We are looking for a dynamic and client centred occupational Therapist to join our team in Lewisham!

The course is a postgraduate certificate and applicants are required to demonstrate aptitude to study at postgraduate level, either through degree level education or demonstration of academic equivalence.

We are delighted to invite applications for a Specialty Doctor post in Child & Adolescent Psychiatry in Child and Adolescent Mental Health / Paediatric Neurodisability / General Practice with experience in mental health (10PAs) to join the established Centre for Interventional Paediatric Psychopharmacology and Rare Diseases (CIPPRD) & CIPP Rett Centre. We are a national and specialist outpatient Tier 4 Child and Adolescent Mental Health Service. Part time applications will be considered.

We are looking for a highly motivated and dedicated specialty doctor who is committed to providing and maintaining high quality and standard of psychopharmacological service and patient care for children and young people with multiple neuropsychiatric and neurodevelopmental conditions, major mental illness, and rare disease conditions which can be degenerative and life-limiting.

The service provide expert consultation with services nationally on whole-person medicine and integrated mental and physical healthcare. We are a national specialist psychopharmacology centre which provides whole-person medicine and have excellent links with other national paediatric specialties, brain injury and third sector organisations. We are well-regarded by services and service-users across the UK.

The postholder will provide specialist psychiatric assessments and physical examinations of children with severe and complex neurodevelopmental and / or neuropsychiatric difficulties. They will also follow-up and care co-ordinate these children in the Medication Stabilization and Monitoring Clinic and in the CIPPRD Follow-up Clinic under the supervision of the consultants. The post holder will need to complete online clinical outcomes using the HealthTrackerTM, a digital health monitoring platform, along with assisting in collecting psychophysiological measures using non-invasive sensor-based wearables (watches) and review patient reported outcomes routinely, to provide optimal clinical care.

The Schools Outreach Team (SOT) is a small dynamic team (including 2 full time clinicians and dedicated support from the clinical service lead). SOT has a dual remit and offers mental health support, consultation and teaching and training to New Woodlands School, a specialist provision for children with Social Emotional Mental Health Needs as well as providing a CAMHS service to the Lewisham’s Outreach Inclusion Service. The Outreach Inclusion Service offers every Lewisham mainstream Primary and Secondary school access to support for children aged 4-16, and their families, who are experiencing difficulties in participating fully in their educational setting.

The Mother and Baby Unit (MBU) is a 13-bed acute female ward, providing evidence-based mental health treatment for both pregnant women and women with babies under one year old.

Our service is for women who present with an acute or relapsing mental illness e.g. schizophrenia, bi-polar disorder, affective disorders. Additionally, and unique to the perinatal specialism, is the presentation of postpartum psychosis which is considered a psychiatric emergency.

Our treatment programmes are designed collaboratively with the service user, their families and the extensive MDT to which the nurse provides a unique voice. Our aim in promoting recovery is twofold: a) for women to develop confidence in their vital role as mothers and to have a lasting return to their community and b) for babies to grow up to be physically healthy, emotionally capable and securely attached so that they can lead happy lives.

We do not expect you to come to us as experts in this specialist field. However, you can expect us to train you to become expert over time. As well as in-house training, you will have opportunity to have specialist perinatal training. Within your two-week induction, you will be given access to ‘A brief introduction to perinatal mental health’ which is a 5-hour online self-study. Additionally we train all our nursery nurses in baby massage/baby yoga/breast feeding .



As part of a rich multi-disciplinary team, you will be responsible for providing nursery nursing interventions and support to mothers with acute perinatal mental health needs and their babies. This will include mothers in the antenatal and postnatal period within the MBU setting.

1)To work as a member of the MBU delivering interventions which meet the needs of the baby for those mothers who are receiving inpatient care. The nursery nurse will also promote and support the health of the mother and baby.

The role

We are pleased to offer this post, in the assessment and management of dysphagia and communication within the mental health of older adults inpatient setting. It is an ideal opportunity for anyone with experience and interest in brain injury and mental health to develop their skills and to contribute to the development of our service. We welcome applicants with experience of working with neurological disorders in any setting and who have a commitment to reducing health inequalities for people living with severe and enduring mental health conditions. The post is split between working within the Neuropsychiatry Brain Injury Service on the Lishman Unit, which has 7 beds and supporting the Head of Service to work with service users across the Trust. They may have a range of diagnoses, including psychosis, anxiety, depression and dementia.

The role includes, but is not limited to:
• Assessing and managing complex communication and swallowing difficulties after brain injury
• Assessing and managing communication difficulties caused by mental health conditions such as psychosis, or neurological impairments such as Parkinson’s Disease or stroke.

