As a Human Services / Support Coordinator or Qualified Professional with RHA, you will serve as the center of the interdisciplinary service team, which consists of nurses, Direct Care Associates, the Administrator and Group Home Manager, to guide the development of person-centered plans and overall programs based on the information you gain from interviewing the people we serve. A Qualified Professional serves as the primary individual contributor, coordinating and monitoring the array of services and supports needed to address each person’s goals and desired outcomes as identified through the personal outcome interview, including health and well-being, psychological, and promotion of personal independence. The QP assists each person in identifying and communicating his or her requests and needs for services and supports. The QP supervises, integrates and coordinates person centered plans, and monitors progress towards personal, clinical, and functional outcomes. The QP initiates periodic reviews, investigations, modifications and adjustments by soliciting the person’s feedback as the person wants and needs change. The QP may supervise the work of others.

Annual Salary: $48,000

Sign on bonus: $3,000

Job Responsibilities

• Assisting each person we support to identify and communicate his or her requests and needs for services and supports through direct interviews
• Supervising, integrating and coordinating person-centered plans and monitoring progress towards personal, clinical and functional outcomes
• Initiating periodic reviews, investigations, modifications, and adjustments by soliciting the feedback of the people we support as their needs and goals change
• Reviewing assessments completed by the people we support and other members of the interdisciplinary team to best help the people we support to meet their full potentials
• Supervising and writing the person-centered plan based on assessments, interviews, and observations
• Overseeing many aspects of residential activity, including home appearance, leisure materials, cleanliness, and community integration to ensure that the people we support are healthy and safe
• Visiting vocational centers, residential homes and any other area RHA provides services in to assess how person-centered plans are implemented
• Reporting to the Administrator and working closely with colleagues to maintain communication and provide feedback, standardize procedures, expedite person-centered plan implementation and workflow and improve employee performance
• Monitoring activities to ensure that quality assurance and/or state survey improvements are being made
• Helping to recruit, hire, train and dismiss Group Home Managers
• Serving as the link between the people we serve and their families and legal guardians to gain approval for services provided
• All other duties as assigned.

 
Job Requirements:

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degree
• Working knowledge of applicable regulations that apply to our service offerings
• Valid driver’s license, auto insurance, and reliable transportation
• Supervisory/management experience and experience writing person-centered (individualized service) plans preferred

Provides prescribed medical treatment and personal care and services to persons with disabilities in residential homes and/or vocational centers at a single business location. Relies on knowledge, experience, and judgment to accomplish tasks in combination with clinical supervision from the registered nurse and direction from the Registered Nurse and/or Administrator. Requires graduation from an accredited licensed nurse program and current LPN license with no restrictions to practice nursing.

Pay: $29 hourly

Schedule: 6am-10pm

Job Responsibilities:

• Provide holistic nursing care to individuals with intellectual and developmental disabilities
• Observe, assist with gathering health related data, and report the health needs or the people we support to the registered nurse
• Coordinate preventative and routine medical care for people supported
• Document nursing care provided
• Assist physicians with on-site medical clinics where applicable
• Provide health related trainings to unlicensed personnel according to state regulations
• Conduct medication pass observations in the home to ensure staff competency according to state regulations
• Conduct medication closest assessments
• Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
• Observe for and report any abnormal health related findings to the physician
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported

Job Requirements

• Graduation from an accredited licensed nurse program
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
• Must be able to work independently, be organized, and possess the ability to prioritize work tasks

Requires a degree in a Human Services field with preferably an emphasis on Rec Therapy and experience with the population served and supervising the work of others.

Our Vocational Program Manager will supervise classroom and vocational activities and coordinate the schedules/events of Direct Support employees, Vocational Instructors, etc.  Successful candidates will be able to rely on experience, knowledge, and judgment and will lead and direct the work of others to promote the vocational service's curriculum philosophy and mission.

Job Responsibilities

Quality Assurance:

• Formulates training policies, programs, and schedules, based on knowledge of individual people supported needs, company production processes, business systems, or changes in products, procedures, or services.
• Devises special teaching tools, techniques, and equipment and creates learning materials geared to each people supported ability and interest.
• Arranges and adjusts tools, work aids, and equipment utilized by people supported in classroom such as specially equipped worktables, computers, typewriters, and other mechanized and non-mechanized aids.
• Directs and monitors contract work.
• Negotiates rates with vendors to establish contract work.
• Ensures confidentiality regarding sensitive material, and including people supported rights to privacy, employee complaints, supervisor and unit problems, investigations, and legal proceedings
• Reports all suspected or observed incidents of people supported abuse, neglect or exploitation, to management.

Coaching:

• Instructs people supported in all facets of their individualized programs including but not limited to: improving sensory motor and perceptual motor development, perception, memory, language, cognition, and social and emotional development.
• Works with people supported to enhance their motivation, provide consistent reinforcement to learning, continuous assessment of level of functioning, and continuous feedback to people supported and clinical team for all learning activities.

Team Management

• Assists members of clinical & medical staff in rehabilitation programs for people supported.
• Monitors and assures responsibilities of Vocational Instructors and Program Manager stations
• Provides input to staff performance evaluations.

Teamwork

• Confers with guardians, Administrators, Qualified Professional, Habilitation Specialists, social worker, nursing, and others to develop individual instructional program for people supported.
• Interfaces between day program/vocational center, community workers, and employees.
• Plans and assists with the implementation of special events within and outside the day program/vocational center.
• Attends mandatory meetings and training sessions as required and scheduled by unit management staff, Administrator or corporate managers

Career and Staff Development

• Trains staff on day program procedures, modular day training, makes staff assignments, ensures adequate staffing; schedules lunches, breaks and orders supplies.
• Stays informed of new developments, methods, and training techniques in the ID/DD field.

Employee Health and Safety

• Ensures physical conditions in the day program/vocational center are safe and conducive for learning.
• Reports all accidents, hazards, and needed repairs.
• Understands and enforces universal medical precautions by utilizing personal protective and safety equipment.
• Seeks understanding of and follows company policy with regard to personnel, regulations, safety, & blood borne pathogens.
• Understands, teaches and utilizes proper body mechanics while assisting with behavioral intervention techniques, dependent/independent transfers and positioning of people supported regardless of individual weight distribution.

Requirements

• Successful candidates of the Vocational Program Manager must be able to meet the following requirements:
• Manages up to 12 employees.
• Is responsible for the overall direction, coordination, and evaluation of these employees from the first shift.
• Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
• Responsibilities include:  training employees; planning, assigning, and directing work; appraising performance; motivating, disciplining, promoting team performance; addressing complaints and resolving problems.

• Coordinates work orders with Home Managers and other staff supervisors to determine when planned maintenance will least interfere with operation of residential or center activities.
• Builds, repairs, and installs counters, cabinets, benches, partitions, floors, doors, walls, building framework, and trim.
• Paints interior and exterior walls and trim. Installs glass in windows, doors, and partitions.
• Installs items such as window shades, Venetian blinds, curtain rods, wall & ceiling fans and lights and door locks. Replaces damaged ceiling tile, floor tile, and wall coverings.
• Assures compliance to building code.
• Develops and coordinates with Administrator and Manager - Materials and Assets, plans for reconstruction, capital improvement or installation of new equipment, or buildings.
• Seeks bids for construction/renovation projects as appropriate.
• Suggests cost savings equipment when replacement is necessary.
• Estimates cost of parts, and supplies to be used during scheduled maintenance.
• Reviews manufacturer’s service manuals and records of maintenance problems to determine optimum frequency of preventive maintenance.
• All other duties as assigned

Education/Experience:
