Adventist HealthCare seeks to hire a Director of Care Navigation (RN or SW) who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.

As a Director of Care Navigation, you will:
•Develop objectives and operational practices to ensure the Care Navigation team achieves all measures of success including not limited to Patient Experience, Length of Stay, Readmission Rate, and Total Cost of Care
•Ensure collaboration across the continuum of care including but not limited to community providers, hospital providers, ancillary services, Post-Acute Care Center (inpatient and outpatient)
•Possess an expert understanding of insurance/risk contracts so that the director leads performance to achieve cost and quality measures
•Create an environment that generates a high level of employee engagement
•Develop a plan for departmental financial requirements and operates within budget
•Represent department and organization in local and state-wide initiatives
•Role-model positive customer service behaviors through the consistent application and deployment of AHC’s Values and Mission and holds self and others accountable

Qualifications include:
•Bachelor’s degree in Nursing, Social Work, or Health Administration required, Master’s degree preferred
•Minimum of five years of experience in patient navigation, discharge planning, or care coordination required
•Minimum of two years of leadership experience required
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance

Work Schedule:

Primarily Monday through Friday, with oversight of 7-day-a-week business.

As a Director of Wound Care you will:
•Be responsible for the management and the strategic growth of the Wound Care Program and other wound care continuum programs
•Maintain a collaborative and consultative client relationship within and outside the hospital organization and for creating effective working relationships between Healogics and the hospital.
•Manage the Center’s daily operation
•Manage direct reports in conjunction with the company and hospital HR departments. This includes interviewing, hiring, motivating, coaching, counselling, establishing performance expectations, and conducting performance reviews
•Perform Financial Management
•Manage Quality/Performance Improvement functions
•Manage clinical functions
•Manage Relationships
•Manage Community Education/Marketing functions
•Perform other duties as required

Qualifications include:
•Bachelor’s degree in Business Administration, Healthcare Administration, Nursing or related field
•Current Registered Nurse (RN) license
•Five (5) years or more in clinical nursing, clinical operations experience or clinical management