• Providing relevant training for staff and groups for service users

The Service

We are a small but committed team of 4 SLTs. We meet regularly to share knowledge and ideas and support each other. The Service is led by the Head SLT who clinically covers working age wards across the Trust, with the support of 0.4 within this post and the 0.6 SLT in neuropsychiatry. The Older Adult Team consists of a two SLTs covering 4 wards - a Band 7 and a Band 6. We work together closely and provide clinical cover for each other when needed, so there are opportunities to develop a range of skills. We also have links with Kings College Hospital for peer supervision and support.

Although SLT is relatively small service within SLaM, the role on the Lishman Unit has been well recognised and supported and there are many opportunities for personal and service development. We also encourage flexible working where that is desired and we have been improving people’s ability to work more of their non-clinical time from home where that is appropriate.

Location

This post covers multiple sites , across 4 London Boroughs, but your primary base would be on the Lishman Unit. This is currently on the Bethlem Royal Hospital site, but will be moving to the Maudsley site later in the year and into a brand new development. Patient Age Group

This post provides speech and language therapy (SLT) to both neuropsychiatry services and Trust wide. In neuropsychiatry, the post holder will work as part of a multidisciplinary team (MDT), providing specialist communication and dysphagia assessments and interventions to patients on the Brain Injury stream of 7 patients on the Lishman Unit. The other part of the role will be working as part of a small speech and language team responding to referrals from other units across SLAM in the boroughs of Lambeth, Southwark, Lewisham and Croydon relating to the assessment and management of communication and eating/swallowing difficulties presented by adults with mental health difficulties.

The post is based in the Psychological Medicine and Older Adults Directorate.

The post holder will contribute to the development of the SLT service and the provision of multi-professional training in communication, feeding and swallowing disorders in adults for the Trust.

Lishman Unit
• To autonomously manage a caseload of 7 service users in the neuropsychiatry brain injury service and carry out assessments, interventions and give advice regarding the daily management of service users to the MDT.

The SLaM Psychology in Hostels (PIH) Service is an innovative multi award-winning specialist homeless psychology service. For 12 years PIH has led in the development of ‘Psychologically Informed Environments’ (PIEs) delivering ‘Trauma Informed Approaches’ and developing best practice for working with excluded and multiply disadvantaged populations across Lambeth and Westminster in partnership with the third sector.

The client group experience multiple disadvantage and have high levels of complex trauma and interpersonal difficulties coupled with substance misuse issues and risk-taking behaviour. To support them we deliver a ‘PIE’ approach. A PIE takes into consideration how the psychological and emotional needs of the clients are affected by the environments they inhabit and how services support them. This includes the approaches adopted by housing and outreach staff; the training and support offered; the psychology specific interventions delivered and the integration of psychological and trauma informed approaches to the design of services. The current PIE psychology model uses Mentalization-Based Treatment (MBT) and other evidence-based treatments for working with complex needs populations (Incl. DBT, CBT, Psychodynamic, Narrative, Systemic). We aim to facilitate recovery by improving mental health and wellbeing, reduce substance use, work with behaviours that ‘challenge’, increasing engagement, reducingsocial exclusion and barriers accessing mainstream services.

This role will be based in a newly created Lambeth Housing First project run by our third sector partners, and supervised by one of the senior Psychologists in the team.

Housing First is an approach to ending homelessness through housing and support provision. It prioritises access to housing with intensive tailored support for the client, and emphasises choice and control.

The post holder will:
• Work closely with council and third sector partners and other psychologists to develop PIE and trauma-informed approach to understanding and managing clients’ difficulties.
• Support and train the third sector partners, project MDT including peer advocates and wider network of services.

We are looking for enthusiastic people to be part of our well established and growing service. They will be organised, with good data entry and statistical skills.

You would receive supervision from a qualified psychologist. As a department, we have close links to the clinical psychology doctoral training programmes at the IOPPN and Salomon’s training centres, and frequently have clinical psychology trainees on placement.

The role will include delivering guided self-help, assisting with group interventions, and delivering some clinical work on an individual basis, under supervision. You will assist the Day Services team in running the weekly programme, offering meal support, and supporting patients with behavioural experiments. The Eating Disorder Unit (EDU) is internationally renowned for providing best practice, evidence-based care for adults with the full range of eating disorder difficulties. Outpatient treatments include guided self-help, group and individual interventions. We are the developers of MANTRA, now recommended as part of NICE 2017 guidelines for the treatment of Anorexia Nervosa, and of FREED (First Episode Rapid Early Intervention for Eating Disorders).

