Primary Objectives of Position
Provide holistic care and support to the physical and emotional well-being of cilents with special needs between 7 to 18 years old in AWWA Special Student Care Centre (SSCC).
Job Responsibilities
· Work closely with the team to provide holistic care and support to the physical and emotional well-being of clients in SSCC.
· Set Individual Care Plan (ICP) and Activities of Daily Living (ADLs) for clients under care and follow through with the plan and training schedule.
· Monitor and record progress and activities of clients in the case notes and submit monthly. This includes but not limited to the Happy Face Survey, attendance recording and skin/temperature records which are recorded weekly/monthly.
· Communicate with parents pertaining to client’s progress, enrichment activities and operation’s matters promptly.
· Assess, develop, plan, implement and evaluate enrichment activities for the clients.
· Supervise and/or assist clients during napping, mealtime, showering, grooming, toileting and other activities pertaining to adaptive daily living.
· Manage client’s behavior using appropriate behavioral intervention techniques.
· Monitor the inventory of designated rooms and update the staff in-charge for replenishment.
· Record and maintain incident reports and update other relevant documents timely.
· Support annual Individual Care Plan and progress discussion with parents/caregivers.
· Ensure that a safe, hygienic and supportive learning environment is provided for the clients.
· Buddy new programme assistant / interns and relief staff in orientating their roles into the centre using the standard guidelines.
· Assist Centre Manager/Supervisor to provide guidance to programme assistants when necessary.
· Champion and lead one committee with a team of members:
- To coordinate, execute and monitor feedback/updates and follow-up.
- To assign roles to team member, guide and monitor progress/commitment.
- To submit weekly/monthly report and action plans.
- To review Committee process and guidelines.
(Volunteer Management Committee, Safety Committee, Resource Management Committee, and Food Hygiene Committee).
· Deputize the Centre Manager/Supervisor during his/her absence in addressing issues related to enrichment activities, volunteer’s engagement and student well-being.
Other Responsibilities
· Maintain a high degree of confidentiality and discretion in matters involving client care.
· Perform any other duties as directed by AWWA Senior Management.
Job Requirements
· GCE O Levels or NITEC in Nursing/Community Service
· Preferred to have at least 3 years working experience with persons with disabilities with constant good performance. Entry Level Candidates are welcome to apply.
· Behavior management for children
· Microsoft Office Skills particularly in Powerpoint, Excel & Microsoft Words
· First Aid
· Operating assisting equipment, e.g. hoist
· Team player, positive mindset and proactive
· Adaptable
· Be able to anticipate of change and make quick and fair adjustment to the daily operation according to work.
