The Supervisory Social Science Program Specialist duties are as follows:

The CEPC is responsible for community capacity development for suicide prevention by serving in a leadership role in communicating with, organizing and managing community partner interactions and in helping to build coalitions to implement suicide prevention programs aimed at reducing Veteran suicide at the community, state, and Veterans Integrated Service Network (VISN) levels. • Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate.
• Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments.
• Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs.
• Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals.
• Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease.
• Reviews current health behaviors and develops customized treatment plans with Veterans.
• Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure.
• Meets with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans. are consistent with best practices for treatment and management of chronic illness.
• Performs other related duties as assigned.
• Utilizes motivational interviewing and health coaching techniques and processes, learned through pre requisite health coach training, to assist members in developing a realistic plan to accomplish their goals.
• Fosters an environment of individual responsibility to maximize each Veteran's ability to adhere to a lifestyle management plan.
• Develops trust with Veterans and incorporates trust principles in day-to-day activities. Works collaboratively within a team setting to provide positive outcomes for participants, peers and overall operations.
• Maintains education and knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, including behaviors that improve Veterans' health and well-being and those related to poor clinical outcomes.
• Coordinates with others on the Veteran's health care team to ensure optimal patient care.
• Uses VISTA, CPRS, Microsoft office software, Intranet and Internet and other applicable software to track and support patient progress. Communicate proactively and effectively with Veterans, peers and supervisors.
• Conducts outreach activities to inform Veterans and family members of available programs. identified goals.
• Provides educational opportunities for staff-including lunch and learns, grand rounds, and presentations at staff meetings-in order to promote health coaching and encourage referrals to these services.

Starting at $49,309 Per Year (GS 7-9)

Duties include, but may not be limited to the following:

• Establishes and chairs the Employee Whole Health Committee. Assembles a diverse
  planning team of interdisciplinary health professionals and representatives from various
  services within the medical center including labor partners.
• Conducts needs assessment(s) of employees to determine program needs and
  interests. Develops core program components based on assessment results.
• Develops and delivers classes, seminars, workshops, etc. on various Employee Whole
  Health topics including but not limited to nutrition, physical activity, and mental health
  topics or works to identify and collaborates with relevant subject matter experts.
• Creates a personalized health plan (PHP) with employees who desire one by exploring
  the employee's mission, aspiration and purpose (MAP); creates goals and action plans
  with the employee based on his/her MAP.
• Maintains and updates local policies and procedures related to Employee Whole Health
  as needed to ensure compliance with VHA-level directives and regulations, VISN
  guidelines, medical center policy, Union Master Agreements, professional guidelines,
  and all accrediting and regulatory agencies.
• Manages program budget and is the fund control point official for the Employee Whole
  Health program.

Promotion Potential: The full performance level of this position is GS-11.
