We are searching for a Care Coordinator to join our friendly and supportive team to oversee and coordinate the care and support of the patients of the designated team. The post holder will allocate work according to demand and ensuring team members are working to their competencies.
They will ensure that KPIs are met in terms of turnaround time targets. To be proactive in recognising potential issues with the view to resolving them quickly to avoid escalation.
The post-holder will be the go-to person for a designated team of clinicians looking after a specific patient cohort. The workload of all team members will be pooled and the Care Coordinator will have oversight of this and will assign team members to work on each stream of work according to the demand.
The Care Coordinator will sift through the workflow to ensure that it is all appropriate for the clinical team to undertake. Where any particular task is administrative, the Care Coordinator will draft this on behalf of the team. The Care Coordinator will also respond to letters from Secondary Care requesting that we carry out work that is contracted to them.
