The post holder will be responsible for the efficient and effective provision of the non-medical endoscopy service in conjunction with other members of the multidisciplinary team. They will work with the MDT and contribute to the development, management and delivery of a high quality general and specialist mixed GI endoscopy service.

1. Upon the location of abnormal pathology, undertake biopsies, photographs and complete pathology requests as required.

1. In collaboration with Clinical Director, Clinical lead, Endoscopy clinical operation manager and other Advanced Practitioners , contribute to strategy for the development of non- Medical Endoscopists to support the Trust performance in achieving all national and local wait times targets and develop guidelines for investigations, drug interventions and the administration of therapies which may be instigated and initiated by non-Medical Endoscopists
2. Be responsible for the validation of surveillance endoscopy waiting lists and initiation of bowel preparation in accordance with clinical guidance and GRS standards for clinical quality.
3. Maintain accurate and up-to-date confidential and timely patient records ensuring that all patient documentation (both written and electronic) is in line with agreed Trust Standards and NMC guidance.
4. Take responsibility for inputting clinical information for individual patient reports following examination to ensure National Endoscopy database is kept up to date.
5. Take responsibility for checking own data on the JETs National Endoscopy database to record own clinical outcomes.

Daily Commitment: Day to day care of the patients on the wards, including transfers from other hospitals and occasional emergency admissions. Attending ward and board rounds and multidisciplinary rehabilitation and discharge-planning meetings, and on certain days attending patients in outpatient clinics and in the Day Hospital. There are four CMU wards, each with 2 FY1 grade doctors and 1 Consultant. You will provide middle grade supervision to the FY1s and report to the consultant. ST3+ trainees are available on CMU wards for advice and support. The Stroke Unit has a daily consultant ward round and there are two ST3+ doctors. The perioperative liaison team consists of a consultant physician and a fellow. The Day Hospital post includes seeing new patients in an outpatient setting.

The Clinical Coding Department at WHTH is undergoing a complete transformation to be more responsive to the changing demands of the organisation, especially, with the Trust adopting Electronic Patient System (EPR) recently.

There is a requirement for high quality data collection and reporting with implementation of EPR to accurately reflect clinical activity and outcomes. This in turn will ensure that our services are set up to support our patient case mix enabling us to provide the right care for our patients.

You will report to and assist the Coding Quality Assurance Lead in designing and delivering a robust clinical coding training programme for WHTH ensuring the highest quality coding data to satisfy NHS regulatory bodies that the Trust exemplifies best practice and promotes a culture of continuous improvement.

You will deliver regular in-house coding training including the mandatory clinical coding standards courses and speciality workshops in line with the Approved Trainer Code of Conduct.

You will be required to work alongside the clinicians, coders, and managers to enhance the accuracy and completeness of the clinical documentation provided for coding purposes.

You will respond to external FOI requests, disseminate training feedback, advise the coding team of new coding guidelines and updates to national standards and local policies, and provide appropriate support where required.

