Tibor Rubin - History: Background about the Veteran who VA Long Beach Medical Center is named after.
The RN is accountable to the Patient Safety Manager, Quality, Safety, and Value Service and is responsible for working closely with the service, nurse managers, front-line staff, leadership, and external vendors to improve current processes and implement new process to reduce falls throughout the organization.
The Falls Coordinator responsibilities also include, but not limited to:
- Receiving all Joint Patient Safety Reporting (JPSR) falls reports to add findings, recommendations, and actions.
- Investigating each fall with supervisor and frontline staff to track and trend data provide recommendations to prevent future falls.
- Ensuring appropriate documentation is completed in the patient chart post-fall.
- Completing annual Patient Safety Assessment Tool (PSAT) required by the National Center for Patient Safety (NCPS).
- Completing thorough facility wide chart reviews and tracers for at risk patients and departments.
- Chairing the monthly interdisciplinary Falls Prevention Committee.
- Working in collaboration with the Safe Patient Handling and Mobility Coordinator.
- Participating in all Root Cause Analyses (RCAs) related to patient falls in collaboration with Patient Safety.
- Evaluating the compliance with Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), Office of Inspector General (OIG), and Ascellon and implementation of such regulation.
- Eliciting support, and networking with interdisciplinary personnel, falls program experts/resources outside the facility and the VA Safety Center and equipment vendors.
- Serving as a backup for the Safe Patient Handling and Mobility Coordinator.
