Employee plans and organizes work general assistance from the supervisor as to meet local and VA - wide time frames to complete analysis requirements, reports and special assignments and only refers to the non-routine and high technical problems to his/her supervisor. The incumbent is responsible for reporting backlogs, difficulties in meeting deadlines, uncooperative physicians, or difficulty in securing information from clinicians or the record. Employee is responsible for maintaining the confidentiality of information obtained to carry out required duties.
Routinely communicates with physicians, case managers, and other allied health professional staff using sound interpersonal skills and is constantly aware of the sensitive information. Otherwise, works independently making decisions based on guidelines, precedents and regulations and performs duties in such a manner that only minimal supervision is required. Performance is judged in terms of overall effectiveness of the operation through broad general objectives, the adequacy and accuracy of the work, timeliness, and conformance to established procedures, policies, and instructions. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, and works independently under their scope of practice as defined by the individual medical center to directly care for patients in the inpatient or outpatient mental health setting. The primary function of a CPS is to assure the safe and appropriate use of medications, and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; evaluation, dispensing and providing medications, and drug information. The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. The CPS also applies standards relating to all aspects of distribution and control of scheduled and non-scheduled drugs. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. The CPS is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics.

Clinical Pharmacy Services:
-Serves as a pharmacist with advanced scope of practice within the ambulatory care through provision of patient care during new patient & follow up visits for chronic disease states.
-Reviews patient pain medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, & therapeutic outcomes.
Communicates findings with prescribers & provides appropriate alternatives to current treatment plans.
-Documents & ensure completion of clinical interventions, encounters, alerts & consults in CPRS in a timely & professional manner.
-Obtains medication histories from patients & summarizes significant findings in the medical record and/or to the provider.
-Works with other Primary Care/Mental Health/Pain Team Staff & assists in providing education & medication management to patients in both individual & group settings.
-Performs medication use evaluations on behalf of national, VISN & local medical center initiatives, for assigned patients.
-Orders, performs, reviews & analyzes appropriate laboratory tests & other diagnostic studies necessary to monitor & support the patient's drug therapy.
-Monitors for & reports drug errors, adverse drug reactions, allergies, & patient compliance issues.
-Reviews & evaluates requests for non-formulary & restricted drugs for appropriateness & compliance with established criteria where applicable.

Education/Teaching/Training
-Directly involved in the training or support of pharmacy students, pharmacy residents, medical students, psychology interns, fellows & other graduate or post-graduate trainees.
-Responsible for the self-development, mandatory training, & education necessary to maintain licensure, stay current with the practice of pharmacy & Joint Commission & OIG standards, & meet facility training requirements.
-Provides leadership, guidance, & consultation to staff thorough the Medical Center & associated Community Based Outpatient Clinics.
Outpatient Pharmacy Support
-Counsels patients appropriately regarding their medication & documents this in the notes section of CPRS.
-Serves as an authority on drug usage, interactions, over-dosages, & compliance to medical staff, nursing staff, technicians, & students.
-Verifies electronic orders entered by the physician for accuracy while performing a prospective drug utilization evaluation considering drug-drug interactions, drug-allergy contraindications, drug-disease interactions, & dosage form.
-Evaluates & utilizes professional judgment when determining service connection status, applicability of Agent Orange or combat veteran status, & co-pay status of outpatient prescriptions & resets incorrectly marked prescriptions.
-Document information into VA Adverse Drug Event Reporting System (VA ADERS) & follows up as appropriate on unique cases.

Assists in quality improvement & research initiatives. Participates in medical center & service process assessment & action teams & continually seeks to improve services.

The Advanced Medical Support Assistant assists all VISN 21 facilities by managing complex scheduling actions and by ensuring compliance to Office of Veterans Access to Care scheduling directives and practices. The position is required to exercise organizational skills, problem-solving ability, and content expertise in the various scheduling programs and best practices. The A/MSA communicates effectively with field staff and VISN Clinical Resource Hub (CRH) leadership on a variety of administrative priorities related to scheduling, order processing and consult management. Major Duties: Clinical Practice
1) Practices as the Clinical Pharmacy Specialist for the assigned PACT at the main DC facility or Community Based Outpatient Clinic (CBOC). The Clinical Pharmacy
Specialist supervises all pharmacotherapy provided by team providers to ensure that National Treatment Guidelines are being followed in addition to adherence to National Formulary and local policy. The Clinical Pharmacy Specialist will independently manage unstable pharmacotherapy patients in his/her pharmacotherapy clinics, improving the clinic system efficiency and quality.
2) Utilizes the highest level of clinical privileges for Clinical Pharmacy Specialists to manage assigned patients.
3) Interfaces with providers, including physicians, PAs, ARNPs in the design of the veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy.
4) Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used in treatment of the assigned patients.
5) Evaluates the drug literature by analysis of experimental design and methodology, in order to compare and contract therapeutic regimens and roles for new drugs, with a special emphasis in medications used in referred patients.
6) From a verbal or written patient presentation of signs, symptoms, laboratory tests and diagnoses, recognizes and lists important medical problems, disease states, symptoms or abnormal laboratory values that may necessitate altering the therapeutic regimen or which may be induced by drug therapy.
7) Establishes and expands the patient data base through interpreting the drug history, medical history, physical examination and ongoing patient specific monitoring of therapy, with documentation in the formal patient medical record.
8) Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an
appropriate drug and related therapy, and establishing an appropriate dosage regimen.
9) Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters.
10) Under appropriate supervision by a physician and with approved clinical privileges assumes authority to direct therapy, order lab tests as indicated for appropriate patient monitoring.
11) Provides consultation on selection of appropriate therapeutic regimens as a follow-up of non-therapeutic drug concentrations obtained from
ambulatory care and inpatients.
12) Provides highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy,
with special emphasis on Pharmacotherapy.
13) Works to develop, upgrade, and disseminate acceptable criteria for use of formulary and nonformulary medications and other high cost items directed at employing cost effectiveness without compromising the quality of patient care.
14) Provides physicians and other providers a focal resource for discussion of medical problems requiring a pharmaceutical intervention.
15) Interviews each patient conducting a medication history and formulating a pharmaceutical care plan.
16) Identifies all drug-induced problems that may be affecting the patient or contributing to a need for hospitalization. This would include drug side effects, allergic reactions, toxicities, improper dosage regimens, noncompliance, etc.
