Job Overview:
• Collaborate with clinical pharmacists and ward teams to deliver a medicines optimisation service, ensuring the safe and efficient usage of medications.
Key Responsibilities:
• Conduct medicines reconciliation for new patients, gathering drug histories from various sources and liaising with healthcare professionals to ensure appropriate medication.
• Assess patient medications at bedside/inpatient areas to determine suitability for use.
• Facilitate patient discharge by conducting medicines reconciliation, counselling patients on medications, and coordinating with healthcare providers for ongoing medication needs.
• Order and dispense medicines for individual patients, ensuring adequate supplies during inpatient stays and discharge.
• Perform final accuracy checks on clinically screened medicines, ensuring safe dispensing.
• Advise on the storage of patients' medications on wards, ensuring safety and security.
• Provide information and counselling to patients on medication usage.
• Mentor technicians and assess their competency in medicines optimisation activities.
• Participate in policy review and implementation to improve service delivery.
• Collect and analyse data to monitor and enhance the performance of the medicines optimisation service.
Statutory Responsibilities:
• Comply with relevant laws and regulations concerning project management and health and safety.
• Take responsibility for personal and colleagues' health and safety.
• Adhere to data protection regulations.
Communication and Relationships:
• Communicate information regarding medications to patients/carers and healthcare professionals.
• Effectively communicate technical information to hospital staff and clinicians.
• Demonstrate sensitivity and clarity in communication, especially in challenging situations.
Analytical Skills:
• Make professional judgements on medication discrepancies and conduct analysis on patient data.
• Ensure accuracy in dispensed prescriptions and medication handling.
Planning & Organisation:
• Coordinate technician-led medicines optimisation services.
• Organise supervision of support workers or trainees.
Qualifications
Essential:
• BTEC / NVQ/ SVQ level 3 in Pharmaceutical Sciences, or National Certificate in Pharmaceutical Science or equivalent.
• Current registration as a Pharmacy Technician with the General Pharmaceutical Council of Great Britain.
• Qualified Technician Checker (to National Standard).
• Accreditation in Medicines Optimisation.
Knowledge
Essential:
• Specialised expertise, supported by theoretical understanding.
• Familiarity with pharmaceutical technical procedures acquired through formal training.
• Proficiency in checking dispensed prescriptions and overseeing technical aspects within the pharmacy.
• Up-to-date awareness of national guidelines and legislation concerning medicines usage and pharmacy operations.
• Solid grasp of clinical governance, medication incidents, and risk management.
• Practical knowledge in maintaining records in adherence to confidentiality and data protection protocols.

Experience
Essential:
• Minimum of 3 years of post-registration experience in hospital pharmacy practice, with exposure to medicines optimisation.
• Completion of comprehensive rotations in hospital pharmacy, including medicines management.
• Experience in effective collaboration within a multidisciplinary team.
• Facilitation and mentoring of pharmacy staff.