Key Responsibilities:

Clinical and client care
• To provide support for service users in the EDU service under supervision.
• To provide patients with protocol-based psychological interventions, such as supported guided self-help.
• To support protocol-based group psychological interventions for service users and carers.
• To assist psychologists in the development of individual or group clinical materials or training materials.
• To assist the Day Care team with meal support with patients and supporting specific patients with 1:1 support via behavioural experiments.



Contributing to team or service clinical functioning
• To contribute to the effective working of the service by managing the outpatient audit databases and contributing to analysis/ dissemination where required.
• To act in a manner that supports a psychologically informed framework for the service.
• To participate in the team’s delivery of accessible and acceptable services to diverse local communities.



Research and development
• To assist in the design and implementation of audit and research projects to inform the service and others in the development of Trust psychology and other services, training provision, and research activities.
• To undertake data collection, analysis, the production of reports and summaries, using IT and statistical programmes, to be responsible for audits and reports, under the guidance of senior staff.
• To undertake searches of evidence-based literature and research to assist in the development in the Trust of evidence based practice.
• Under guidance, to conduct surveys with service users and carers to gather views on service provision.



Teaching and training
• To disseminate research and service evaluation findings through presentations or published articles.
• To participate in demonstrating work methods to newly recruited staff and to support their development of necessary skills.



Maintaining professional standards and continuing professional development
• To receive regular clinical and professional supervision, in accordance with professional practice guidelines.
• To develop skills and competencies that assist in the delivery of current duties.
• To undertake personal training relevant to the post in agreement with the supervising psychologist and the manager.



Care or management of resources
• To take care of, and use carefully, the Trust’s equipment and physical resources.
• To be responsible for safe-keeping of databases and have copies of questionnaires ready.



Record-keeping and information governance
• To ensure that all information generated by own work is recorded as required by Trust policies and local procedures.
• Under the supervision of the team leader, to maintain the highest standards of clinical record keeping and report writing, according to professional and Trust guidelines, including electronic data entry.
• To maintain databases and enter data, and perform analyses and produce reports.



General
• To respond appropriately and professionally to emotionally distressing situations especially around meal times, weighing times, and holding professional and service boundaries. To support others involved in such situations.
• To work flexibly which may include offering some regular commitment to late shifts and early, within the overall Job Plan.
• To travel to other clinics and settings off the unit as appropriate and across the Trust when required.

This is an exciting opportunity for a skilled Band 6 practitioners with an interest and/or experience in the criminal justice system to be an integral part of both a court liaison and diversion team and a police custody suite assessment team. You will be working with both adults and young people, within what is a very interesting and rewarding area of care that is continuing to develop further.

As a Liaison and Diversion Practitioner, some of the responsibilities you can expect will include:
• Proactive screening of cases in police custody or at court.
• On-the-day triaging and assessment - including full mental state examinations, risk assessment and gathering of background history for individuals presenting with a wide variety of established or suspected needs. These can include anxiety, challenging behaviour, serious mental illness, personality disorder, neurodiversity needs speech and language difficulties, substance misuse and associated social issues within police custody.
• Responding to detainees who present with urgent and non-urgent clinical issues with appropriate interventions. Interventions may include onward referrals for a Mental Health Act assessment, advice, signposting or making onward referrals into primary, secondary or voluntary care agencies.
• Providing advice, information and support to custody/court personnel.
• Provision of written assessment summaries to police, court and other relevant agencies in the criminal justice pathway.
• Liaising with CAMHS, YOT practitioners or other relevant agencies whilst screening young people.
• Recording data for regular auditing and evaluation.


Some weekend/bank holiday working may be involved in the role.

To work with colleagues, police, custody healthcare staff and other custody/court staff (inc Appropriate Adults) in order to provide a comprehensive and integrated vulnerability screening and assessment service.



To demonstrate strong assessment skills, solid problem solving and decision making and developing holistic and patient centred care plans whilst also being able to recognise when they may need senior clinical/managerial advice/support and to actively seek this in the clinical setting.



To facilitate appropriate signposting to/referring on, or interventions in close collaboration and liaison with other relevant agencies. Developing a good working knowledge and understanding of local services and pathways will be important in this role. Knowledge of clinical pathways will enable the applicant in the ambition of facilitating a persons diversion away from the criminal justice pathway at the earliest possible opportunity. (The term ‘diversion’ being used in its widest possible sense and includes urgent and non-urgent pathways).

IMPORTANT

Please note– this position is offered subject to a successful Police Vetting application that will enable you to enable work in police custody locations. As such, you will be subjected to a significantly higher level of vetting than is standard. This vetting is conducted by the Metropolitan Police and is essentially a detailed background check. One of the most essential criteria is that you are required to have resided in the UK for at least 3 years.



This requirement is a lengthy process that could add several months to the existing pre-employment checks that will be conducted before a start date can be agreed for this position.
