systems Maintaining information across multiple complex computer systems, including Microsoft packages, shared drives, EPR , E-referral System, and NHS Spine Portal. Providing support and guidance to GP surgeries, secretaries and consultants on how to use each of the computer systems where necessary. Managing multiple systems is complex and attention to detail is essential to ensure accuracy. Help patients to overcome any boundaries that may prevent them from attending appointments. Provide information regarding transport, wheelchair availability, portering services and interpreting services. Provide communications in a range of formats including large print or braille. Adjust communication techniques supporting those that cannot read, or do not understand or are scared about their appointment. Book interpreters if necessary and ensure patients are aware using the correct communication tools. Where a patient is not responding the GP may need to be contacted by the Appointments Administrator to ask for their assistance in contacting the patient Autonomously organise and prioritise your own workload according to the variable demands of the specialties and the department; adjusting the priorities throughout the day as further work is emailed in to the department or when processing more complex bookings and dealing with both internal and external calls. Liaise with consultants to advise of any issues around slot booking and overbooking of clinics where guidance has not already been provided To have a flexible approach to the departments demands and to assist where appropriate in the provision of cover for other colleagues and departments during emergencies, sickness and holidays.
