The role of pharmacy within General Practice is part of an exciting programme of transformationwhich is underpinned by the NHS Long Term Plan. This is an innovative role to work within the Lakeside Healthcare Pharmacy team to ensure safeand effective use of medicines for patients within Lakeside Healthcare. As a starting point the role will initially focus on medicines reconciliation from clinic and dischargeletters, and will develop further as the role of the Pharmacy team expands within theorganisation. DUTIES & AREAS OF RESPONSIBILITY Job Responsibilities/Principal Duties to include: Medicines reconciliation from clinic and discharge letters Take a central role in the clinical aspects of shared care protocols Level 1 medication reviews to include: o Identifying patients on repeat medication who have not had a medication review o Check medical notes to assess if any monitoring is required and arrange as appropriate o Update medication as appropriate following defined protocols o Remove any old or unnecessary medication from repeat o Update quantities to provide appropriate supply each month o Review dosage instructions to ensure concise and clear instructions Ensure prescribing is in-line with the Lakeside Drug Formulary whilst taking consideration of CCG prescribing guidance. Participate in practice meetings, and other meetings to improve engagement of the role ofpharmacy technician within the Lakeside Pharmacy model and to promote issues relevant toprescribing. Undertake training and development, to ensure the required knowledge and skills forpractice-based work. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Assist in the delivery of medicines optimisation incentive schemes and audits relating toMHRA alerts, drug monitoring and other patient safety initiatives. Develop relationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team across health and socialcare including primary care, community pharmacy, secondary care, and mental health. Training and personal development: You will participate in any training and development programmes implemented by Lakeside Healthcare. This could include: Participation in individual performance reviews, including maintaining a record of your own personal and professional development. Working with the Senior Pharmacist and the Management team to assess own learning needs and undertaken any training as appropriate. An exciting opportunity has arisen to recruit for Complex Health Coordinators for a project (Horizontal Project) that is being undertaken within the South Peterborough Primary Care Network (SPPCN). The aim of the Horizontal Project is to educate and to provide support to specific groups of patients who have been identified as those with complex health needs who could benefit from some additional input from Primary care to improve their quality of life. The successful candidate will be responsible for conducting well-being and personalised care plan assessments for cohorts of patients with specific health conditions, in a variety of settings, using effective communication skills to support individuals with complex conditions to identify their next health and well-being steps, including referrals to appropriate service providers (both clinical and social). They will require strong motivation and dedication to facilitate both individual and group work interventions to make health and well-being changes, seeking clinical advice when indicated. They will need to be able to use clinical judgement to evaluate individuals difficulties and progress, including the recording and analysis of objective and subjective measurements. • Understand and evaluate the project requirements and project plans particularly, materials, schedule, equipment and labour loading.
• Assist the Senior Engineer/construction superintendent in assigning tasks to work teams.
• To explain the site situation, technical requirements, project specifications, special engineering requirements, coordination procedures, execution approach and activities priorities to the various work teams to seek contributions for further refinement.
• Ensure that various disciplines (including quality and safety personnel) are carrying out each tasks and tests/inspection in accordance with specification and standards as per the contractual requirements.
• Assist in resolving engineering issues at site, coordinating with the respective engineering support units and referring to the Construction Superintendent for guidance, if necessary report discrepancy (if any) on labour status, equipment, materials etc. to the Senior Project Engineer.
• Prepare the necessary documents for any additional works of V/O’s, F.I. etc.
• To update the project portal regularly.
• Ensure, monitor and maintain site billing and invoicing comprehensive process flow/procedure for client and subcontractors/APs.
• Together with the Assistant Engineer prepare and submit the monthly progress billing to clients
• Carry out other duties as and when assigned by immediate superior or higher management.
• Preparation of project close-out report for the Senior Project Engineer/Project Manager to review before submission to clients.
• Ensure final acceptance of the project by client in accordance with contractual requirements

Responsibilities

· Responsible for meeting daily service and repair needs and driving customer satisfaction.

· Installs, repairs and maintains equipment in the field; provides customer training as required.

· Documents all inspections, maintenance, repair work and submits paperwork in a timely basis.

· Participates in Service sales opportunities and assists with promoting and implementing revenue programs.

· Ensures that tools and test equipment are properly maintained and calibrated.

· Assesses product/equipment performance based on field support data; recommends modifications or improvements.

· Performs problem diagnosis, service, and preventative maintenance on instruments for lab automation.

· Handles essential tasks as assigned, and responds to change productively

Requirements

Prefer following disciplines – Electrical & Electronics, Mechanical, Mechatronic, or related disciplines.

· Candidate must be diligent and possess good communication skills - oral and written - to be able to interact with customers and suppliers effectively.

· Able to be a team player.

· Have strong organizational skills and strong follow-up skills

· Able to complete all tasks assigned in a timely fashion

· Possess a high level of commitment and initiative.
