An exciting opportunity has arisen for a community rehabilitation band 6 physiotherapist to join a supportive and enthusiastic community ward therapy service as a Physiotherapist, working in in patient rehabilitation delivering rehabilitation and supporting discharge planning

The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.

We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions.

You will be working with patients with a large variety of conditions and disabilities. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention .

6 day working is in effect - Monday- Saturday and bank holidays on a rota basis

Acute Pain Team participating in the ongoing development of the acute pain service and supporting the utilisation of anaesthetic and pain management techniques
• Work collaboratively with multidisciplinary colleagues to provide evidence-based pain management

The role of the Advanced Practitioner is as a non-medical practitioner, working as part of the Clinical Oncology team, under the supervision of a Consultant Medical and Clinical Oncologist.

The post holder will provide clinical expertise and advice for oncology chemotherapy patients. They will demonstrate a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of malignant oncology conditions and pre and post chemotherapy patients. Be involved in consultant clinics and assist in pre chemotherapy assessments and planning, and emergency attendance avoidance. They will have a key role within the MDT acting as a liaison between all members of the multi-disciplinary team, to allow for seamless and efficient care.

The Dietetic single point of access referral to book is a busy, fast pace service with a high volume of different Dietetic specialty clinics and domiciliary visits to manage. The post would suit candidates that enjoy working in this type of environment.

The main duties of this role is to ensure the safe and efficient management of Dietetic outpatient referrals from receipt of referral to booking of patient's first appointment.

The role is computer based so good IT skills are essential.

The department runs on the EMIS patient record and booking system. Experience working with patient electronic record systems/booking systems is desirable.



The Dietetic single point of access referal to book team is based at Brierfield Health Centre with the community Dietetic team.

The single point of access referral to book team is a friendly team made up of 6 staff - 1 team administrator and 5 administrators. The team has support from the Dietetic Admin Lead as well as the wider Dietetic Management Team and Clinical Leads.

The Dietetic Department is one of the biggest in the region and has multiple bases across East Lancashire. The department covers a wide geographical patch across East Lancashire covering both inpatient and out patients services. The team manages all the Dietetic outpatient referrals.

To provide a high quality front of house administration & clerical service for all users of the Integrated Musculoskeletal, Pain & Rheumatology Service (IMPReS), ensuring that the administration associated with all patient appointments is completed accurately and in a timely manner. Dealing with in-bound and out-bound calls to agreed professional standards and providing administration and monitoring support for the referral pathways.



Under the guidance of the Team Lead the post holder will on a daily basis monitor the demand of referrals that have been activated through e-referrals and ensure enough triage time Is allocated and scheduled across MSK and Physio Extended Scope Practitioners to ensure that they are triaged within the 24 hour target set by the CCG Commissioners and to ensure there are no breaches.

MAIN DUTIES

Act as the first point of contact for all users of the Integrated MSK, Pain & Rheumatology Service. Be committed to continuous improvement of data quality within the SPOA and Booking Teams and to contribute towards and support service improvement

Excellent customer service skills when dealing with enquiries from patients, members of the public and staff, in a polite and appropriate manner at all times. Receive sensitive and contentious information/queries of a confidential nature and to be able to deal with it in an appropriate manner, using tact or persuasive skills, to neither offend or antagonise when there are barriers due to culture, language differences or complexity of their conditions.

Liaise with patients to arrange a mutually convenient appointment whilst taking into consideration clinical priority, availability of clinicians and the constrictions of the Access Booking Targets and enter the information onto the Integrated MSK, Pain & Rheumatology Service e-scheduling- and information management systems.

Ensure patients are inputted onto the correct waiting list recording accurate and up-to-date confidential data on to computerised systems, i.e. EMIS, Choose and Book, Continuum, PAS etc.

Organise as and when required the attendance of interpreter/link workers for patients attending clinics as per Trust guidelines.

Ensure DNA’s are actioned in line with Trust Policy.

Provide cover for receptionist when required.

General clerical duties

Able to work without supervision and as part of a team and manage own time maintaining up to date and efficient administration systems.

Responsible for recording of all triage decisions made by clinician’s that come through SPOA and then re-direct them to the relevant service/teams.

Record triage data on a daily basis onto the SPOA spreadsheet.

Ensure urgent and routine referrals are managed appropriately and within agreed timeframes.

Maintain Triage waiting list on a weekly basis

Action rejected referrals from secondary care

Monitor NHS Net to pick up any referrals within the IMPReS folder from Orthopaedics, Pain Management and Rheumatology.

COMMUNICATION.

Respond to telephone and e-mail enquiries, contact with members of the public and staff of all disciplines, with tact, sensitivity and confidentiality at all times, being aware of The Data Protection Act, Caldicott and Freedom of Information policies

Participate in regular Department Meetings putting forward issues and suggest possible solutions,

Ensure all patient demographics are correct when receiving activation calls.

To liaise with GP Surgery’s, secondary care and other agencies regarding IMSK service.

Clinical

• Actively contribute to the development of the Trust Strategy for Nursing and Midwifery, leading systematic implementation within the Division in conjunction with Divisional Director of Nursing/Head of Midwifery and Matrons.

A fantastic opportunity has arisen for an experienced, motivated and reliable individual to join our closely knit team on Digestive Diseases Unit C2 at Royal Blackburn Hospital as a Band 6 Sister/Charge Nurse.

C2 is a 26 bedded Digestive Disease Unit which sits within the directorate of Medicine and Emergency Care.

