To work as a Clinical Nurse Specialist within the Community Specialist Palliative Care Team across Liverpool and South Sefton geographical area, supporting patients to be symptom managed in their usual place of residence. The focus of the role is to provide clinical expertise and leadership within the community setting. Support the team leader by monitoring and recording sickness and absence, capability and other issues within the team, reporting them as soon as possible. In conjunction with other caseload holders, provide day-to-day supervision and support for the team. Support the team leader in providing induction to the local working environment, clinical services and policies for new team members and students. Undertake personal development plans with other members of the team, as delegated by the team leader. Act as an authorised signatory for mileage and time cards for submission to the team leader. Communicate with the multi-disciplinary team to provide best outcomes for patients/clients; this may include organising case conferences related to highly complex cases. Participate in individual and group supervision. Monitor record keeping within the team and inform the team leader of any issues. Support the training and development of team members who need to improve on the level of patient care delivery. This includes record keeping and improving clinical competency. Support the implementation of any initiatives to improve quality within the clinical governance framework. Participate in patient satisfaction reporting to improve patient care. Work with the team leader to ensure achievement of your own personal development plan. Maintain up to date knowledge and competence in line with professional and service requirements and demonstrate critical thinking, decision-making and reflective skills to ensure own professional development. Support the Team Leader in creating an environment in which learning and practice development is fostered, evaluated and disseminated. Facilitate effective learning within the area of practice for all students and practitioners. Facilitate pre and post registration student placements within the team. Demonstrate high level of judgemental/analytical skills when interpreting and performing patient assessments and care planning. Paediatric Gastroenterology Clinical Nurse Specialist.

Band 7 CNS

A rare and exciting opportunity has arisen for a full time Band 7 CNS to work within our Gastroenterology team. You will care for children with diverse gastroenterological conditions, including but not exclusively, Inflammatory Bowel Disease, Coeliac disease, eating disorders and liver disease. Therefore, some degree of knowledge of these conditions would be advantageous.

The Bristol Royal Hospital for Children (BRHC) provides a local service for Bristol Children and a referral service for specialist care for families across the Southwest. The CNS will work closely with the multidisciplinary team in hospital and the community to ensure a co-ordinated quality service and will be actively involved in education, audit and research.

Whilst the post is based at BRHC the team attend peripheral clinics in Truro, Swindon, Plymouth, Torbay and Exeter

Main Duties and Responsibilities
• The post holder will engage and lead discussions in multidisciplinary team meetings.
• To provide specialises, highly skilled nursing care to children with gastroenterological problems and liver, of which at least 50% will have Inflammatory Bowel Disease (IBD).
• Improve standards of care for all children with gastroenterological conditions attending This includes IBD patients, patients with chronic gastroenterology & liver conditions.
• To work as a member of the multi-disciplinary team, co-managing the caseload of families seen by the Paediatric Gastroenterology To support the families through diagnosis and treatment in their social, emotional and health needs, referring to appropriate professionals and services where necessary.
• To develop and strengthen clinical and educational links with Health Visitors, Practice Nurses, Community Nurses and other regional medical and paramedical staff.
• To take an active part in initiating and leading research and audit
• To contribute to professional and Nurse Specialist Groups throughout the Directorate and wider nursing

JOB SUMMARY: Supporting the overarching Lakeside Healthcare Partnership strategy, the Lead nurse/ACP/ACP is responsible for leading and managing the Nursing Teams and responsible for all aspects of performance related to their clinical teams and specialty clinical services. Retain clinical credibility and act as a role model in delivery of patient care, maintain an overview of the effectiveness of care and services by staff within area of remit. Work closely with the HoN and NED to help lead, deliver and embed the Lakeside Nursing Strategy. Be conversant with national professional nursing agenda and provide local professional nursing leadership; this includes line management, mentorship, advice and support, monitoring and compliance, including adherence to Lakeside Protocols, Procedures, Guidelines and Policies. Be available and accessible to patients, public and staff to discuss issues of concern and take appropriate action where indicated. Workforce Management In partnership working with the Practice Leadership Team, the Lead nurse/ACP will be jointly responsible for delivering the following: - Provide effective leadership to all staff for whom they are accountable including motivation, support and development, effective communication with and between all staff, and adherence at all times by all staff to the highest quality and behavioural standards. Within this to ensure that all staff have an annual appraisal and agree clear objectives against which they are managed appropriately. To ensure attendance at work, for nursing staff, is managed appropriately and in line with Lakeside policy and process. Continually work to develop forward-thinking manpower plans to achieve resilient and robust daily performance. Enhance the performance, capability and development of others through formal and informal interactions. Motivate nursing staff by increasing their commitment to the organisations objectives and strategy, clarifying the vision and developing effective team working. Support the monitoring systems in place to provide assurance that all nursing staff are appropriately qualified and registered with the NMC. Ensure all mandatory training is delivered with ongoing training to enable staff to competently and safely undertake their roles.

The following skills and experience will be required and are essential for the successful candidates: Clinical To operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available time. To accept referrals from medical / nursing staff both within and external to the Trust, of newly diagnosed and existing cancer patients. To periodically review caseload and to discharge patients appropriately within the divisional guidelines. Educational Responsibilities To ensure own practice is up-to-date and evidence based, undertaking relevant study and further education as identified through the personal development process or in response to changing service needs. To attend relevant study days/conferences and network with other advanced / specialist nurses as required. To maintain an up to date knowledge in oncology, using information to effect change in practice and ensure the effective dissemination of new knowledge. To develop and deliver educational programmes to meet the learning needs of health care professionals in hospital and community, to include medical, nursing students and doctors.
