As a Social Worker in one of the Child Protection & Court Teams you will: -

• Work alongside the Assessment & Intervention Teams and be based in the same locality;
• Work with children subject of Child Protection Plans or within Court Proceedings;
• Work in an organised and planned way to ensure timescales set by the Trust and the Court are consistently met;
• Produce reports for Court, write court statements, care plans and assessments in a succinct manner, demonstrating highly attuned skills in gathering, assessing and analysing key information leading to evidenced-based recommendations for children;
• Represent the Trust in giving evidence in Court

About you: -

• You will need to have a recognised professional Social Work qualification and be registered with Social Work England;
• Have completed your ASYE programme;
• Be able to demonstrate experience of Court work;
• Have a good level of experience of working in a children and families setting and have managed a complex case load;
• Have a sound knowledge of legislation and guidance;
• Be able to demonstrate the ability to analyse information and make informed decisions;
• Good knowledge of child development.

JOB SUMMARY:

• The South West London and Liaison Psychiatry Service provides a comprehensive specialist psychological medicine service to patients, clinicians and carers on the St Helier Hospital site and works closely with the Crisis and Home Treatment teams to minimise the need for admissions to psychiatric hospital.

• The team will provide a 24hr service across seven days. As a new member of the team at St Helier Hospital you will be expected to assess patients who are referred primarily from the ED/inpatient areas. Patients seen may present with mental health problems, self-harm or as a psychiatric crisis. You will also be expected to work with patients presenting with psychosomatic disorders, as well as a variety of psychological issues related to physical disorders.

KEY RESULT AREAS:

The post-holder will:

1. Provide a consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases.
2. Promote choice and hopefulness and ensure a recovery orientated approach.
3. Promote and initiate any actions to safeguard vulnerable children and adults.
4. Under supervision, be responsible for the assessment of care needs of patients presenting with a wide variety of clinical conditions out of normal working hours. Following this, the post-holder will be responsible for planning and referring on for appropriate follow up where necessary.
5. Provide a consultation-liaison service to the ED and individual clinicians, particularly in assisting them develop emergency plans of care and make difficult decisions in cases where patients are exhibiting complex psychosocial needs and/or challenging behaviours.
6. Be expected to participate fully in quality assurance initiatives, with particular emphasis on a 'patient first' approach when required.
7. The post-holder will act as a role model in providing a service for people with mental health problems for all staff within the ED/main hospital.
8. The post-holder will participate in audit/research projects associated with the work of the service when required.
9. The post holder represents Mental Health Trust in the general hospital setting and will play a vital role in communicating and maintaining a good working relationship between the Trusts.

JD:

To work as a member of a multi-disciplinary Community Recovery Service (CRS) to provide community psychiatric nursing care to people with severe mental health needs in the context of the care programme approach in secondary care. As a qualified nurse, the post holder will carry a caseload provide care co-ordination, implement and evaluation of care for people with mental health problems in the community. They will work in partnership with the multi-disciplinary team and contribute to the continuing development of the service.

The post holder will report directly to the Team Manager and will work in partnership with members of the multi-disciplinary team consisting of community psychiatric nurses, Occupational Therapists, Nursing Assistants and the wider medical team. Personal Advisors support care leavers to develop the skills needed to live independently and to be aspirational in identifying and achieving their goals, including healthy living, stable homes and good educational opportunities. Transitioning into adulthood is a challenging time for any young people. This is especially so for our Care Leavers who have often experienced trauma and may have complex needs including mental health, substance use and offending behaviours.
