3. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations.
4. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges.
5. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.
6. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
7. This position works under local supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with other health unit secretaries and other staff at all levels throughout the facility. Customers are patients, families, physicians, unit staff, case managers, patient service staff and medical records staff. Interacts with a wide variety of external customers.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Requires skills and abilities typically attained with 1 year of medical clerical experience; preferably in a hospital or physician office setting, or prior experience in customer service. Must possess excellent organizational skills, as well as effective human relations and communication skills. Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, Boswell Skilled Nursing Facility or the Whole Health Clinic must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

The Banner Home Care department is looking for a Registered Nurse to join their dynamic team! This position will entail case managing a wide variety of patient in an acute home care setting. Day to day, you can expect to do wound care, IV antibiotic administration and teaching, lab draws, central line care, disease and medication management teaching, comprehensive assessments and much more!
This position is scheduled Monday - Friday 8:00am - 5:00pm providing home visits with a weekend rotation every 2/3 months. Enjoy a flat rate of $3/hour weekend shift differential.

Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. 7. Collaborate with the Program Director, APD, and/or Program Chief(s) to design and implement yearly rotation and vacation schedules for house staff. Work performed is reportable to the Graduate Medical Education Leaderships. Serves on at least one GME subcommittee annually.
8. Maintains accurate records on all current and former house staff necessary to verify successful completion of residency program using the Residency Management Software Prepares all cyclical reports for residents, staff and attending physicians. Maintains discretion relative to confidential and sensitive residency/fellowship issues.
9. Internal contacts include physicians, administration, and employees. External customers include various medical education accreditation agencies and federal, state and local regulatory agencies for medical education.
MINIMUM QUALIFICATIONS
Must possess knowledge of medical education coordination as normally obtained through the completion of a bachelor’s degree in business, human relations or related field.
Must have C-TAGME or acquire the certification within one year. Must possess or obtain active Notary Public commission within 90 days of hire.
Must possess an understanding of graduate medical education as normally demonstrated with five years of experience in graduate medical education.
