This is an OPEN CONTINUOUS ANNOUNCEMENT. Applications will be accepted on an ongoing basis and will remain on file through December 31, 2023. Eligible applicants will be referred each month as vacancies occur on an as-needed basis until positions are filled.
The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, and works independently under their scope of practice as defined by the individual medical center to directly care for patients in the inpatient or outpatient mental health setting. The primary function of a CPS is to assure the safe and appropriate use of medications, and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; evaluation, dispensing and providing medications, and drug information. The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. The CPS also applies standards relating to all aspects of distribution and control of scheduled and non-scheduled drugs. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. The CPS is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics.

Clinical Pharmacy Services:
-Serves as a pharmacist with advanced scope of practice within the ambulatory care through provision of patient care during new patient & follow up visits for chronic disease states.
-Reviews patient pain medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, & therapeutic outcomes.
Communicates findings with prescribers & provides appropriate alternatives to current treatment plans.
-Documents & ensure completion of clinical interventions, encounters, alerts & consults in CPRS in a timely & professional manner.
-Obtains medication histories from patients & summarizes significant findings in the medical record and/or to the provider.
-Works with other Primary Care/Mental Health/Pain Team Staff & assists in providing education & medication management to patients in both individual & group settings.
-Performs medication use evaluations on behalf of national, VISN & local medical center initiatives, for assigned patients.
-Orders, performs, reviews & analyzes appropriate laboratory tests & other diagnostic studies necessary to monitor & support the patient's drug therapy.
-Monitors for & reports drug errors, adverse drug reactions, allergies, & patient compliance issues.
-Reviews & evaluates requests for non-formulary & restricted drugs for appropriateness & compliance with established criteria where applicable.
-Promotes & monitors compliance for established drug therapy policies.
-Works with providers to ensure compliance with national, VISN, & local initiatives.
-As required, the Pain CPP will provide tale-medicine assessments & treatment.
-Refers patients to another appropriate clinic and/or provider.
-Educates patients on their drug therapy with respect to compliance & proper administration, signs of toxicity, adverse effects, & interactions with other medications, inquiries about side effects & refill history, evaluates patient's ability to understand instructions, provides verbal & written instructions to promote patient medication adherence, documents all elements of patient consultation.
-Participates in Professional Peer Evaluation.
-Recognizes suicide risk factors & understands facility policies regarding suicide prevention, support systems, & referral resources.
Education/Teaching/Training
-Directly involved in the training or support of pharmacy students, pharmacy residents, medical students, psychology interns, fellows & other graduate or post-graduate trainees.
-Responsible for the self-development, mandatory training, & education necessary to maintain licensure, stay current with the practice of pharmacy & Joint Commission & OIG standards, & meet facility training requirements.
-Provides leadership, guidance, & consultation to staff thorough the Medical Center & associated Community Based Outpatient Clinics.
Outpatient Pharmacy Support
-Counsels patients appropriately regarding their medication & documents this in the notes section of CPRS.
-Serves as an authority on drug usage, interactions, over-dosages, & compliance to medical staff, nursing staff, technicians, & students.
-Verifies electronic orders entered by the physician for accuracy while performing a prospective drug utilization evaluation considering drug-drug interactions, drug-allergy contraindications, drug-disease interactions, & dosage form.
-Evaluates & utilizes professional judgment when determining service connection status, applicability of Agent Orange or combat veteran status, & co-pay status of outpatient prescriptions & resets incorrectly marked prescriptions.
-Document information into VA Adverse Drug Event Reporting System (VA ADERS) & follows up as appropriate on unique cases.
Program Evaluation
Assists in quality improvement & research initiatives. Participates in medical center & service process assessment & action teams & continually seeks to improve services.
Other Significant Responsibilities
Serves on Medical Center or Healthcare System, VISN, and/or National VA committees or task forces as appointed or assigned.

Major Duties: Clinical Practice
1) Practices as the Clinical Pharmacy Specialist for the assigned PACT at the main DC facility or Community Based Outpatient Clinic (CBOC). The Clinical Pharmacy
Specialist supervises all pharmacotherapy provided by team providers to ensure that National Treatment Guidelines are being followed in addition to adherence to National Formulary and local policy. The Clinical Pharmacy Specialist will independently manage unstable pharmacotherapy patients in his/her pharmacotherapy clinics, improving the clinic system efficiency and quality.
2) Utilizes the highest level of clinical privileges for Clinical Pharmacy Specialists to manage assigned patients.
3) Interfaces with providers, including physicians, PAs, ARNPs in the design of the veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy.
4) Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used in treatment of the assigned patients.
5) Evaluates the drug literature by analysis of experimental design and methodology, in order to compare and contract therapeutic regimens and roles for new drugs, with a special emphasis in medications used in referred patients.
6) From a verbal or written patient presentation of signs, symptoms, laboratory tests and diagnoses, recognizes and lists important medical problems, disease states, symptoms or abnormal laboratory values that may necessitate altering the therapeutic regimen or which may be induced by drug therapy.
7) Establishes and expands the patient data base through interpreting the drug history, medical history, physical examination and ongoing patient specific monitoring of therapy, with documentation in the formal patient medical record.
8) Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an
appropriate drug and related therapy, and establishing an appropriate dosage regimen.
9) Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters.
10) Under appropriate supervision by a physician and with approved clinical privileges assumes authority to direct therapy, order lab tests as indicated for appropriate patient monitoring.
11) Provides consultation on selection of appropriate therapeutic regimens as a follow-up of non-therapeutic drug concentrations obtained from
ambulatory care and inpatients.
12) Provides highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy,
with special emphasis on Pharmacotherapy.
13) Works to develop, upgrade, and disseminate acceptable criteria for use of formulary and nonformulary medications and other high cost items directed at employing cost effectiveness without compromising the quality of patient care.
14) Provides physicians and other providers a focal resource for discussion of medical problems requiring a pharmaceutical intervention.
15) Interviews each patient conducting a medication history and formulating a pharmaceutical care plan.
16) Identifies all drug-induced problems that may be affecting the patient or contributing to a need for hospitalization. This would include drug side effects, allergic reactions, toxicities, improper dosage regimens, noncompliance, etc.

Procurement (50%)
Serves as subject matter expert for service contracts for the service line and is responsible for the coordination and monitoring of all contracts assigned. Prepares contract specifications to include complex Statements of Work and obtains accurate and sufficient data to support continuation or cancellation of contractual obligations through quality monitoring. Initiates and completes modifications by the contractor, completes reconciliation of invoices to timecards for accurate and prompt payment, initiates payment and completes increases and decreases to the financial obligations funding the contract. Determines make versus buy to recommend pursuing a medical services contract versus hiring additional providers. Prepares detailed and complex contract solicitation packages for submission to the Veterans Integrated Service Network (VISN) Contracting Office for award of the necessary contract services. Completes necessary documents, packets, quotes, Statements of Work, and other required materials for the
procurement of medical equipment and supplies necessary for patient care.
-Participates in annual budget forecasting, budget submissions and supplemental budget requests. Serve as a Control Point Official and initiates and approves necessary forms and electronic request transactions. Monitors fund control points on a day-to-day basis and requests transfers between accounts as needed throughout each fiscal year to ensure service functions remain within budget allocations. Assembles and complies budget information for annual and quarterly reports.
Resource Management and Planning (25%)
Develops and implements processes to evaluate and monitor administrative activities (contract management, funds management, clinic flow, clinic schedules, staffing efficiency, access, and communication) and makes recommendations for program improvements. Assists and/or supports clinical and administrative leadership in implementing actions designed to improve operations. Performs review and analysis of provider productivity data and develops action plans that lead to improved performance and/or productivity, implementation of management controls, and short and long-range planning. Analyzes productivity to include physician productivity (Relative Value Units, book ability), mid-level provider productivity and utilization rates. Analyzes and monitors provider productivity and assist service line leaders in creating action plans to correct problems that come from this analysis. Performs short- and long-term planning regarding program development, staffing and resource allocation. Performs staff support work in short- and long-range planning efforts and complies with directives and national guidelines in preparing annual and cyclic planning documents.
Project Management Support (25%)
Collaboratively develops strategies for the service to meet its goals and mandates in efficient and effective ways, ensuring alignment with Medical Center strategic initiatives. Prepares and reviews reports involving trend analyses and ensures follow-up in these areas for Specialty Care
programs. Provides support relative to programmatic matters related to the daily successful operation of Specialty Care services in the Medical Center. Develops and implements methods and procedures for collecting, processing, compiling, editing, and analyzing administrative, financial, and clinical data. Identifies administrative problems and takes action to resolve those problems. Uses available data to determine current performance, develops and implements local performance measures as needed to assess current status, and determines areas targeted for improved effectiveness and efficiency. Identifies program inefficiencies and develops studies or data collection techniques related to identified problems. Organizes, compiles, summarizes and analyzes the data collected; develops reports which reflect the scope of activities; and identifies trends and opportunities for improvement. Utilizes individual judgement to determine the most appropriate means to display the data to ensure it is meaningful to those viewing the data. Discusses problem areas with personnel concerned to identify and implement remedial action to be taken. Follows through to ensure program requirements are met and planned actions have been initiated. The Medical Administration Specialist (AOD) is delegated full authority and responsibility to exercise the Medical Center Director's and Chief, Medical Administration Service for administrative activities occurring on other than normal tour of duty hours.
