General Responsibilities of the Consultant General Surgeon The following is not an exhaustive or exclusive list of the duties of a Consultant Grade. The appointee is expected to: Provide senior clinician services to both the Emergency Department and the General Surgery Department as per job plan or as agreed within the Trust, and as required when on call. Work with consultant colleagues and nursing staff to ensure efficient use of Trusts resources and contribute actively to clinical governance in the department and to development of quality enhancement projects in the unit. Provide senior clinician services to area of special interest as per the job plan or as agreed within the Trust. Show flexibility and undertake different appropriate clinical tasks within allocated DCC sessions at the request of the clinical manager, as the need arises. Actively participate in the Units Teaching Programme and undertake supervision, teaching and training of medical trainees and other multi-professional staff, in line with guidance from the relevant Royal Colleges or specialty bodies. Actively participate in clinical guidelines development, audit and CPD. On-call frequency will be 1 in 12 including weekends (STC) and includes weekend ward rounds to comply with London Emergency Surgical Standards. Unpredictable work is carried out from home, returning to work within 30 minutes as dictated by the caseload. There are 10 SPAs. Thus this post represents 7.5 DCC to 2.5 SPA. Upon agreement with the Divisional Clinical Director, the post holder can take on additional SPA activities. It is envisaged that the post holder will maintain a subspecialty colorectal/ upper GI interest to complement existing services.

The post holder also will provide clinical service and leadership to the recently established regional Thrombotic Thrombocytopenia Service. As such the post holder is responsible for overseeing the Emergency Plasmapheresis Service for newly diagnosed patients with Thrombotic Thrombocytopenia in East Anglia. The post holder is also expected to liaise with the national team and NHS England as well as the district general hospitals referring patients to improved and refine pathways and to improve performance over time through education and improved pathways. In addition the post holder will have regular clinics for Medical Haematology, Thrombophilia and Haemophilia as well as for TTP. The post holder will provide clinical service for General Haematology inpatients on a rotational basis, GP advice and liaison service and cover for laboratory queries in General Haematology and Transfusion both for CUH and Royal Papworth as part of the on-call, support/initiation of clinical research/trials, supervision of Trainees, introduction of new technologies/services in line with Pathology overall strategy as well as lead on regulatory accreditation issues. This is an exciting time within Medical Haematology with a portfolio of trials and clinical research. This is a consultant post in Infection (Medical microbiology or Medical Microbiology and infectious diseases) The service is delivered mainly for 2 acute sites, the University Hospital of North Durham and Darlington Memorial hospital. Based on service needs and in agreement with the appointing manager and applicant, the applicants base will be University Hospital of North Durham OR Darlington Memorial Hospital. The post holder will however be required to work at any location within the Trust. The appointee will be expected to be involved in implementing the Trusts Clinical Governance programme. This includes active participation in clinical audit, quality, clinical guidelines/pathways, and professional development. The post holder will provide, working with other colleagues, clinical advice on medical microbiology to clinical staff in hospitals and in the community, and within the area served by the laboratory and consultants in communicable disease control, environmental health and others. This will include participation in surveillance, hospital infection control and public health microbiology. The appointee will be expected to work with local managers and professional colleagues in the efficient running of the services and will share with Consultant colleagues in providing a clinical microbiology service to the Trust and local CCGs. This post will predominantly cover clinical service but in addition may have the need for the appointee to attend RCA, MDT meetings where required. Clinical Provide appropriate advice and result interpretation on the tests performed within the department under the overall supervision of the Consultant Microbiologist. Liaison with clinicians, hospital staff and GP`s concerning the diagnosis and management of patients and prevention of infection. Liaison with Public Health England and Health Protection Unit to manage infections and notification of notifiable diseases. Attend and contribute to multidisciplinary team meetings as required. Current opportunities include C. difficile MDT, OPAT MDT (Outpatient antimicrobial therapy), Ortho MDT, and Endocarditis MDT To provide medical input and oversight of policies, procedures and guidelines produced with microbiology, or those produced by other departments that impact on microbiology. Provide advice and interpretation of test results to clinicians, either directly, or via participation in the departments duty rota. This post will predominantly cover clinical service but in addition may have the need for the appointee to attend RCA, MDT meetings where required. Out of Hours Responsibilities Once the post holder is established they would be expected to participate in the on call rota. There will be a compulsory on call commitment as part of this post Infection Control The substantive post will involve working with the Consultant colleagues and infection control nurses to deal with day to day infection control management across the Trust. Active involvement in investigation of infection control incidents and in action planning and implementation to reduce hospital acquired infection is expected. There will be a need to be involved in root cause analysis following infection control incidents. There will also be a need to be involved in decontamination issues. Laboratory To be involved in all aspects of the day-to-day operation to UKAS standards of the Clinical Microbiology laboratory with appropriate technical support. Regular daily liaison with the laboratory technical staff Reporting of laboratory results, authorizing and providing medical interpretation as appropriate The appointee will be a responsible for the provision of a comprehensive, efficient and cost-effective diagnostic and clinical microbiology service (including bacteriology, virology, mycology and parasitology). To provide clinical input to the laboratory manager to enable effective management of the automated Clinical Microbiology activities in the Trust including: Advising the section senior on the clinical implications of quality issues (EQA and IQC) that can be used to help inform their action plan. Participate in the introduction of new methods and preparations of business case/plan. The service is embracing new ideas to enable us to support expanding and ever changing demands on microbiology. Share responsibility for quality assurance performance within the department including taking part in all relevant NEQAS programmes. Works with operational members of laboratory staff to implement