The duties are predominantly centred around the physical retrieval, filing and preparation of case notes in conjunction with an efficient and professional outpatient reception service. The successful candidate will assist in the provision of case notes for patient care both Outpatient and Acute ensuring that all case notes are delivered and prepared to Service Level agreements. The post holder will be expected to undertake weeding of existing case notes in line with the organisations Retention and Destruction Policy, ensuring that the Patient tracking system is accurate and up to date. This is a very busy department and we work to tight deadlines in completion of our work therefore it is essential that your application demonstrates good communication, ability to work to tight deadlines, ability to work under your own initiative and ability to prioritise your workload with a high level of accuracy (please see job description and person specification for full details)

Communication and Key Working Relationships The medical records clerk may come into contact with all staff who use paper case notes across the Trust. The main working relationships will be with: Medical Records Clerks/Supervisors Data Access and Disclosure Office Information Governance Team Medical Secretaries Out-Patients Team Ward Receptionists Planning and Organisation Generation of hourly print outs for case note requests Ensuring case notes are sent out in a timely manner Ensuring case notes are sent to the correct locations Ensuring that searches are made for missing case notes, this may include full bay searches Answering the phone to take emergency requests, ensuring appropriate information is collected to identify the correct patient and case notes

To assist in the retrieval of case notes as required. To accurately file case notes returned to the Department in a timely manner. To assist in the security of case notes in the department. Initial duties include: the collection and filing of Health Record Folders into the storage areas and archiving (scanning folders) folder using the Trust archiving system. Eventually, in time leading on to further duties which include: Health Record Folder retrieval, updating patients demographic details in their folder and on the Trusts PAS system and reception duties. It should be noted that this post is very active and involves a substantial amount of lifting, walking, pushing trolleys and transferring of patient's folders.
