JOB SCOPE / KEY RESPONSIBILITIES
1. Contribute as a team player in a multidisciplinary team in employment planning, placement and support services for client with acquired physical disabilities to return to employment.
2. Provide job coaching including pre-employment preparation, on-the-job training and facilitation of a support network for clients through regular goal setting and review.
3. Conduct vocational assessment and exploration, job search, advocate for clients, engage with and educate potential employers on internship and employment issues for clients.
4. Secure training courses, open employment and apprenticeship opportunities/ job trials for clients. Tap on relevant schemes and subsidies for persons with disabilities that may facilitate their return to work process.
5. Co-ordinate job placement process, including work place assessment, employment negotiation and job task analysis. Communicate effectively with all relevant stakeholders, including allied health and medical professionals where necessary
6. Plan and facilitate groups to support client’s goal in returning to work.
7. Plan, organise and lead client’s care in the programme as a Case Manager on a rotational
basis.
8. Maintain up-to-date documentation of interventions and client information including assessment, job placement and job support records.
9. Engage in self-learning and professional development activities to stay updated on the best practices in the provision of employment support services. To equip oneself with knowledge of local laws and policies related to return to work and employability.
10. Organise and participate in outreach activities with stakeholders to engage them in training and employment of persons with disabilities.
11. Responsible for data tracking and analysis to measure the team performances based on programme’s milestones and outcome indicators as well as providing progress reports to management.
12. Work closely with Employment Support Programme (ESP) in SPD to maximise resources and outcomes for clients.
13. Any other duties as assigned by the Programme Head as and when necessary.
B. JOB HOLDER REQUIREMENT
1. Diploma/Degree holder, preferably in psychology, social work, occupational therapy, physiotherapy, human resource management or related disciplines. A plus if candidate has an Advanced Certificate in Supported Employment (ACSE).
2. At least 2 years' of experience in relationship management, customer service, career coaching or vocational counseling will be an advantage.
3. Proficient in computer skills.
4. Appreciation of the healthcare landscape in Singapore and exposure/ interest in working with persons with disabilities.
5. Strong communication and interpersonal skills