the departmental policies. Where possible ensures guidelines issued by the MHRA and NICE are compiled with. Assesses the budgetary, clinical and logistical implications of new guidance, whether National, Regional or local and liaise with other departments in order to provide the required level of service. Provide clinical input whilst the department is evaluating new methods or equipment and producing reports including statistical analysis and graphical representations of data. Monitor and advice on budgets and resources in the service as required. Provide clinical leadership and training of staff. Managerial Roles There is the opportunity to take an active role in developing one of the key roles supported by the microbiology clinical team. These include developing and leading: The OPAT service. Antimicrobial Stewardship team Infection Prevention and Control Orthopaedic Infection Diabetic foot Infection service Endocarditis MDT There is also the opportunity for a suitably experienced candidate to include the Microbiology Clinical Leadership role as part of this post. Additional activities would be considered through job planning and allocation of funding (PA) once in post where appropriate. These roles are managed on rotation and the candidate will be required to deliver one or more of these roles. The appointee will be expected to cover colleagues in their absence, and will be expected to deputise in the absence of colleagues in cases of annual leave, study leave and sickness as far as is practicable and ensure continuous patient care Liaison with other Consultant Microbiologists Liaison with other consultants includes attending meetings of the Microbiology Speciality Group and the Northern Region Consultant Microbiology Group (NERMG). MDT- Participation in MDTs is expected. Currently there are MDTs for C. difficile (which is trust wide), Arthroplasty infection and Endocarditis Other roles Teaching The Consultants will be expected to participate in teaching of other junior medical staff, medical students and other staff as appropriate. Research Research activities are considered an inherent part of normal clinical duties and would therefore be encouraged. Research funding may be available for successful applications to the regional research committee. Audit and Clinical Governance The Trust has established Clinical Audit and Governance Departments. Within Pathology there is an active combined research & audit group with representatives from each speciality. It is expected that the appointees will actively participate within departmental audit and support cross-specialty audits

This post is offered on a full time (10 PA) basis although less than full time applications will be considered. Duties will be based primarily on our Wakefield (Pinderfields) site although all our medical staff are involved in delivering care to patients on all our sites. The main duties of the post will include: This post holder will be based at Pinderfields, and along with colleagues will provide Haematology OPD clinics, support to the chemotherapy daycase units and in-patient ward. The post-holder will be expected to join the trust intra-thecal chemotherapy register and help deliver intra-thecal treatments. The post holder will be required to attend the other hospital sites to support out- patient clinics, day case work and on occasions other duties as requested by senior colleagues. There will be the opportunity to develop sub-specialty areas according to the experience and interests of individuals and take an active research role/further develop departmental research depending on the candidates wishes. We would welcome applications from candidates who have a particular subspeciality interests and are happy to accommodate and develop sub-specialty roles. There will be mentorship provided from senior member(s) of the team within the department depending on the successful candidates needs. The post holder will share responsibility with colleagues for the supervision and training of junior medical staff and supporting the laboratory haematology service. The role of Laboratory lead is already taken but all consultant team members contribute with blood film and bone marrow review (although all marrows are formally reported at the haematological malignancy diagnostic service (HMDS) in Leeds), support to the BMS led anticoagulation dosing service and blood transfusion dept. We run a weekly MDT, monthly transplant meeting /MDT and clinic with representation from the Leeds NHSBT and the Regional Transplant Director, which the post holder will have the opportunity to contribute. We also run regular Quality meetings and have a dedicated haematology/transplant quality management team. Urgent and Emergency Care duties including out-of-hours on-call; schedules will take due account of the provisions of working time regulations and safe working On-call duties are currently shared with 6 other colleagues on a {1:6}, currently assessed as 0.886 Category A 5% PA Supporting Professional Activities in accordance with the Trust Framework, with at least a minimum of 1.5 SPAs included in the core 10PA timetable. Additional SPA or DCC time may be commissioned in line with the skills of the successful candidate and changing requirements of the service. As a result of some of the funding for the post being provided from Acute Medicine, the successful candidate will work one session a week as an Acute Medicine Physician. The candidate would be involved in providing an in-reach service for the 42 bedded Acute Medicine Unit to ensure an early review of patients with respiratory failure, respiratory tract infections and pleural effusions. The department is keen to recruit staff who have skills in bronchoscopy, endobronchial ultrasound service (EBUS) and thoracoscopy. The consultant team works closely with specialty trainees and a variety of specialist nurses. There are currently two lung cancer nurses, two COPD nurses who run an Early Discharge Service and two asthma nurses (One WTE). In addition to this, there is also a physiotherapist who supports the bronchiectasis clinics. The post holder will join a substantive multi-disciplinary team and the successful candidate will be well supported working in a friendly, modern and well equipped unit and will be joining a trust with excellent professional and academic facilities. Acute and outpatient gynaecological services are delivered at our Barnet site and elective and outpatient gynaecological services are delivered at our new state of the art Chase Farm site. The successful candidate will be supported by a multi-disciplinary team of gynaecologists, specialist nurses and allied health professionals. The candidate should be keen to work as part of a team and develop good relationships with other staff within and outside of the department. The candidate will have an interest in medical education and will be required to provide dedicated medical student teaching sessions. They will also be required to assist in progressing plans to develop the 4th site for clinical medical training of UCH medical students, in terms of infrastructure and support, governance and placements. They will report to the Departmental Undergraduate Lead in the first instance but may also be required to provide assistance to the Site Undergraduate Tutor and the UCLMS Sub Dean. The appointee will be expected to stay within 30 minutes travelling time from the hospital when on call.
