Governance: To ensure the service complies with Trust policies, Standing Orders, Standing Financial Instructions and relevant legislation. Minimum Requirements

Social Worker:

• Master’s degree in social work from and accredited program is required.
• Current valid LCSW license required.
• Social Worker in a community or acute care setting or three (3) years’ experience as a Care Manager in an acute care setting.

• At least five (5) years of care management in an acute care setting
• Care Management Certification is preferred.

Essential Job Functions

The Case Manager works with minimal supervision to assess, plan, implement, and evaluate patient care.  Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, decision making, priority setting, work delegation and work organization.  The Case Manager supports agency policies, procedures, protocols, and standards of nursing practice recognizing each person as an individual.  The Case Manager promotes positive patient experiences, demonstrates the agency’s core values, and exemplifies the agency’s philosophy in daily work activities, promotes and maintains effective relationships and communication in all professional endeavors. Effectively communicates with clinical managers and members of the care delivery team.  Provides oversight of LPN and HHA if applicable and completes required supervisory visits.

The case manager develops, establishes, implements and evaluates an individualized plan of care for patients that corresponds with physicians’ orders. Performs assessments of patient's needs, condition, environment, and consults with other health team members and updates the plan of care as necessary.  Maintains ongoing responsibility for assigned case load. Provides teaching to patients, families, and caregivers.  Participates and communicates in scheduled case care conferences with the health care team.  Responsible for oversight of patient case load, visit frequencies, orders, insurance authorizations, and overall coordination of care for assigned patient. Monitoring delivery of services by the other team members, directing care toward the achievement of outcomes.  Adheres to productivity standards set forth by the agency.  Schedules patient visits weekly and communicates schedule plan to the scheduling team and communicates any changes to the plan. Develops, implements, and communicates discharge plan in conjunction with the patient, caregivers, health care team, and physicians.

Proficient and competent in completing assessment and required OASIS documentation. Completes all required documentation timely, per agency policy, procedure and requirements. Demonstrates an understanding of documentation and assessment to quality outcomes and reported measures.  Demonstrates financial stewardship and complies with agency budget.  Uses agency equipment and supplies as required with cost effectiveness in mind.

The Financial Center Manager I will be responsible for the achievement of specified banking center and banking center staff performance goals, on-going achievement, profitability, deposit and loan growth, and fee income. The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. Note: The Certified Social Worker Intern License can be utilized for up to 24 weeks (6 months) while employees are working toward preparing, testing, and passing the State Boards examination (obtaining an CSW license). After that 24 week period, or if the CSW Graduate fails the State Board exam a second time, the employee can no longer remain in the CSW-I License. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e. SSW, psych tec, etc.). Failure to either pass boards or to be hired into another job will result in termination.

Minimum Qualifications

• Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.
• Current State Licensure as a Certified Social Worker or LCSW.
• Computer skills (email, word processing, and spreadsheets).