The role requires working 12 hour shifts covering days, nights and weekends. You will be familiar with the role and its requirements, deputising for the ward manager in her absence and supporting and developing skills and knowledge of junior staff. This post is an essential development for the delivery of a fully integrated discharge service across Pennine Lancashire. The Integrated Discharge service therapist undertakes highly skilled and specialised work to support in the assessing and treating of patients, to facilitate complex discharges and improve clinical flow throughout the trust to improve access to acute services.

The post holder will assist and provide a proactive and efficient Complex Case Management Service for all adult patients across all ELHT hospital sites. They will use their specialist therapy skills to provide therapy assessment and case management in the integrated discharge service. They will provide education and advice to maintain a high standard of discharge planning processes. Ensure and promote the timely access to health and social care resources to enable the safe and effective discharge of patients from hospital.

The post holder will need to be flexible and work across all sites of the trust, including the whole geographical area that East Lancashire Hospitals Trust serves within the community. The post holder may be required to work a pattern in line with extended service operating hours, over seven days a week to deliver the ‘Discharge to Assess’ model of care.

Maintain cooperative professional working relationships.

· Liaise with Multi-disciplinary Team members to facilitate decision making regarding discharge planning.

· Promote and facilitate robust communication channels between all agencies and professionals in Health and Social Care settings.

· Maintain effective paper and electronic communication systems within Complex Case Management, work in partnership with other teams within ELHT Transfer of Care Service, managed by Head of Clinical Flow.

· Ensure that all actions promote equality and diversity in accordance East Lancashire Hospitals Trust policies, ensuring all patients/clients and staff are treated as individuals, and with dignity and respect.

· Support and educate ward staff to make appropriate referrals into discharge services, monitoring the progress of patients along the discharge planning pathway.

Give both verbal and written feedback about the progress of a patient’s assessment and treatment, this will include the typing of comprehensive documentation from visits and clinical input.

· Taking the patient home upon discharge to settle (e.g. in situations described above) or visiting post discharge to check the patient is able to manage – and to prevent re-admissions.

· Utilise mental health knowledge – able to carry out in depth cognitive assessments.

· To be able to carry out ‘second opinion’ assessments when there is a dispute between a health professions opinion and the patient or their family. For example a decision around home rather than bed base.

· To use social services IT systems in conjunction with the trusted assessment document and set up new packages of care for patients.

The pelvic, obstetric and gynaecological physiotherapy service provides outpatient assessment and treatment for a wide range of pelvic floor dysfunction conditions and obstetric MSK issues across the East Lancashire and Blackburn with Darwen footprint. We are looking for a full time band 6 physiotherapist to join our dynamic team, working as part of a very well established multidisciplinary urogynaecology and colorectal team. As an early implementer in the now England-wide Perinatal Pelvic Health Service we have already embedded many of these improvements in our service. Previous experience is an advantage but not essential as we would consider a band 5 to 6 training post dependent on experience.

Undertake comprehensive specialist assessments, set patient centred goals, create treatment plans, monitor and evaluate a designated caseload of patients, using evidence based clinical pathways.

Independently maintain a clinical caseload and manage clinical risk, in a variety of settings including individually or in groups.

Formulate treatment plans and goals in partnership with the patient, clearly communicating your clinical reasoning to justify your recommendations and utilising most appropriate treatment/intervention.

To provide specialist clinical advice, support and educate patients and carers in the management of their condition.

Assess capacity and gain informed consent from patients for any intervention carried out. Complete mental health capacity forms as required.

Contribute to reviews involving multi-disciplinary and multi- agency staff, providing evidence-based expertise within your specialist area. The post holder may be required to take the key worker role.

Work across agencies as required and have a working knowledge and understanding of the systems within Health, Education and Social Services to facilitate appropriate treatment/interventions, equipment, access, and adaptation issues.

Postholder will participate in own supervision and supervise delegated staff in line with supervision policy.

Undertake training and development of students and less experienced staff.

Provide leadership, supervision, and support to other members of the team.

East Lancashire Hospitals has an accredited Urogynaecology Unit and an established multidisciplinary team . We offer a range of specialist services and are a combined acute and community trust with a large AHP workforce.

The post holder will work independently in various outpatient settings across East Lancashire and Blackburn with Darwen offering assessment and treatment of a range of pelvic floor dysfunction conditions along with pregnancy-related pelvic girdle pain and postnatal perineal trauma patients. There is also opportunity to work within consultant-led clinics, to teach students and to engage in education and promotion of the specialist field to colleagues and the public. You will have the opportunity to develop your clinical skills in this area with support from the other members of the physiotherapy team and wider MDT.

They will be working with a highly skilled team of radiographers, radiologists and radiology assistants, who provide a patient-focused MRI service.


These posts could be full time posts, or additionally we are happy to hear from applicants requiring part time or flexible working.


The department is set over 2 sites, Blackburn Royal and Burnley General and currently consists of 5 CT Scanners and 5 MRI scanners.


We are also awaiting delivery (July 2022) of a 1.5T Philips Ingenia Evolution that will be sited on Rossendale, Clitheroe and Blackburn mobile pads. This service will run from 8am-8pm, 7 days per week.

The post holder will be expected to rotate across all the MRI and/or CT departments to support service and professional development. This is a forward thinking Radiology department that supports Radiographer advanced practice, we have 2 CT Head Reporting Radiographers, 1 MRI Reporting Radiographer and trained CT Colonography Radiographers.

You should have evidence of current ongoing CPD activities incorporating current advances in both CT and/or MRI.

You will have demonstrated the skills necessary to work as part of a small multidisciplinary team and have the ability to take the lead in the absence of Senior Colleagues.
