Perioperative Service Clinical Manager - RN – Perioperative Services - Days
As a Perioperative Service Clinical Manager, you will drive efficiency and compassion as well as directly impact the quality of care in all areas of Periop. Be a part of inspirational and collaborative work amongst your nursing colleagues and leaders. Support the delivery of exceptional care, coordinate patient care throughout, in addition to being an agent of health and hope. You’ll regularly participate in helping to gather staff perspectives to help positively impact the units by mentoring and motivating staff. Partner closely with nursing team leaders on quality measures, guide independent thinking, and patient-focused compassion.
This is a full time position working four 10-hour days a week starting at 7:00am at NewYork-Presbyterian/Allen Hospital.
Preferred Criteria

Experience in all areas of Perioperative Services, especially the operating room

Job Description:
The Nurse Care Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs. This position educates patients and families to help them manage their health care needs. The incumbent facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.Shift Details: 8:00am - 4:30pm Monday through Friday with a small weekend commitment of two weekends every six weeks.
Scope
As a Case Manager RN you need to know how to:
Coordinate the care and services for patients identified as needing assistance or meeting Care Management criteria and develop a plan to meet those needs.
Work with patient and family/caregivers to determine specific goals and actions based on assessment.
Assess the patient’s prior level of functioning, access to and/or use of community resources and available support systems.
Identify appropriate admission and continued stay issues. Help to identify strategies for reducing length of stay and appropriate utilization of services. Utilize criteria including clinical pathway data to monitor efficiency and evaluate outcomes.
Attend, facilitate and participate in rounds and case conferences. Collaborate with Physicians and other members of the health care team on the patient’s behalf. Coordinate discharge planning.
Requirements:
Graduate of an accredited school of nursing
Associates Degree in Nursing (ADN/ASN) Current Colorado RN license or compact license with the ability to obtain Colorado RN license
Minimum of three (3) years of experience as a Registered Nurse in an acute care setting

At least five (5) years of nursing experience

Participates and communicates in scheduled case care conferences with the health care team. Responsible for oversight of patient case load, visit frequencies, orders, insurance authorizations, and overall coordination of care for assigned patient. Monitoring delivery of services by the other team members, directing care toward the achievement of outcomes. Adheres to productivity standards set forth by the agency. Schedules patient visits weekly and communicates schedule plan to the scheduling team and communicates any changes to the plan. Develops, implements, and communicates discharge plan in conjunction with the patient, caregivers, health care team, and physicians.
Proficient and competent in completing assessment and required OASIS documentation. Completes all required documentation timely, per agency policy, procedure and requirements. Demonstrates an understanding of documentation and assessment to quality outcomes and reported measures. Demonstrates financial stewardship and complies with agency budget. Uses agency equipment and supplies as required with cost effectiveness in mind.
Attends and participates in staff meetings. Participates and contributes to Quality Assurance and Performance Improvement projects. Demonstrates behaviors that are in line with Mercy Health mission, values, and standards of performance. Acts as a collaborative team member and promotes teamwork across all disciplines. Required Minimum Education: 2 Year/ Associates Degree - Nursing
Preferred Education : 4 year/ Bachelors Degree - Nursing
Licensure/Certification Required - RN currently licensed to practice nursing in the State of Ohio. Certified in basic CPR. Valid driver's license with a driving record that meets BSMH insurability requirements.

Job Description:
The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. Job Essentials
Note: The Certified Social Worker Intern License can be utilized for up to 24 weeks (6 months) while employees are working toward preparing, testing, and passing the State Boards examination (obtaining an CSW license). After that 24 week period, or if the CSW Graduate fails the State Board exam a second time, the employee can no longer remain in the CSW-I License. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e. SSW, psych tec, etc.). Failure to either pass boards or to be hired into another job will result in termination.