General Paediatrics Great Western Ward provides the general paediatric in-patient service plus many sub-specialities including allergy, nephrology, neurology, haematology, infectious diseases as well as surgical specialities (general paediatric surgery, orthopaedics, ophthalmology, ENT, and urology). There is support available from Westminster CAMHS. The patient acuity and complexity is unusually high and many children have severe underlying health problems or neurodevelopmental problems. The Paediatric Short Stay Unit (PSSU) provides short stay in-patient care (anticipated to be <72 hours) and ambulatory care. Patients can be referred for intravenous antibiotics if these cannot be given in the community. There is a high volume of outpatient reviews including nurse led clinics and GP rapid referrals, and review slots for ENT, dermatology and diabetes. There is a prolonged neonatal jaundice service which GPs and midwifes/health visitors can refer directly to. The Childrens Decision Unit (CDU) based in A+E There are 4 dedicated beds and one cubicle for patients anticipated to have stay <24 hours. Westway Ward manages day-case procedures, including surgical day cases and radiology investigations. The pre-operative assessment clerking clinic is led by the Paediatric Surgical FY1 supported by the PSSU ward team and surgical/anaesthetic teams as required. Outpatient clinics include general paediatrics and a wide range of sub-specialities. The Inpatient Ward has special facilities for managing children with severe infections. We are a HCID (high consequence infectious diseases) centre, and all staff will be trained in the protocols for caring for patients with HCID, including simulation training. The SPR is expected to maintain excellent communication with referring hospitals and GPs regarding inpatients. The SPR is exepcted to particiapte in the antibiotic stewardship programme. There is also the opportunity to be part of the BMT team and learn about complex infections in this higly immunocompromised population. The ward registrar also carries the Paed ID pager which takes consults for management advice across all areas of paediatrics within the Trust including our Haematology/BMT service, NICU, PICU and external calls for referrals or advice, with support from the attending Paed ID consultant. Day case patients are also referred from other hopsitals for support with management and review. Much of the clinical work of the department is outpatient based so trainees during their time with the ID unit are encouraged to gain experience of outpatient management of ID patients including the two weekly multi-disciplinary HIV clinics, TB clinic, Inflammation clinics, hepatitis clinics, general infectious diseases and congenital infection clinics. In-patients are managed on Grand Union Ward by one SpR and two ST2-3s. ICHT has a full program of Post Graduate Education. The SpR is encouraged to attend and contribute to this program. It includes weekly Paed ID/PICU combined teaching rounds, antimicrobial stewardship rounds, ID Journal Club, Paediatric Grand Rounds, Hospital Grand Rounds, and twice weekly multiprofessional clinical meetings with adult ID, microbiology, virology and infection control teams for the Trust. The focus of the Academic Department of Paediatrics of the Imperial College School of Medicine is paediatric infectious diseases and immunology. The Department provides a tertiary service in paediatric infectious diseases with a special interest in life-threatening infections, HIV infection, toxic shock syndrome, Kawasaki disease and tropical medicine. The unit has a large research program focusing on HIV management and transitional care, mechanisms and treatment of sepsis, management of viral hepatitis, biology and diagnosis and management of tuberculosis and mechanisms and management of Kawasakis and toxin mediated diseases. The Unit has led national and international work on the impact of COVID on children. The registrar will be encouraged and supervised in research projects relevant to their interests and training needs. There is a weekly journal club / seminar series, coordinated by the national grid trainee Paediatric Neurology, Neurodisability and Community Paediatrics The children's neurology service provides care for children and young people with complex neurological conditions (genetic, congenital and acquired). The team provide inpatient consultation and outpatient care to children who are referred by their paediatrician or by another specialist with a range of childhood neurological disorders. They offer opinion in relation to acquired neurological problems, suspected neuromuscular disease, unexplained moderate to severe neurodevelopmental disability and complex headache syndromes. The team run sub-specialist clinics, including foetal medicine clinics, combined headache clinics (joint adult and paediatric neurology input) and combined neuro-HIV clinics. The community and neurodisability teams offer comprehensive multidisciplinary and multi-agency services for children with neurodisability, communication disorders, and behavioural problems. They work in partnership with community and acute service providers in a variety of settings. Where relevant, the team provides health input for statutory areas of responsibility for education, looked after children, adoption and fostering. Paediatric Cardiology This new post includes protected training time in Paediatric Cardiology. The post is designed to meet the competencies for the RCPCH special interest module (SPIN) in Paediatric cardiology. The post holder will obtain skills in congenital echocardiography with a view to obtaining formal EACVI accreditation. The post holder will have access to education and training opportunities in Paediatric Cardiology at the Royal Brompton and Evelina Childrens Hospitals. Communication and working relationships

1. Principal Responsibilities:

In the Emergency Department, the Clinical Fellow will be responsible for the initial assessment and management of unselected acutely ill and injured patients. He/she/they will help to train and supervise more junior doctors (GP trainees and FY2 doctors). He/she/they will assist with clinical administration, for example, checking X-ray reports, corresponding with patients and their general practitioners. He/she/they will have opportunities to participate in the department’s Clinical Governance activities, including departmental audit, and national audits required by the Healthcare Commission. He/she/they will also be expected to attend departmental teaching sessions. He/she will be responsible for keeping his/her/their resuscitation training up to date.

The division of responsibilities between sites will be discussed at interview and arrangements made clear in writing prior to appointment. Factors to be considered will be the preferences of the applicant, their training needs and skill sets, and the needs of the service in so far as they can be foreseen.

We are looking for a motivated health care professional to join our expanding team of Advanced Clinical Practitioners in Acute Medicine at the Royal Bolton Hospital. The successful applicant will have the opportunity to work alongside a well-established and highly skilled team of seven Advanced Clinical Practitioners and two trainee Advanced Clinical Practitioners who provide a seven day service to the Same Day Emergency Care Unit and rotate across all areas of Acute Medicine. This is a fulltime post (37.5hrs/week) with a salary at 8a. Please note that non-medical prescribing is a requirement for this post.

The successful applicant will ideally have experience of working within an adult acute or urgent care division, and possess knowledge and a range of skills that cover all aspects of Acute Medicine. They will need to show a willingness and aptitude to learn and develop. They will need to be self-motivated with a desire to improve patient care through application of advanced clinical skills and knowledge, as well as leadership, education and research skills. The Advanced Clinical Practitioner will be supported with clinical supervision from an allocated medical consultant mentor and Lead ACP in Acute Medicine, and have access to continuing professional development that reflects all four domains of advanced practice.

• Be able to manage their own caseload in response to unpredictable patterns of work.
• Act as a clinical expert in patient care, able to make high level clinical decisions, informed by advanced diagnostic, assessment and treatment skills.
• Facilitates ‘expert’ inter-professional care for the ill patient and their dependants by direction, supervision and example; and maintenance of all records.
• Able to provide advanced clinical skills and knowledge to medical and nursing staff within the acute medicine and urgent care division.
• Initiates, undertakes and evaluate research and audit.
• Develops and deliver educational programmes for staff locally and will work with Advanced Clinical Practitioners locally/nationally to develop advanced nursing practice.
• Ordering and interpreting radiological imaging is an integral part of the role.
• This a designated non–medical prescribing position.
• It is anticipated that 80% of the time will be spent in the clinical area with 20% non-clinical time relating to the other three pillars of advanced practice.
